[Effect of high dose pergolide mesilate on restless legs syndrome associated with Parkinson disease].

Restless legs syndrome (RLS) is one of the common nocturnal disturbance seen in Parkinson's disease (PD) patients. The prevalence of RLS with PD is greater than that of general populations; however, etiology of RLS in patients with PD is still controversial. We report a 63-year-old man with PD, who was admitted to our hospital with uncontrollable unpleasant feeling in both legs leading to sleep disturbance. At age 59, he experienced numbness and nocturnal myoclonus in his right foot. One year later, he developed resting tremor and bradykinesia in his right hand, and was diagnosed as PD. Levodo

The parkinsonian phenotype of spinocerebellar ataxia type 2.

BACKGROUND: We recently reported that spinocerebellar ataxia type 2 (SCA2) caused familial parkinsonism in 2 brothers with predominant symptoms of resting tremor, rigidity, and bradykinesia that responded to levodopa. OBJECTIVE: To investigate SCA2 as the possible cause of familial parkinsonism in our series and subsequently to analyze the correlation between the clinical manifestation and CAG repeat size in the ataxin-2 gene product. PATIENTS: One hundred thirty patients from 41 families with familial parkinsonism were examined for SCA2. Another 8 patients with the classic ataxic phenotype

The role of positron emission tomography imaging in movement disorders.

PET imaging provides the means to study neurochemical, hemodynamic, or metabolic processes that underlie movement disorders in vivo. Because the extent of presynaptic nigrostriatal dopaminergic denervation can be quantified in PD even at an early or preclinical stage of the disease, PET imaging may allow the selection of at-risk subjects for neuroprotective intervention trials. These techniques may also provide markers to follow progression of disease or evaluate the effects of neurorestorative interventions in patients who have more advanced disease. PET is expected to play an increasing role

EpiPen Jr versus EpiPen in young children weighing 15 to 30 kg at risk for anaphylaxis.

BACKGROUND: The EpiPen Jr (0.15 mg) and EpiPen (0.3 mg) auto-injectors, widely prescribed for the out-of-hospital treatment of anaphylaxis, have not been compared prospectively in young children. OBJECTIVE: The purpose of this investigation was to study the rate and extent of epinephrine absorption after use of the EpiPen Jr and the EpiPen in children weighing 15 to 30 kg. METHODS: In a randomized, double-blinded, parallel-group pilot study, children at risk for anaphylaxis self-injected epinephrine using either an EpiPen Jr or an EpiPen with the aid of a physician. Plasma epinephrine concen

Clinical correlates of vascular parkinsonism.

BACKGROUND: Parkinsonism may be due to other causes besides Parkinson disease (PD). Vascular parkinsonism (VP) has not been well defined and the clinical correlates of VP have not been clarified. OBJECTIVES: To seek evidence for or against the role of cerebrovascular disease in parkinsonism, and to identify clinical features that suggest a vascular origin. DESIGN: Retrospective chart review of patients with parkinsonism. A vascular rating scale was used to identify 2 patient groups, 1 with strong evidence of cerebrovascular disease (VP), and 1 with idiopathic PD. Clinical features of parkins

[A case of fasciitis associated with Basedow's disease and polymyositis].

A 39-year-old female suffered from diffuse goiter, palpitation, finger tremor and body weight loss for about one year. Then she developed acute onset of myalgia and swelling of calves, and muscle weakness of proximal limbs. She could not walk because of myalgia and muscle weakness, and was admitted to our hospital 4 days after the onset of muscle symptoms. On admission, her pulse was 110 per minute and she had finger tremor of 11-12 Hz. The thyroid gland was markedly and diffusely enlarged with an elastic soft surface. She presented muscle weakness of proximal limbs and neck, and had intermitt

Suspected postprandial hypoglycemia is associated with beta-adrenergic hypersensitivity and emotional distress.

Suspected postprandial (reactive or idiopathic) hypoglycemia is characterized by predominantly adrenergic symptoms appearing after meals rich in carbohydrates and by their rare association with low blood glucose level (< 2.77 mmol/L). We studied heart rate, blood pressure, plasma insulin, C-peptide, and catecholamine responses during a 5-h oral glucose tolerance test in eight patients with suspected postprandial hypoglycemia and eight age-, sex-, and body mass index-matched healthy controls. We also evaluated beta-adrenergic sensitivity by using the isoproterenol sensitivity test. Psychologica

EMG-Driven Musculoskeletal Modelling Framework for Virtual Simulation of Upper Limb Activation-Modulated Impairment Scenarios.

