The impact of an intervention by a mobile geriatrics team including a pharmacist on potentially inappropriate drug prescription: results of the PharMoG study.

BACKGROUND: Polypharmacy and pharmacokinetic alterations expose elderly patients to an increased risk of iatrogenic events, hospitalisation, and healthcare costs. Therapeutic optimisation is based on identifying potentially inappropriate drug prescriptions (PIDPs) using an explicit approach based on standardised criteria such as STOPP & START and/or an implicit approach. Mobile Geriatric Teams (hereinafter MGT) and clinical pharmacists work together to reduce PIDPs, but pharmacists are not always part of these teams. The PharMoG study included a pharmacist in the MGT (PharMoG team) from a Fren

Urinary tract infections at an Australian sexual health service: bacterial etiologies, antibiotic susceptibilities, and antimicrobial prescribing patterns.

Urinary tract infections (UTIs) are one of the most common infections. Management of UTIs often involves empiric antibiotics, which can contribute to antibiotic overuse. We aimed to investigate the causative organisms, prevalence of antimicrobial resistance, and empiric prescribing practices to improve antibiotic use for UTIs. We conducted a retrospective audit of UTIs in premenopausal nonpregnant persons with a vagina aged 18-51 years attending the Melbourne Sexual Health Centre from 2 January 2018 to 24 January 2023. We calculated the proportion of culture samples with pathogen(s) isolated,

Role of serum amyloid A in Mycoplasma pneumoniae pneumonia and related respiratory diseases.

Serum amyloid A (SAA) is an acute-phase protein that can increase in plasma concentration by up to 1000-fold during infection and inflammation. Clinical studies have identified significantly elevated SAA levels in Mycoplasma pneumoniae pneumonia (MPP), correlating with disease severity, lung injury, and prognosis. Emerging evidence indicates that SAA is not merely a passive inflammatory biomarker but an active contributor to MPP pathogenesis, which engages receptors such as formyl peptide receptor 2 (FPR2), Toll-like receptor 2 (TLR2), Toll-like receptor 4 (TLR4), and the receptor for advanced

Restoring circadian disrupted gut microbial metabolite rhythms with phytochemicals: a new avenue against metabolic disease.

The global epidemic of metabolic diseases-encompassing obesity, type 2 diabetes mellitus (T2DM), non-alcoholic fatty liver disease (NAFLD), and cardiovascular disease-represents a defining public health challenge of our era. The traditional model of simple caloric excess vs. expenditure has proven insufficient, giving way to a paradigm that acknowledges complex interactions between genetics, environment, and lifestyle, mediated by intricate physiological systems. Among these, the host circadian timing system and the gut microbiome have ascended as pivotal, deeply intertwined regulators of meta

Efficacy of azithromycin sequential therapy combined with montelukast in the treatment of pediatric mycoplasma pneumonia: A meta-analysis.

BACKGROUND: Mycoplasma pneumoniae pneumonia (MPP) stands as a predominant cause of community-acquired respiratory infections in children. With the increasing prevalence of macrolide-resistant Mycoplasma pneumoniae strains, the therapeutic efficacy of azithromycin monotherapy has become a growing concern in clinical practice. Montelukast sodium, a selective leukotriene receptor antagonist, has emerged as a promising adjunctive agent, as it can effectively alleviate airway inflammation and reduce airway hyperresponsiveness. This meta-analysis was designed to systematically evaluate the efficacy

Biobank-scale characterization of Alzheimer's disease and related dementias identifies potential disease-causing variants, risk factors, and genetic modifiers across diverse ancestries.

Alzheimer's disease and related dementias (AD/ADRDs) pose a significant global public health challenge, underscored by the intricate interplay of genetic and environmental factors that differ across ancestries. To effectively implement equitable, personalized therapeutic interventions on a global scale, it is essential to identify disease-causing mutations and genetic risk and resilience factors across diverse ancestral backgrounds. Exploring genetic-phenotypic correlations across the globe enhances the generalizability of research findings, contributing to a more inclusive and universal under

Targeting super enhancers for liver disease: a review.

