Acute acalculous cholecystitis due to Vibrio cholerae.

The case of a 57-year-old woman admitted with symptoms and signs suggesting an intestinal infection caused by Vibrio cholerae, and who also developed a clinical picture compatible with acute cholecystitis, is presented. Cholera was diagnosed by examining a fresh sample of stools and cultures. An abdominal sonogram disclosed signs of acute acalculous cholecystitis. She underwent cholecystectomy, and cultures of a clear fluid and a "milky" sediment found within the gallbladder were also positive for V. cholerae. This microorganism was seen at the gallbladder mucosa microscopically. The strain wa

Acalculous cholecystitis in bone marrow transplant patients.

BACKGROUND: Acalculous cholecystitis (ACC) is an uncommon disorder of the biliary tract, accounting for approximately 6% of acute cholecystitis cases. In this study, cholecystitis was seen in 8 of 770 bone marrow transplant recipients, with ACC occurring in five (63%). METHODS: Records of 592 allogenic and 150 autologous BMT patients were reviewed for risk factors associated with ACC. RESULTS: Only the number of blood transfusions administered and the use of total parenteral nutrition were associated with ACC development. ACC occurred in 4 of 42 (9%) allogeneic recipients who required exchan

[Acute acalculous cholecystitis. Results of surgical treatment].

During an 11 year period, 47 patients with acute acalculous cholecystitis were operated on. Two to one male/female ratio was observed with a mean age of 55 age of 55 years. No one had a past history of biliary tract pathology but 70 per cent of the patients had risk factors, mainly diabetes mellitus, cardiovascular and collagenous diseases, some different of those reported in the world literature (sepsis, trauma, non biliary tract surgery, etc.). The ultrasound was the best diagnostic tool. Open cholecystectomy was performed in all patients and some sort of local complication was found in 85 p

[Acute acalculous cholecystitis. A stress-induced complication].

The acute acalculous cholecystitis is a stress induced complication following surgery and trauma. A totally different disease is a large atonic gallbladder during parenteral nutrition. The symptoms of the acalculous cholecystitis are similar to acute cholecystitis in patients with cholelithiasis. The diagnosis can be very difficult, because in the postoperative period patients are usually very sick, treated with analgetic and antibiotic drugs and therefore the symptoms are mostly lacking or equivocal. Typical signs are fever and leucocytosis. An ultrasound scan should be performed at the inten

[Acute acalculous cholecystitis after digestive caustic burn].

A case is reported of acute acalculous cholecystitis in a patient who suffered a digestive burn treated by total parenteral nutrition. Pain in the right upper abdominal quadrant and fever occurred 26 days after the start of parenteral nutrition and 24 h after abruptly resuming oral feeding. Ultrasonography showed a distended gall-bladder with a thickened wall and the formation of sludge. Cholecystectomy was followed by a complete recovery. The part played by each of the corrosive burn, the total parenteral nutrition and the abrupt resumption of oral feeding in the pathogenesis of this complica

Postoperative acalculous cholecystitis due to Torulopsis glabrata.

Acute acalculous cholecystitis due to Torulopis glabrata, an opportunistic yeast, developed postoperatively in a 70-year-old man who had an extremely complicated course after resection of an abdominal aortic aneurysm. The infection first appeared as an acute surgical abdomen, three days after resumption of solid food intake subsequent to a prolonged ileus and after 31 days of parenteral hyperalimentation. The condition was successfully treated by cholecystostomy; at the time of writing, six months after cholecystostomy, there are no gastrointestinal symptoms.

Acalculous cholecystitis presentation in a young patient.

Acute acalculous cholecystitis accounts for only 5-10% of cholecystitis cases and is often associated with severe trauma, critical illness, or chronic disease. Our case describes an otherwise healthy 25-year-old female presenting with acute abdominal pain. After undergoing a magnetic resonance cholangiopancreatography and cholescintigraphy she was diagnosed with acute acalculous cholecystitis. Her symptoms resolved following laparoscopic cholecystectomy which highlights the importance of prompt diagnosis and treatment of acalculous cholecystitis even in the absence of trauma or critical illnes

A Case of Melioidosis Presenting as Acalculous Cholecystitis.

We describe a case of melioidosis presenting as acalculous cholecystitis in a middle-aged Chinese male. The patient presented with clinical features of cholecystitis and computed tomography (CT) imaging did not reveal other obvious sources of sepsis other than acalculous cholecystitis. The decision was made by the hepatobiliary team to proceed with an urgent cholecystectomy in view of patient's septic presentation. Cultures from peripheral blood and intraoperatively obtained bile fluid grew Burkholderia pseudomallei. The patient subsequently completed one month of meropenem, followed by anothe

The clinical diagnosis of chronic acalculous cholecystitis.

