Double Layer Graft Using Temporal Fascia Graft in Tendon Exposure Wound.

BACKGROUND: Soft-tissue injuries with tendon exposure in the upper extremity can be reconstructed using various options, such as free flap and regional flap techniques. While skin grafting is an alternative, its use is limited by complications such as adhesions or graft failure. Hence, the double-layer graft, often using acellular dermal matrix (ADM), is preferred. However, ADM's drawbacks, including high cost and low engraftment rate, led to the temporal fascia being considered as an alternative. METHODS: In this study, instead of the acellular dermal matrix, the temporal fascia was chosen a

Bridging clinical capacity and functional performance: Clinical predictors of independence in advanced wheelchair skills after spinal cord injury.

CONTEXT: Advanced wheelchair skills (ramp, railing, curb, wheelie) are essential for mobility after spinal cord injury (SCI), but predictors of discharge independence remain unclear. OBJECTIVE: To identify admission predictors of improvement and discharge independence in advanced wheelchair skills after SCI. METHODS: We retrospectively analyzed 150 inpatients with SCI (2016-2023) at a single rehabilitation hospital with admission and discharge assessments for four advanced wheelchair skills. For each skill, participants were classified as Group 1 (no improvement or decline and remained depen

Results of finger extension reconstruction in hemiplegic patients after stroke by transferring the flexor carpi radialis branch to the posterior interosseous nerve.

PURPOSE: Contralateral C7 nerve transfer (CC7) surgery has demonstrated success in restoring upper limb motor function after central nervous system injuries. However, deficits in finger extension limit patient independence. This study evaluates a nerve transfer that transfers the motor branch of the flexor carpi radialis (FCR) to the posterior interosseous nerve (PIN) to improve finger extension. METHODS: Two patients with chronic brain injuries, who showed minimal finger extension recovery for over 1 year after CC7 surgery, were enrolled. Both patients then underwent the nerve transfer proce

Effects of asymmetrical stance and movement on body rotation in pushing.

Pushing objects in the presence of body asymmetries could increase the risk of back injury. Furthermore, when the object is heavy, it could exacerbate the effects induced by asymmetrical posture. We investigated how the use of asymmetrical posture and/or upper extremity movement affect vertical torque (Tz) and center of pressure (COP) displacement during pushing. Ten healthy volunteers were instructed to push objects of three different weights using two hands (symmetrical hand use) or one hand (asymmetrical hand use) while standing in symmetrical or asymmetrical foot-positions. The peak values

[Novel functional electrical stimulation for neurorehabilitation].

Our understanding of motor learning, neuroplasticity, and functional recovery after the occurrence of brain lesions has increased considerably. New findings in basic neuroscience have provided an impetus for research in motor rehabilitation. Several prospective studies have shown that repeated motor practice and motor activity in a real world environment have a favorable effect on motor recovery in stroke patients. Electrical stimulation can be applied in a variety of ways to the hemiparetic upper extremity following a stroke. In particular, electromyography (EMG)-triggered electrical muscle s

Asymmetrical ground impact of the hands after a trip-induced fall: experimental kinematics and kinetics.

BACKGROUND: Distal radius fractures are among the most common fall-related fractures. The manner in which the upper extremities are used for protection during a fall may exert a considerable influence on the incidence of injury. Here, we sought to determine the degree to which the assumption of sagittal plane symmetry was valid in unexpected falls after a trip, and to quantify the effects of asymmetrical upper extremity motion on impact kinematics and kinetics. METHODS: The motion of eight healthy older women who fell after being unexpectedly tripped was quantified. Impact kinematics and kine

The AMANDUS Project PART II-Advanced Microperfusion Assessed Non-Union Diagnostics with Contrast-Enhanced Ultrasound (CEUS): A Reliable Diagnostic Tool for the Management and Pre-operative Detection of Infected Upper-Limb Non-unions.

The management of upper-limb non-unions can be challenging, especially when infection is existent. Thus, pre-operative detection of infection plays a relevant role in non-union treatment. This study investigated in a large cohort the diagnostic potential of contrast-enhanced ultrasound (CEUS) as stand-alone method for differentiating between aseptic and infected upper-limb non-unions. Osseous perfusion of 50 upper-extremity non-unions (radius/ulna, n = 20; humerus, n = 22; clavicle, n = 8) was prospectively assessed with CEUS before revision surgery. The perfusion was quantified via time-inten

Upper limb surgery in osteogenesis imperfecta.

Although 90% of the surgical procedures performed on patients with OI are on the lower limbs, intramedullary fixation of the humerus may at times be indicated also in the upper limb for the correction of deformity and improvement of function. Upper limb deformities are generally present only in patients with severe disease (osteogenesis imperfecta congenita or osteogenesis imperfecta tarda I). The recommendations expressed in this paper are based on experience with 36 intramedullary rod fixation procedures on the upper limb in 12 patients with osteogenesis imperfecta, including 24 on the humer

Bannwarth Syndrome in a Patient With Early-Stage Breast Cancer: An Atypical Manifestation of Lyme Neuroborreliosis.

Neurological symptoms in breast cancer patients can arise from metastasis, medication side effects, or underlying neurodegenerative conditions. When imaging is inconclusive, the more uncommon immune-mediated neuropathies should be considered. Guillain-Barré syndrome and Lyme neuroborreliosis are two immune-mediated conditions that share neurological findings. This case involves a 72-year-old Caucasian female who presented with early-stage invasive ductal carcinoma with neuroendocrine features and gradual bilateral upper extremity weakness and paresthesia. Surgery was uncomplicated, but in the
