INTRODUCTION: A surgical technique is described to preserve the pronator quadratus muscle when fixing distal radius fractures with volar locking palmar plates.
TECHNIQUE: Using a classic volar Henry approach to the wrist, the fibrous distal attachments of the pronator quadratus muscle are released. The locking palmar plate is passed under the pronator quadratus muscle and its correct placement is checked by fluoroscopy. Locking screws are inserted through mini-incisions in pronator quadratus.
DISCUSSION: The pronator quadratus muscle plays an important role in wrist function, both in forearm

The objective was to develop a self-administered activity limitation measurement tool for individuals with hand and wrist injuries, and to evaluate the reliability and validity of the new instrument, the Hand Assessment Tool (HAT). The final version of the HAT instrument, the SF12, and Disabilities of Arm, Shoulder, and Hand Questionnaire (DASH) were given to 94 consecutive new hand surgery clinic patients. Statistical analyses indicated excellent internal consistency with Cronbach's alpha of 0.91. Test-retest reliability of the HAT showed a concordance correlation coefficient of 0.73, with 95

Twenty-two patients underwent surgery for 23 complex articular fractures of the distal radius of C.3 type, according to the AO classification. The surgical treatment consisted, in all cases, of a closed, or limited-open, reduction and external fixation. The functional and radiographic results were analysed at a mean follow-up of 40 months. This retrospective study confirms that satisfactory functional results where obtained in 12 out of the 15 wrists where all the intra and extra-articular parameters of the Fernandez' criteria where respected. For those wrists where keeping with Fernandez' cri

Lateral and medial epicondylitis represent overuse tendinopathies of wrist extensor and wrist flexor muscles, respectively. In lateral epicondylitis, a short-term therapeutic efficacy of glucocorticoid injection and limited evidence on the efficacy of acupuncture has been shown. De Quervain tendinopathy is caused by tendinous impingement by a thickened retinaculum. There is limited evidence on the efficacy of glucocorticoid injection in this condition.Trigger finger usually results from tendon entrapment beneath a thickened A1 flexor pulley. An association with hand tool use and diabetes has b

Isolated fractures of the os triquetrum are a very rare condition, since they are usually combined with other fractures of the carpus. An undetected isolated fracture of the os triquetrum in a 28-year-old pieceworker revealed an osseous consolidation with a misalignment of the joint surface on presentation 6 months after his primary injury. During the whole time he was unable to work due to the severely painful restricted range of movement of his wrist. There are no general guidelines for treatment. Instead of correction of the anatomy of the os triquetrum, we decided to perform a subperiostea

Previous reports of lunotriquetral arthrodesis suggest relatively low rates of primary fusion without the use of permanent fixation and/or prolonged immobilization. We performed 26 lunotriquetral arthrodeses in 24 patients with a technique using cancellous bone graft to fill a biconcave space created in the adjoining bones with parallel K-wire fixation. The indications included symptomatic lunotriquetral instability and degenerative arthritis. All patients received conservative treatment before surgery. Primary fusion was achieved in all wrists in an average of 50 days. Postoperative wrist fle

Rising popularity of roller-skates in recent years has resulted in increasing numbers of accidents involving roller-skates. The subject of our study was a consecutive number of patients who were treated at the casualty departments of Svendborg and Middelfart hospitals, Denmark, between July 1997 and June 1998. A total number of 169 patients representing 179 injuries were registered. A high incidence of injuries was found among children of school age. Protective equipment was used, in 31% (n = 52) of cases, which shows an increase compared to previous studies in Denmark (1). Injuries of the dis

Three patients, a woman aged 33 years, a man aged 42 and a woman aged 66, had a loss of hand function. This was due to denervation of a peripheral nerve, in essence the radial, ulnar and median nerves respectively. In the first two cases this was caused by trauma and in the third by compression neuropathy. The problems were loss of wrist and finger extension (a dropping hand) in the first, loss of intrinsic muscle function and thumb adduction in the second and loss of opposition in the third patient. Tendon transfers were performed by diverting a tendon insertion of a still functional muscle t

Compression neuropathy is one of the most frequently encountered conditions in a hand surgeon's daily practice. Median nerve compression at the wrist or carpal tunnel syndrome (CTS) is the most common of all nerve compression syndromes of the upper extremity1.3 and CTS has been used as a model for studying compression neuropathy (CN). This compressive lesion of the median nerve occurs in a tunnel located within the wrist called the carpal tunnel. Severe chronic inappropriately treated CTS can cause prolonged symptoms and lead to permanent nerve damage. Carpal tunnel release (CTR) is currently

This paper presents a different technique of treatment for Bayne type IV radial club hand using a microvascular joint transfer in order to reconstruct the absent half of the wrist joint, aiming for better movement and stability at the wrist joint with preservation of longitudinal growth. The method uses preoperative soft tissue distraction to obtain proper alignment of the hand on the ulna before the second metatarsophalangeal joint with the whole metatarsal bone is transplanted. The treatment takes about 4 months and the optimum period for surgery is during the second year of life. Pollicizat