BackgroundSuper enhancers (SEs) refer to the ultralong regions of a gene accompanied by multiple transcription factors and cofactors and strongly drive the expression of cell-type-related genes. Recent studies have demonstrated that SEs play crucial roles in regulating gene expression related to cell cycle progression and transcription. Aberrant activation of SEs is closely related to the occurrence and development of liver disease. Liver disease, especially liver failure and hepatocellular carcinoma (HCC), constitutes a major class of diseases that seriously endanger human health. Currently,

The Role of the Oral Microbiota Related to Periodontal Diseases in Anxiety, Mood and Trauma- and Stress-Related Disorders.

The prevalence of anxiety, mood and trauma- and stress-related disorders are on the rise; however, efforts to develop new and effective treatment strategies have had limited success. To identify novel therapeutic targets, a comprehensive understanding of the disease etiology is needed, especially in the context of the holobiont, i.e., the superorganism consisting of a human and its microbiotas. Much emphasis has been placed on the role of the gut microbiota in the development, exacerbation, and persistence of psychiatric disorders; however, data for the oral microbiota are limited. The oral ca

Ileal Bile Acid Transporter Inhibitor Improves Hepatic Steatosis by Ameliorating Gut Microbiota Dysbiosis in NAFLD Model Mice.

Nonalcoholic fatty liver disease (NAFLD), characterized by excessive fat deposition in the liver unrelated to alcohol consumption, is highly prevalent worldwide. However, effective therapeutic agents approved for NAFLD treatment are lacking. An ileal bile acid transporter inhibitor (IBATi), which represents a new mode of treatment of chronic idiopathic constipation, leads to increased delivery of bile acids to the colon. We investigated the effect of IBATi against NAFLD through modification of the gut microbiota in mice. IBATi treatment significantly suppressed body weight gain, liver dysfunct

Connected device and therapeutic patient education to promote physical activity among women with localised breast cancer (DISCO trial): protocol for a multicentre 2×2 factorial randomised controlled trial.

INTRODUCTION: Despite safety and benefits of physical activity during treatment of localised breast cancer, successful exercise strategies remain to be determined. The primary objective of the 'dispositif connecté', that is, connected device in English trial is to evaluate the efficacy of two 6-month exercise interventions, either single or combined, concomitant to adjuvant treatments, on the physical activity level of patients with breast cancer, compared with usual care: an exercise programme using a connected device (activity tracker, smartphone application, website) and a therapeutic patie

Intranasal Lipid Particulate Drug Delivery Systems: An Update on Clinical Challenges and Biodistribution Studies of Cerebroactive Drugs in Alzheimer's disease.

BACKGROUND: Alzheimer's is the primary cause of death in the various countries that affect wide strata of the population. The treatment of it is restricted to a few conventional oral medications that act only superficially. It is evident that the delivery of a drug to the brain across the blood-brain barrier is challenging as the BBB is armed with several efflux transporters like the P-glycoprotein as well as nasal mucociliary clearance adds up leading to decreased concentration and reduced therapeutic efficacy. Considering these, the intranasal IN route of drug administration is emerging as a

[New insights into adipose cell biology].

Our research focuses on the molecular mechanisms controlling glycemia in healthy and diabetic individuals. Diabetes is now considered as a worldwide epidemic by WHO, and is predicted to affect several hundred million people in the near future. Current therapies have failed to prevent or control hyperglycemia, as well as the deleterious cardiovascular consequences of the disease have increased. New paradigms are thus needed to develop novel therapeutic strategies. Over the last 15 years, we have been studying the intestine as a major regulator of the integrated cross-talk between the brain, liv

Reconstruction of a resilient and secure community and medical care system in the coronavirus era - English translation of the Japanese opinion released from the Science Council of Japan.

