[Atrio-ventricular nodal reentrant tachycardia--concepts evolution on the propagation of the reentrant wave front].

Atrio-ventricular nodal reentrant tachycardia (AVNRT) is the most common narrow QRS tachycardia. The arrhythmia affects usually patients without any other structural heart disease. AVNRT may be the cause of the broad spectrum of symptoms, ranging from palpitations to clinical signs of impaired coronary and cerebral perfusion. Mechanism responsible for AVNRT is reentry, which involves right and some part of the left atrium. Employed in the early 80's the technique of radiofrequency current (RF) ablation (which allows removal of arrhythmic substrate by modification or destruction of the slow pat

[Clinical thinking and decision making in practice. A patient with postpartum malaise, anorexia, and ventricular tachycardia].

A 34-year-old woman was hospitalised with hypotension, lethargy and ventricular tachycardia, five months after an uneventful pregnancy and delivery. Laboratory values were compatible with anterior (abnormal concentrations of adrenocorticotropin hormone, thyroid stimulating hormone, prolactin) and posterior (diabetes insipidus) pituitary gland insufficiency. An MRI scan revealed that there was no pituitary enlargement and that the pituitary stalk was normal. The prompt recognition of hypophysitis as a potentially fatal condition is important, as a life-saving treatment is available.

Subthreshold stimulation in three types of reentrant supraventricular tachycardia: correlation with the results of catheter ablation.

The effects of subthreshold stimulation (STS) by direct current were investigated in 20 patients with atrioventricular nodal reentrant tachycardia (AVNRT), 27 with atrioventricular reentrant tachycardia (AVRT) and 3 with idiopathic atrial reentrant tachycardia (IART) STS was delivered to each eligible site for ablation prior to radiofrequency application. STS was defined as 'positive' if it could terminate the tachycardia or disrupt the conduction of accessory pathways without myocardial capture and defined as 'negative' if it could not. Radiofrequency ablation was performed irrespective of a

Inducibility of intra-atrial reentrant tachycardia after the first two stages of the Fontan sequence.

OBJECTIVES: We sought to examine the incidence and possible factors for inducible intra-atrial reentrant tachycardia (IART) in a group of patients after two stages of the Fontan sequence but before the operation. BACKGROUND: Intra-atrial reentrant tachycardia occurs in 10% to 40% of patients after the Fontan operation. No data are available regarding the potential for IART after the first two stages of the Fontan sequence but before the operation. METHODS: The IART induction protocol included programmed extrastimulation and rapid atrial pacing, with and without isoproterenol. RESULTS: The m

Radiofrequency catheter ablation from the left sinus of Valsalva in a patient with idiopathic ventricular tachycardia.

We report the case of a 54-year-old woman with idiopathic VT originating in the left ventricular outflow tract. She initially presented with palpitations and light-headedness. The morphology of the PVCs exhibited an inferior axis and tall R waves were noted in all the precordial leads. Spontaneous PVCs were transiently terminated by an intravenous injection of adenosine triphosphate. Radiofrequency catheter ablation from the left sinus of Valsalva successfully abolished the PVCs and the VT.

Radiofrequency catheter ablation of tachycardia in children with and without congenital heart disease: indications and limitations.

From 1993 to 1998, a total of 100 consecutive pediatric patients with tachycardia (45 male and 55 female, aged 1 year 10 months to 17 years, 11+/-4 year) who underwent electrophysiological study were reviewed. Eleven of them were younger than 5 years. Two had tachycardia-related cerebrovascular accident. Congenital heart disease was found in 12 patients. After propofol anesthesia, the clinical tachycardia could not be induced in three (two atrial tachycardia and one AV nodal re-entrant tachycardia) and became nonsustained in five (atrial tachycardia). Mechanical ablation occurred in three and

Mechanical function and substrate oxidation in the neonatal pig heart subjected to pacing-induced tachycardia.

Isolated, paced, isovolumically beating, neonatal pig ( approximately 2 days) hearts were perfused with a crystalloid solution during four periods: (1) baseline, HR 150 bpm; (2) HR-response curves, HR 150-360 bpm; (3) tachycardia, HR 300 bpm; and (4) posttachycardia, HR 150 bpm. Group I was studied with glucose (5. 5 mM) as the sole substrate. During baseline, left ventricular peak systolic pressure (PSP) averaged 123 +/- 7 mm Hg; end diastolic pressure (EDP), 4.9 +/- 0.4 mm Hg; relaxation time constant (Tau), 29.5 +/- 3.9 ms; glucose oxidation (14CO2 from [14C]glucose), 1535 +/- 96 nmol/min/g

Feasibility and safety of a minimally invasive approach to catheter ablation for atrioventricular nodal reentrant tachycardia.

This study assessed the feasibility and safety of a minimally invasive approach to catheter ablation in 72 consecutive patients with AV nodal reentrant tachycardia. A 3-catheter approach was used in the first 19 patients. In the other 53 patients, a 2-catheter approach was employed. Ablation was successful in all patients after a mean of 3 +/- 3 RF applications. Procedure and fluoroscopy times were 62 +/- 20 mins and 8 +/- 5 mins respectively. Slow pathway was ablated in 43 patients (60%). Transient AV block occurred in 6 patients; there was no permanent AV block. These results suggest that it

Limitations of tachycardia confirmation and rate classification algorithms in a third-generation implantable cardioverter defibrillator.

