Ablation of de novo DNA methyltransferase Dnmt3a in the nervous system leads to neuromuscular defects and shortened lifespan.

DNA methylation is an epigenetic mechanism involved in gene regulation and implicated in the functioning of the nervous system. The de novo DNA methyltransferase Dnmt3a is expressed in neurons, but its specific role has not been clarified. Dnmt3a is activated around embryonic day 10.5 in mouse neuronal precursor cells and remains active in postmitotic neurons in the adult. We assessed the role of neuronal Dnmt3a by conditional gene targeting. Mice lacking functional Dnmt3a in the nervous system were born healthy, but degenerated in adulthood and died prematurely. Mutant mice were hypoactive, w

Osteomalacia from Mg-containing antacid: a case report of bilateral hip fracture.

Non-prescribed antacid drugs that contain magnesium and aluminum are widely used in the treatment of gastritis and peptic ulcer. One of the side effects of these antacid drugs is that they bind phosphate in the gut and result in its malabsorption. In this paper, a 42-year-old female patient who used magnesium hydroxide (Magnesie calcinee powder 100 g) to benefit from its laxative feature, and developed osteomalacia after losing 90 kg in 2 years will be presented by going through the related literature. She had widespread joint pain and could hardly walk without the help. Ca, P and vitamin D we

Perceived control reduces emotional stress in patients with heart failure.

We studied patients with heart failure (HF) to determine if perceived control reduces emotional distress (i.e., anxiety, depression and hostility) in chronic, debilitating cardiac illness and whether the demographic, clinical and psychologic characteristics of patients with high and low perceived control differed. Psychological assessment of 222 patients with heart failure included an evaluation of perceived control using the Control Attitudes Scale, as well as anxiety, depression and hostility using the Multiple Affect Adjective Checklist. Using multivariate analysis to control for difference

Stress fracture of the proximal tibia after total knee arthroplasty: a case report.

Stress fracture of the proximal tibia after total knee arthroplasty (TKA) is very rare. We found only 16 cases in a review of the literature. We report the case of a 76-year-old obese woman who sustained a stress fracture of the right proximal tibia without trauma 18 months after TKA. Pain developed in the proximal medial aspect of the tibia during walking. Physical examination showed tenderness of the proximal medial aspect of the tibia and varus deformity of 15 degrees during stress test of the knee. Roentgenography 4 weeks after symptom onset showed an obvious stress fracture line. Treatmen

Twenty four hour continuous non-invasive finger blood pressure monitoring: a novel approach to the evaluation of treatment in patients with autonomic failure.

Occasional sphygmomanometric readings are not an effective way of evaluating the effect of treatment in patients with hypoadrenergic orthostatic hypotension. A novel non-invasive portable device (Portapres) was used to monitor 24 hour continuous finger blood pressure before and during chronic volume expansion in a 66 year old woman with severe orthostatic hypotension. In both conditions pressures while she was standing were lowest in the morning. Her tolerance to standing and walking increased during the day and, as a consequence of a higher upright mean blood pressure, was improved after trea

A syndrome of early recognition of occult hydrocephalus and cerebral atrophy.

A clinical and neuropsychological syndrome for early recognition of occult hydrocephalus and cerebral atrophy is described. Five illustrative patients are reported. The main features of the syndrome are (i) subjective non-specific complaints (headaches, depression and loss of memory); (ii) the tonic foot response of the sole and the grasp reflex of the foot in the absence of the grasp reflex of the hand; (iii) attacks of sudden and transient loss of muscle tone in both lower limbs leading to falls without warning while standing or while walking. These attacks indistinguishable from drop-attack

Home-Based mHealth Platform (Active-Feet) for Children With Idiopathic Toe Walking: Design, Development, and Acceptability Study.

BACKGROUND: Physical exercise and stretching programs are the best initial options to treat idiopathic toe walking (ITW). These programs are designed to improve the flexibility and strength of lower limb muscles, enhancing the ankle's range of motion and allowing for a normal gait pattern. In the pediatric population, one of the major limitations reported by therapists is low adherence to rehabilitation treatments or a lack of follow-up. In this context, children using mobile health (mHealth) tools could play an active and central role in their treatment of ITW, while mobile apps could also al

The diagnostic blind spot of spirometric staging in biomass-related non-smoking chronic obstructive pulmonary disease: a cross-sectional study of spirometry-diffusion discordance in a rural South Indian population.