Background and Objectives: Surface electromyography (EMG) is widely used to assess muscle activation. However, direct interpretation of its functional biomechanical consequences remains challenging. This study aimed to develop and evaluate an EMG-driven musculoskeletal simulation framework for investigating how controlled modifications of muscle activation patterns influence joint-level biomechanics in the upper limb. The objective was not to reproduce specific clinical pathologies but to enable systematic virtual scenario analysis of activation-dependent movement alterations. Materials and Me

Body placement of inertial measurement units differentially affects physical activity assessment accuracy in drug-naïve Parkinson's disease.

Parkinson's disease (PD) affects physical activity, and physical activity reduces the burden of PD. Although shown in studies using inertial measurement units (IMU), it remains unclear at which position physical activity change can best be detected in this population. Within the FAIRPARK-II trial, a subgroup of 25 newly diagnosed persons with PD (pwPD) not taking disease-specific medication yet documented their physical activity and, in parallel, wore IMUs on the most affected ankle, wrist and the lower back for two weeks. Participant-reported physical activity was transformed into Metabolic E

Burst-type deep brain stimulation of the subthalamic nucleus in Parkinson's disease: A case series on tolerability and efficacy.

BACKGROUND: Deep brain stimulation (DBS) has become an evidence-based treatment for movement disorders such as Parkinson's disease (PD). Recent animal studies of DBS suggest that burst-type DBS may specifically activate neuronal subpopulations and that this type of electrical stimulation programing may improve the efficacy and durability of DBS. Burst type DBS is defined as a novel stimulation protocol in which intermittent bursts of traditional high-frequency rectangular wave stimulation are delivered. Implanted pulse generators can deliver such stimulation by setting specific "on" and "off "

Symptom-Specific Networks and the DBS-Modulated Network in Parkinson's Disease: A Connectivity-Based Review.

Objectives: With the development of advanced neuroimaging techniques, including resting-state functional magnetic resonance imaging and diffusion tensor imaging, Parkinson's disease (PD) has increasingly been recognized as a complex brain network disorder. In this review, we summarized research on brain networks in PD to elucidate the network abnormalities underlying its four major motor symptoms and to identify the networks modulated by deep brain stimulation (DBS). Materials and Methods: We searched PubMed and Web of Science for the most recent literature on brain network alterations in PD.

The Potential Role of the Regional Skull Conditions in Predicting the Efficacy of Transcranial Magnetic Resonance-Guided Focused Ultrasound in Patients With a Low Skull Density Ratio.

BACKGROUND AND OBJECTIVES: The therapeutic effect of magnetic resonance-guided focused ultrasound is generally not available to patients with a low skull density ratio (SDR). In this study, we investigated factors associated with treatment success in low-SDR cases with essential tremor and tremor-dominant Parkinson disease, and predictors of maximum temperature rise, with particular regard to regional skull conditions. METHODS: We retrospectively analyzed 171 consecutive cases. Descriptive statistics for the entire skull and averages across 10 regions were obtained for SDR, skull thickness, a

MRI-Guided Focused Ultrasound for the Treatment of Dystonia: A Narrative Review.

Contemporary surgical management of dystonia includes neuromodulation via deep brain stimulation (DBS) or ablative techniques such as radiofrequency (RF) ablation. MRI-guided focused ultrasound (MRgFUS) is an emerging modality that uses high-intensity ultrasound to precisely ablate targets in the brain; this is incisionless, potentially avoiding the surgical risks of a burr hole and transcortical tract to reach the anatomical target. There is some evidence of efficacy in essential tremor and Parkinson's disease (PD), but, to date, there is no study aggregating the evidence of MRgFUS in dystoni

Non-motor symptoms in patients with Spinocerebellar ataxia type 12.

INTRODUCTION: Spinocerebellar ataxia type 12 (SCA12) is a rare autosomal dominant neurodegenerative disorder caused by abnormal CAG repeat expansion in the PPP2R2B gene. This disease is classically characterized by action tremor, dysarthria, ataxia, and hyperreflexia. There are limited reports regarding the non-motor symptoms in patients with SCA 12. We investigated the frequency and factors associated with the selected non-motor symptoms in patients with SCA 12. METHODS: Genetically diagnosed (CAG repeats > 43) and symptomatic cases of SCA 12 were included in the study. Motor severity was as

OBJECTIVE: Familial adult myoclonic epilepsy (FAME) is an under-recognized disorder characterized by cortical myoclonus, generalized tonic-clonic seizures, and additional clinical symptoms, which vary depending on the FAME subtype. FAME is caused by pentanucleotide repeat expansions of intronic TTTCA/TTTTA in different genes. FAME should be distinguished from a range of differential diagnoses. METHODS: The differential diagnoses and frequent presentations leading to misdiagnosis of FAME were investigated from the available literature and reported based on an expert opinion survey.
