Sparser macula microvasculature in neuromyelitis optica spectrum disorder occurs independently of optic neuritis.

PURPOSE: To evaluate the macula microvascular perfusion in neuromyelitis optica spectrum disorder (NMOSD) patients and assess the correlation with their clinical features. METHODS: 35 aquaporin-4 seropositive NMOSD patients (38 NMOSD eyes without optic neuritis, NMOSD-NON, and 32 NMOSD eyes with optic neuritis) and 35 healthy controls (HC) were included in our study. Swept-source optical coherence tomography angiography (SS-OCTA) was used to image and segment the macula microvasculature into the inner macula vascular complex (IVC), superficial vascular plexus (SVC), and deep vascular plexus (

Pathological Confirmation of Optic Neuropathy in Familial Dysautonomia.

Clinical data suggest that optic neuropathy and retinal ganglion cell loss are the main cause of visual decline in patients with familial dysautonomia, but this has not previously been confirmed by pathological analyses. We studied retinas and optic nerves in 6 eyes from 3 affected patients obtained at autopsy. Analyses included routine neurohistology and immunohistochemistry for neurofilaments, cytochrome c oxidase (COX), and melanopsin-containing ganglion cells. We observed profound axon loss in the temporal portions of optic nerves with relative preservation in the nasal portions; this corr

[Situs inversus of the optic nerve. A case report].

INTRODUCTION: Situs inversus of the optic nerve is a congenital anomaly characterised by the emergence of the vessels in the retina towards the nose rather than in a temporal direction. It is caused by an anomalous insertion of the optic stalks into the optic vesicle that gives rise to dysversion of the head of the optic nerve. It is not an isolated condition and usually appears jointly with tilted disc syndrome and in patients with myopia. It is characterised by the presence of inferior conus atrophy, deficiencies in the temporal visual field, refraction defects and ambliopy. CASE REPORT: A

Stalagmite-like preretinal inflammatory deposits in vitrectomized eyes with posterior uveitis.

We report a new clinical sign of vitreous inflammation in patients with posterior uveitis: spectral-domain optical coherence tomography identified stalagmite-like, discrete, diffusely distributed, hyperreflective, preretinal deposits in previously vitrectomized eyes of 2 patients during flares of posterior uveitis. The extent of the deposits correlated with disease activity. The underlying primary diseases encountered were necrotizing retinochoroiditis secondary to toxoplasmosis and primary central nervous system lymphoma.

OCT visualization of corneal structural changes in traumatic dislocation of LASIK flap.

A 45-year-old woman presented with a traumatic laser-assisted in situ keratomileusis (LASIK) flap dislocation in the right eye. Anterior segment optical coherence tomography (OCT) showed the presence of macrostriae, flap stromal edema, epithelial hyperplasia between the striae and epithelial ingrowth. The flap was surgically repositioned with optimal visual recovery. Anterior segment OCT can efficiently visualize corneal structural changes associated with LASIK flap dislocation.

Radiology and laryngoscopy for the diagnosis of laryngeal carcinoma.

A comparison is made of the radiologic and clinical findings in 51 patients with laryngeal carcinoma. The radiologic methods comprised conventional films, tomography and laryngography; the clinical examination technique included microlaryngoscopy and the use of a 90 degrees optical instrument. Laryngography was found to be superior to the other two radiologic methods, for the delineation of the tumors. Radiology should be performed before laryngoscopy and biopsy.

Optic disc drusen: Recent developments on a slow-moving condition.

Optic disc drusen (ODD) are benign acellular calcified deposits that typically form early in life. They do not normally cause symptoms, so ODD are often detected incidentally as an abnormal or potentially swollen optic disc appearance. Distinguishing ODD from true optic disc edema is essential, and now, multimodal imaging, particularly enhanced-depth imaging optical coherence tomography, can detect ODD accurately and reduce the need for further investigations. The origins of ODD are unknown, although the potentially unique finding of heavily calcified extracellular mitochondria in the optic di

Secondary chiasmal herniation after pituitary adenoma treatment: a diagnostic pitfall.

A man in his 30s with a prior diagnosis of presumed primary open-angle glaucoma presented with progressive visual deterioration and headaches. Initial optic disc cupping and the established glaucoma diagnosis suggested glaucomatous optic neuropathy; however, visual field defects respecting the vertical meridian and optical coherence tomography changes inconsistent with glaucoma prompted neuroimaging, which revealed a large invasive pituitary macroadenoma compressing the optic chiasm, later confirmed as a mixed prolactin-secreting and growth hormone-secreting tumour. Following medical treatment

Fibrous dysplasia: A rare cause of optic neuropathy.

Fibrous dysplasia (FD) is a progressive and benign osteodystrophic disease where cranial bones are most commonly affected. In this case report, we present a 27-year-old patient with previous diagnosis of FD who was referred to our clinic with sudden loss of visual acuity and color discrimination. Examination of the right eye was normal, whereas visual acuity on the left eye was 6/9 and color vision (CV) with Ishihara test plates was 9/12. The visual field (VF) demonstrated a peripheral concentric defect on the left eye. As visual acuity in the left eye decreased to 6/30 and computed tomography

BILATERAL SEROUS CHORIORETINOPATHY AND PIGMENTARY GLAUCOMA - WHAT IS THE ASSOCIATION?

We present a patient with concurrent pigmentary glaucoma, bilateral central serous chorioretinopathy and unilateral optic disc pit, and propose a possible association of these conditions. Comprehensive ophthalmic examination of a 36-year-old man who was complaining of blurry vision and pain in the eyes showed reduced visual acuity on the left eye, elevated intraocular pressure in the right eye, bilateral signs of pigment dispersion syndrome, and bilateral central serous chorioretinopathy, combined with optic disc pit in the left eye. Visual field and optical coherence tomography findings demon

The optic nerve: a "mito-window" on mitochondrial neurodegeneration.

Retinal ganglion cells (RGCs) project their long axons, composing the optic nerve, to the brain, transmitting the visual information gathered by the retina, ultimately leading to formed vision in the visual cortex. The RGC cellular system, representing the anterior part of the visual pathway, is vulnerable to mitochondrial dysfunction and optic atrophy is a very frequent feature of mitochondrial and neurodegenerative diseases. The start of the molecular era of mitochondrial medicine, the year 1988, was marked by the identification of a maternally inherited form of optic atrophy, Leber's heredi

Assessing structure and function of the afferent visual pathway in multiple sclerosis and associated optic neuritis.

The afferent visual pathway is commonly affected in MS. Assessment of the afferent visual pathway using clinical, imaging and electrophysiological methods not only provides insights into the pathophysiology of MS, but also provides a method of investigating potential therapeutic measures in MS. This review summarises the various assessment methods, in particular imaging techniques of the visual pathway. Retinal nerve fibre layer (RNFL) thickness is usually reduced following an episode of optic neuritis. Techniques such as optical coherence tomography, scanning laser polarimetry, and confocal s