Chronic obstructive pulmonary disease (COPD) is heterogeneous, yet severity is conventionally graded by spirometry [forced expiratory volume in 1 second (FEV1)] on the assumption that airflow limitation reflects parenchymal damage. Biomass-related non-smoking COPD (NS-COPD) is common in low- and middle-income countries. In this hospital-based cross-sectional study, we compared the physiological phenotypes of smoking COPD (S-COPD) and biomass-related NS-COPD and tested the concordance between Global Initiative for Chronic Obstructive Lung Disease (GOLD) spirometric severity and diffusion-based

Effect of a 12-Week Structured Exercise Program on Aerobic Capacity and Flexibility in Cardiac Physiologic Pacing Pacemaker Patients.

BACKGROUND: Cardiac conduction disorders causing bradyarrhythmias often require permanent pacing. Although physiologic pacing better preserves ventricular activation than conventional right ventricular pacing, many patients continue to experience reduced exercise capacity, flexibility, and functional independence after implantation. Despite the recognized benefits of cardiac rehabilitation, evidence regarding structured exercise programs in patients with cardiac physiologic pacing remains limited. OBJECTIVE: To evaluate the effects of a 12-week structured aerobic and flexibility exercise prog

Multichannel Transcranial Direct Current Stimulation Combined With Treadmill Gait Training in Patients With Parkinson's Disease: A Pilot Study.

BACKGROUND: Gait problems are critical impairments in Parkinson's disease (PD) and are related to increased risk of fall and negatively impact activities of daily life. Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation technique that can modify the cortical excitability of gait-related brain regions. In this study, we investigated whether multichannel tDCS with simultaneous treadmill gait training could improve gait in PD. METHODS: Twenty-four patients with PD were assigned randomly to a real or sham tDCS group. Before intervention, one patient of the real tDC

Paroxysmal tilt illusion after blinking-an unusual form of stroke manifesting with parietal vertigo.

A man in his 70s had an episode of vertical visual tilt after blinking, as if the room tilted to the right, and of deviation of the body to the right when he walked, together with slight vertigo and nausea. He was initially suspected to have a transient ischaemic attack and was investigated with neuroradiology, telemetry and echocardiography.Neurological examination showed horizontal saccadic eye movements and deviation to the right with the Unterberger's test. Brain MRI revealed a minor stroke in the right parieto-occipital junction and an older, smaller infarction in the right cerebellum. Th

Experiment-free exoskeleton assistance via learning in simulation.

Exoskeletons have enormous potential to improve human locomotive performance1-3. However, their development and broad dissemination are limited by the requirement for lengthy human tests and handcrafted control laws2. Here we show an experiment-free method to learn a versatile control policy in simulation. Our learning-in-simulation framework leverages dynamics-aware musculoskeletal and exoskeleton models and data-driven reinforcement learning to bridge the gap between simulation and reality without human experiments. The learned controller is deployed on a custom hip exoskeleton that automati

Neurological Spectrum of Epstein-Barr Virus Infection: A Rare Clinical Case.

The Epstein-Barr virus (EBV) is a DNA virus that has been infecting humans since ancient times, capable of causing a wide range of pathologies and affecting approximately 90% of the population. A 61-year-old male with no significant medical history presented with a 5-day history of imbalance and difficulty walking. Neurological examination revealed specific findings, including absent reflexes, bilateral asynergy, and gait abnormalities. Contrasting with Guillain-Barré Syndrome, lumbar puncture suggested a central nervous system infection. Serological testing confirmed Epstein-Barr virus (EBV)

Agreement between Optoelectronic System and Wearable Sensors for the Evaluation of Gait Spatiotemporal Parameters in Progressive Supranuclear Palsy.

The use of wearable sensors for calculating gait parameters has become increasingly popular as an alternative to optoelectronic systems, currently recognized as the gold standard. The objective of the study was to evaluate the agreement between the wearable Opal system and the optoelectronic BTS SMART DX system for assessing spatiotemporal gait parameters. Fifteen subjects with progressive supranuclear palsy walked at their self-selected speed on a straight path, and six spatiotemporal parameters were compared between the two measurement systems. The agreement was carried out through paired da