BACKGROUND: Chronic acalculous cholecystitis (CAC), a diagnosis of exclusion, appears to be more common than was previously thought. We correlated the pathology with test results and response to treatment in a prospective study to obtain data for evidence-based management. METHODS: The study subjects were patients with chronic biliary symptoms but had normal sonogram results. They were further investigated with esophagogastroduodenoscopy, multidiscipline evaluations, and cholecystokinin-stimulated scintigraphy (CCK-HIDA). They were offered laparoscopic cholecystectomy (LC) when the symptoms w

Acalculous cholecystitis in patients with acute leukaemia.

Cases of acalculous cholecystitis in patients with acute leukaemia are rare. Manifestations of the primary disease often mask the acute cholecystitis symptoms. A high index of suspicion and ultrasonographic examination in leukaemic patients with abdominal pain and unexplained fever may allow earlier diagnosis, before the development of complications. We report two cases of acute acalculous cholecystitis during the course of acute leukaemia.

Acute acalculous cholecystitis in the critically ill.

Acute acalculous cholecystitis, inflammation of the gall-bladder without evidence of calculi, accounts for 2 to 15% of all cases of acute cholecystitis. The incidence of acute acalculous cholecystitis in adults undergoing cholecystectomy may be as high as 15 per cent and up to 32 per cent in the pediatric population. During the past 10 years, 22 patients who were being treated in the intensive care unit for other reasons underwent cholecystectomy for acute acalculous cholecystitis. Eighteen (82%) of the patients were male, the average age was 61 years, and patients spent an average of 19 days

[Acute acalculous cholecystitis as a surgical problem].

Experience in operative treatment of 228 patients for acute acalculous cholecystitis is analysed. This disease differs from acute calculous cholecystitis essentially in the main clinical and morphological signs. Four types of acute acalculous cholecystitis were distinguished according to origin: vascular, enzymatic, enzymatic-vascular, and obturative. The clinical forms are shown: primary vascular, emphysematous, posttraumatic, gallbladder torsion, enzymatic, obturative. It is pointed out that acute acalculous cholecystitis is characterized by rapid destruction of the cystic wall and frequent

Radiology and pathology in canine acalculous cholecystitis.

Although the value of hepatobiliary scan and ultrasonography are well established in calculous cholecystitis, their role in acute acalculous cholecystitis (AAC) is less certain. This study assesses the diagnostic reliability of these tests in AAC chemically induced in 10 dogs with rutin, a compound known to induce AAC. Ultrasonography demonstrated pericholecystic fluid or wall thickening in 8 of 10 dogs. Hepatobiliary scans were abnormal in only 2 of 9 dogs. Pathologic evaluation showed significant abnormalities in all gallbladders. Our studies confirm the usefulness of ultrasonography in diag

Acalculous Cholecystitis Secondary to Hepatitis C Infection.

Acute acalculous cholecystitis (AAC) represents gallbladder inflammation without evidence of gallstones. This typically results from gallbladder stasis and/or ischemia, which then causes a local inflammatory response within the wall. The condition is typically multifactorial and seen in critically ill patients, with associated risk factors that include trauma, burns, infections, total parenteral nutrition, and surgery. We present the case of a patient with acute-on-chronic hepatitis C infection leading to AAC.

Kawasaki Disease Presenting as Acute Acalculous Cholecystitis.

Acute acalculous cholecystitis (AAC) is a rare, potentially serious disease that has been associated with Kawasaki disease (KD) in children. Studies suggest that patients presenting with severe abdominal symptoms secondary to KD have increased resistance to intravenous immunoglobulin (IVIG), and a higher rate of coronary artery aneurysms. We describe an eight-year-old boy who presented to the emergency department with severe abdominal pain and was diagnosed with AAC and KD. He was treated with IVIG and high-dose aspirin, achieving good response with complete symptom resolution. He had no coron

Fatty meal ultrasonography in chronic acalculous cholecystitis.

Chronic acalculous cholecystits typically presents with biliary symptoms, normal blood tests and unremarkable ultrasound, computerized tomography and magnetic resonance cholangiopancreatography. However, cholescintigraphy may show reduced gallbladder ejection fraction (GBEF). There are no reports on using ultrasound to measure GBEF in adults. Twenty-eight patients with the above presentation underwent ultrasound before and after ingestion of a standardized fatty meal. Consequently, GBEF was calculated. Seven patients had reduced GBEFs (<38%). Two of these patients underwent cholecystectomy and

Acalculous candidal cholecystitis after pediatric renal transplant.

Источник пометил публикацию как отозванную

AAC caused by Candida is an uncommon entity usually seen in the critically ill. Here, we present the case of an 18-month-old renal transplant patient who developed candidal AAC during the post-operative period. Previous articles have addressed acalculous cholecystitis secondary to a variety of causes, or addressed a wide variety of Candida infections in the biliary tract, but this is the first discussion of cholecystitis caused by Candida without confounding factors such as biliary calculi or multiple pathogens. After the discussion of our patient's case, we also reviewed the English-language