BACKGROUND: Over 3 years have passed since the outbreak of novel coronavirus disease 2019 (COVID-19), a disease associated with a high risk of severe illness and death among older individuals. This period has brought to light regional and social issues, including issues in overall and regional healthcare, that existed before the epidemic. "COVID-19-related frailty" is defined as secondary damage to health caused by inactivity and disconnection from human interaction owing to prolonged isolation among older individuals. Now in its fourth year, COVID-19 cannot be taken lightly, even though it is

Exploring Concepts of Understanding in the Working Practice of Speech and Language Therapists in the UK: A Preliminary Survey.

BACKGROUND: All communication involves the expression and the reception of information, both of which are considered by speech and language therapists (SLTs) when assessing and planning intervention for language difficulties. Comprehension, or understanding, has to be inferred, whereas expressive behaviours are directly observable. Possibly because of the level of inference, views and assessments of a person's understanding often vary: between different practitioners, professionals, family members and carers. The aim of the research was to examine how and why such different perspectives occur

Peripheral and central immune features in insomnia: Integrative bulk and single-cell transcriptomic analyses reveal core hub genes and candidate compounds.

Insomnia is closely associated with immune dysregulation, yet the overall pattern of peripheral-central immune disequilibrium and its underlying molecular basis remains incompletely understood. To characterize the peripheral-central immune features associated with insomnia, identify key immune cell populations and core molecular programs, and prioritize candidate therapeutic compounds with preliminary experimental validation. Peripheral blood bulk transcriptomic dataset GSE208668 was analyzed using differential expression analysis, weighted gene co-expression network analysis (WGCNA), and func

Delivery of person-centred virtual care for middle-aged and older adults: A mixed methods systematic review of patient and healthcare professional perceptions.

OBJECTIVES: Virtual care is a core component of health systems worldwide. However, the operationalisation of person-centred care (PCC) in virtual settings remains fragmented across populations and service contexts. Middle-aged (45-64 years) and older adults (≥ 65 years) may face digital, cognitive, and contextual barriers that influence engagement with virtual services. This mixed methods systematic review examined how PCC can be delivered through virtual consultations for this population. METHODS: MEDLINE, Embase, CINAHL, and PsycINFO were searched from inception to June 2024, supplemented b

EmotionaL and Language Recovery in Aphasia (ELLA): Codesigning and Proof-of-Concept Testing a Novel Intervention.

PURPOSE: People with aphasia are at risk of becoming depressed, anxious, and isolated. They want therapy to focus on improving communication, confidence, and finding ways to live well; family members want to be involved in the therapy process. We aimed to codesign and test a novel intervention (EmotionaL and Language recovery in Aphasia [ELLA]) that would address these priorities through integrating two evidence-based therapy approaches: elaborated semantic feature analysis (SFA), a naming therapy, and solution-focused brief therapy (SFBT), a psychological therapy. METHOD: A series of worksho

The microbiota-gut-brain axis perspective: mechanisms and intervention strategies for the comorbidity of chronic constipation and depression.

BACKGROUND: Chronic constipation and depression are highly prevalent worldwide. These two conditions frequently co-occur in clinical practice. Accumulating evidence indicates that gut microbiota dysbiosis mediates this comorbidity through the microbiota-gut-brain axis (MGBA). METHODS: This narrative review systematically summarizes current research on MGBA bidirectional communication mechanisms, gut microbiota alterations in comorbid patients, and microbiota-targeted intervention strategies. RESULTS: The MGBA facilitates bidirectional communication through four major pathways: neural pathway

Digital reminiscence therapy in dementia care: a systematic review and meta-analysis.

BACKGROUND: Dementia is a major neurodegenerative condition with limited treatment options, making non-pharmacological interventions increasingly important. Reminiscence therapy (RT) has shown benefits in dementia care, and its digital formats - delivered via virtual reality, mobile applications, or multimedia - may offer greater personalization, accessibility, and flexibility in clinical practice. However, evidence of their effectiveness remains fragmented and requires systematic synthesis. METHODS: We conducted a systematic review and meta-analysis to evaluate the efficacy of digital RT in d
