We describe two cases of a bronchopleural fistula developing in a calcified fibrothorax several decades after the initial pleurisy. In a follow-up lasting many years, the fistulae are persistent, with only minor symptoms. We suggest that this benign course without serious illness is due to the fact that the calcified shell effectively seals off the infection, and that a conservative approach is likely to be successful.

BACKGROUND: Bilateral renal cell cancer (RCC) is an imperative indication of nephron sparing surgery (NSS). In the present study, we examined a new modality for NSS; intraoperative ultrasonography, argon beam coagulator (ABC) and fibrin glue.
PATIENTS AND METHODS: We performed NSS against 7 kidneys in 5 patients, including 3 with synchronous and 2 with asynchronous tumors. Radical nephrectomy was performed on the contralateral kidney in 2 patients including one with asynchronous and the other with synchronous RCC.

We treated a patient with a mechanical ileus resulting from encasement of a loop of the small intestine by the great omentum, caused by abnormal adhesion. Apart from its occurrence postoperatively with laparotomy, strangulation of small intestine into the omentum is very rare, and its occurrence without underlying disease, except in children, has not been documented previously.

We conclude that untreated women with FMF have up to a 30% incidence of infertility due to ovulatory dysfunction and peritoneal adhesions. Some of these women conceive during ovulation induction with or without insemination. We have described the first case of a successful and normal pregnancy in a patient with FMF following in vitro fertilization (IVF) while on prophylaxis colchicine therapy after other treatments for infertility were unsuccessful.

A chronic tuboovarian abscess with vaginal fistula occurring after vaginal hysterectomy was removed laparoscopically. The fistula was closed by a combined laparoscopic-vaginal procedure. A complete cure was achieved without complications.

We used gelatin-resorcin-form-aldehyde (GRF) glue to fuse the false lumen of type A acute aortic dissection in four patients. All were operated on within 3-24 hours after onset, and gluing of the two cylinders of the dissecting aorta could be done safely in a short time. Perforation of the gallbladder with spillage of stones into the peritoneal cavity occurs frequently during laparoscopic cholecystectomy. The diagnostic laparoscopy revealed ascending multifocal affections of the chlamydial origin in the organs of the small pelvis. Between January 1987 and May 1990, forty-four women were treated in our department by laparoscopic surgery for distal tubal pathology. Among them, 10 had an intrauterine pregnancy (22.7%) and 2 had an ectopic pregnancy (4.5%) within 28 months after surgery. Subsequently, our results were expressed in terms of the type of operation performed (fimbrioplasty or neosalpingostomy) and according to the tubal and adhesion scores (Mage and Bruhat). We found that most pregnancies were obtained within the first 16 months, that fimbrioplasty gave better results than neosalpingostomy (35.7% versus 16.6%)

Transcervical fallopian tube catheterization is rapidly gaining favor as a minimally invasive diagnostic and therapeutic technique. On occasion, the presence of filmy adhesions not identified on HSG obstruct the passage of the cannula to the cornual angle.