In the present study we describe the case of a 11 years old child, female, who was accidentally fulgurated by the left arm and after has presented an episode of pallor, shaking, confusion, throbbing and tingle of the arms. For this reason she was admitted in our Department of Pediatrics, University of Rome "La Sapienza". The child presented also a moderate pain at the left wrist. For this reason was performed a wrist X Ray which showed an incomplete fracture of the distal extremity of the radial diaphysis, with a small ulnar infraction of the same side. Life parameters and the ECG were normal

One female and seven male patients (median age 25.5) presented with traumatic avulsion of the triangular fibrocartilage complex (TFCC), type 1B according to Palmer's classification. Reattachment of the TFCC near its anchoring point was combined with an intraarticular shortening osteotomy of the ulnar head. This provides an excellent approach to the TFCC and a well vascularized anchoring surface. The mooring point is biomechanically appropriate and the tissues with the best biomechanical properties are used. The functional results with a mean follow-up of 3 years were encouraging, as demonstrat

Over a two year period from 1987-1989 all fishing-related injuries treated in the Emergency Room at Bornholm's central hospital in Rønne and at some general practices were registered prospectively. One hundred injuries were registered altogether. Accidents occurring in connection with putting out and recovering apparatus constituted respectively 14% and 36%. Working with the trawler scoop was responsible for 18% of the accidents and was the most common direct cause, followed by work with winches which constituted 11%. Falls made up almost a quarter of the accidents. Finger injuries constituted

An aberrant muscle is demonstrated by magnetic resonance (MR) imaging in a patient presenting with focal wrist swelling and compression neuropathy of median and ulnar nerves following 4 months of carpentry work. The muscle originated from the palmaris longus tendon and ulnar antebrachial fascia at the lower half of the forearm as a single belly, then diverged medially from palmaris longus tendon and bifurcated. Both portions of the split muscle extended into the distal ulnar tunnel or Guyon's canal. One segment joined with the abductor digiti minimi muscle and the other with the flexor retinac

Peripheral neuropathies of the ulnar nerve at various locations in the upper extremity are common, but compression of the ulnar nerve at the level of the wrist due to an anomalous muscle is rare, though reported in the literature. Three cases of an abnormal abductor digiti minimi origin were first reported by Wood in 1868, but no mention was made of an associated ulnar neuropathy. To data, there have been 8 reported cases of an anomalous abductor digiti minimi, with sensory or motor abnormalities of the ulnar nerve the most common presentation. This case report describes a patient who had subj

Muscle tonus in patients with rheumatoid arthritis (RA) was collated with the activity of the pathological process and functional articular insufficiency (FAI) in different variants of polyneuropathy (P). Hetero- or unidirectional hypotension of muscles was the most characteristic sign in RA. In cases of the sensory variant of P, patients with an early period of the disease and 1-0 degree FAI showed severe damage to the central motor neuron, linked with a dissociated type of muscular dystonia (predominantly hypotension of the flexory group of legs) and a peculiar dystonic phenomenon (a vestibu

Tenosynovectomy in patients with rheumatoid arthritis is performed most commonly in the region of the wrist and the ankle. Complications with regard to gliding ability and rupture of the flexor tendons are feared in the so-called no-man's land. The goal of this study was the morphologic examination of the regeneration of synovial membrane in flexor tendons after radical tenosynovectomy. Chronological and qualitative changes of the membrane as well as possible negative effects on the tendon such as adhesions and trophic disturbances were of interest. Synovectomies of flexor tendons were perform

The management of patients with clinical evidence of a fracture of the carpal scaphoid bone but without radiological evidence of a fracture is based on dogma emphasizing the need to immobilize the wrist in all cases. Because of the apparently high proportion of patients who spend up to six weeks in a plaster cast and in whom no fracture is ever demonstrated radiologically, a study was undertaken to determine the fate of those wrist injuries diagnosed as clinical fractures of the scaphoid. All patients who presented with clinical or radiological evidence of fractures of the scaphoid over a one

In the treatment of multi-fragment fractures of the base of the radius plate osteosynthesis using the small-fragment instrument set is preferred to conservative treatment due to the superior results achieved. However, one still cannot dispense with an additional immobilization of the fracture in a plaster cast. The extension of intra-articular comminuted fractures of the base of the radius is a technically relatively simple procedure and does not require a supporting plaster cast. In this paper we report the results of the treatment of twenty-seven wrist-fracture patients who were treated by e

The reported case is in its development, diagnostic, course, and legal aspect so extraordinary, that an explicit report is justified. The patient complained of pain in his right wrist region some days following an unusual work with a drilling machine for eight days. The X-rays six months later showed an irregular joint space between the lunate and triquetrum bone, while on his left wrist a synostosis between these bones was found. These changes seemed to be suspicious for a pseudarthrosis of a congenital synostosis, but this diagnosis led to a six years' controversy in several legal procedures