Newer ICDs provide antitachycardia (ATP) and bradycardia pacing and cardioversion and defibrillation shocks based on sensed interval criteria. The objectives of this investigation were to determine the algorithm related errors in tachycardia confirmation and rate classification that occurred in patients with a third-generation, noncommitted, tiered ICD therapy. Forty-three consecutive patients with the Guardian ATP 4210 ICD, which uses an X out of Y sensed interval counting algorithm for tachycardia detection, confirmation, and classification were studied. Paroxysmal atrial tachycardia recorded by Holter monitoring during an episode of fear. A case report.

A twenty-four-year-old, white, athletic woman, free of heart disease, experienced an episode of fear when she was assaulted in the street without physical injury while under-going twenty-four-hour Holter monitoring. She developed an important sympathetic response in which, besides the symptoms characterized by palpitations, chest pain, dyspnea, asthenia, dizziness, nausea, and profuse cold sweating, she had an episode of paroxysmal atrial tachycardia. The causes and mechanism of this not well-documented event in humans are discussed.

Clinical spectrum, therapeutic management, and follow-up of ventricular tachycardia in infants and young children.

We reviewed 40 infants and young children with VT. Median maximum VT rate was 214 beats/min (range 152 to 375 beats/min). A cause was defined in 20 (50%), the most common being cardiomyopathy or myocarditis in 8 (20%). There were six deaths (15%) related to VT, three of which occurred at diagnosis and in patients less than 1 week old. In 5 of 6 deaths related to VT, a cause was defined. At follow-up, 31 (91%) of 34 survivors did not have VT. The presence of symptoms was a predictor of death related to VT. The outlook for asymptomatic patients and those who survived more than 6 months after dia

Development of ventricular fibrillation after intravenous calcium chloride administration in a patient with supraventricular tachycardia.

The i.v. administration of calcium before or shortly after treatment of supraventricular tachycardia with verapamil has been suggested to counteract a hypotensive response to verapamil. We discuss the case of a patient who presented to the emergency department with an accelerated wide-complex tachycardia and minimal symptoms. Immediately after i.v. administration, of 1 g calcium chloride as pretreatment for verapamil administration, ventricular fibrillation developed. Emergency physicians should be aware of potential dangers after the administration of i.v. calcium preparations when trying to

[Supraventricular tachycardia in a pregnant woman with tertiary malaria; a medical experience from the Indonesian period].

A 22-year-old Indonesian woman became ill with fever and chills in the 7th month of her fourth pregnancy. Two days later she complained of palpitations. At admission again two days later, the temperature was 40 degrees C. The heart rate was 175/min. A blood slide contained trophozoites of Plasmodium vivax. The ECG showed an atrioventricular junctional tachycardia. Treatment consisted of 2 intramuscular injections of 0.5 g quinine hydrochloride administered with an interval of 4 hours. The following morning the temperature was normal and the heart rate had fallen to 100/min. The ECG showed a si

Successful treatment of supraventricular tachycardia-induced cardiomyopathy with amiodarone: case report and review of literature.

Permanent junctional reciprocating tachycardia (PJRT) is a rare cause of supraventricular tachycardia in the pediatric population and is resistant to most pharmacological therapy. A case of supraventricular tachycardia that, on the basis of postnatal electrocardiographic and Holter monitor evidence, was diagnosed as PJRT and presented in utero as an atypical tachycardia with severe tachycardia-induced cardiomyopathy confirmed postnatally is presented. It is the only case of which the authors are aware that was controlled in the neonatal period by amiodarone and that resulted in complete resolu

Cure of interfascicular reentrant ventricular tachycardia by ablation of the anterior fascicle of the left bundle branch.

INTRODUCTION: Fascicular reentrant ventricular tachycardia (VT) using the anterior fascicle of the left bundle anterogradely is rare and may produce identical QRS morphology during sinus rhythm and VT. Catheter ablation of this type of VT has not been described in detail. METHODS AND RESULTS: In a postinfarct patient with dilated left ventricle and recurrent VT (showing a QRS configuration of right bundle branch, left posterior fascicular block), endocardial recordings from the His-Purkinje system showed that VT was due to interfascicular reentry. Induction of VT occurred after progressive re

[Tachycardia with wide QRS. Limitations of electrocardiographic criteria in patients treated with anti-arrhythmia agents].

The diagnosis of ventricular tachycardia is difficult, often requiring endocavitary investigation to obtain proof. Many authors have attempted to define criteria used to distinguish between ventricular tachycardia and supraventricular tachycardia with wide QRS in the surface electrocardiogram. These criteria have been determined in patients not taking antiarrhythmic drugs. The authors report the case of a patient treated with flecainide for atrial fibrillation and hospitalised because of faints and tachycardia with wide complexes. The diagnosis of ventricular tachycardia was justified by surfa
