Mid-level health providers (MLHPs) in delivering and improving access to primary health care services - a narrative review.

BACKGROUND: For primary healthcare systems to bring care closer to the communities, the availability of appropriate human resources is crucial. The primary care workforce in the world is expanding to include non-physician health workers (NPHWs) to increase its capacity. Also, NPHWs as mid-level health providers (MLHPs) are currently being employed in high- and low-income countries to assist doctors and specialists to make up for the scarcity of health professionals. Given the wide prevalence in the deployment of NPHWs as mid-level health providers, this article collates recent evidence on the

Examining Access to Primary Care for People With Opioid Use Disorder in Ontario, Canada: A Randomized Clinical Trial.

IMPORTANCE: People with opioid use disorder are less likely than others to have a primary care physician. OBJECTIVE: To determine if family physicians are less likely to accept people with opioid use disorder as new patients than people with diabetes. DESIGN, SETTING, AND PARTICIPANTS: This randomized clinical trial used an audit design to survey new patient intake at randomly selected family physicians in Ontario, Canada. Eligible physicians were independent practitioners allowed to prescribe opioids who were located in an office within 50 km of a population center greater than 20 000 peopl

Access to dental services and oral health-related quality of life in the context of primary health care.

The aim of the present study was to evaluate the association between access to oral health care in the Primary Health Care (PHC) and Oral Health-Related Quality of Life (OHRQoL). The present study was a cross-sectional study, and the sample was composed of 412 users living in the areas covered by the public PHC services who visited a health unit for an oral exam or treatment in the last 24 months. Participants in the study responded to a home-based interview with questions that addressed socioeconomic status, behavioral, general health, dental prostheses, access to dental services in the PHC a

The association between primary source of ambulatory care and access to and outcomes of treatment among AIDS patients.

OBJECTIVES: To examine the relationship between having a primary source of ambulatory care (PSAC), access to AIDS treatment and prophylaxis for opportunistic infection, and hospital and mortality outcomes among heterosexual men and women with AIDS. METHODS: Using a linked AIDS Registry-Medicaid database, 366 adults were identified (1989-1991) with at least 1 year of continuous Medicaid enrollment before AIDS diagnosis, who survived 2 weeks after diagnosis, and with no antiretroviral use or Pneumocystis carinii pneumonia (PCP) prophylaxis during the pre-diagnosis year. Outcomes included times

Integrating Physical Therapy into Primary Care to Enhance Veteran Access to Healthcare: Findings from the Veterans Health Administration's PACT-PT Program.

BACKGROUND: The integration of physical therapy (PT) into primary care has shown significant benefits in patient outcomes and healthcare efficiency. Veterans suffering from chronic musculoskeletal (MSK) pain, mobility, or vestibular issues often face challenges in accessing non-pharmaceutical treatments. Previous studies have indicated that early access to PT can lead to better outcomes, higher patient satisfaction, and cost savings. OBJECTIVE: This evaluation was conducted in collaboration with the Veterans Health Administration (VHA) operational partners to assess the implementation and eff

The Practices of Portuguese Primary Health Care Professionals in Palliative Care Access and Referral: A Focus Group Study.

Background/Objectives: The prevalence of people with incurable and progressive diseases in primary health care is high. Family doctors and nurses must be active agents in the early identification of palliative needs and the implementation of palliative approaches in cases of low to intermediate complexity. While there is a need for early referral of more complex palliative care (PC) cases to specialized teams, primary health care (PHC) professionals lack the confidence or skill to describe their role. This study sought to explore and describe (a) the practices of PHC professionals regarding th

Impact of a decline in Colorado Medicaid managed care enrollment on access and quality of preventive primary care services.

BACKGROUND: Beginning in 1997 the Colorado Medicaid program de-emphasized managed care and shifted children from enrollment in a health maintenance organization (HMO), which required an enrollee to have an assigned primary care physician, to either the unassigned fee-for-service (UFFS) program in which the enrollee was not required to have a primary care physician (PCP) or to the primary care physician program (PCPP) in which the enrollee was required to select a participating PCP if one was available. The proportion of Medicaid enrollee-months in HMOs dropped from 75.4% in 1997 to 29% in 2003

Preventive service gains from first contact access in the primary care home.

BACKGROUND: The patient-centered medical home (PCMH) concept recently has garnered national attention as a means of improving the quality of primary care. Preventive services are one area in which the use of a PCMH is hoped to achieve gains, though there has been limited exploration of PCMH characteristics that can assist with practice redesign. The purpose of this study was to examine whether first-contact access characteristics of a medical home (eg, availability of appointments or advice by telephone) confer additional benefit in the receipt of preventive services for individuals who alread

Impact of primary care accessibility and availability on inappropriate emergency department attendance in Singapore.

INTRODUCTION: Primary care providers (PCPs) can safely manage non-emergency acute medical cases, reducing emergency department (ED) visits. This study examined how proximity and availability of PCPs affect inappropriate attendance (IA) at a tertiary hospital ED in eastern Singapore. METHODS: We conducted a retrospective cohort study of eligible ED visits between 1 January 2023 and 31 December 2023. Reporting followed STROBE guidelines. Multivariable logistic regression assessed the association between IA and (a) travel time to an available PCP, (b) travel distance to an available PCP, and (c)

Open access echocardiography: a prospective audit of referral patterns from primary care.

Following recently published recommendations and guidelines, a prospective audit of 222 consecutive patients referred for open access echocardiography was conducted over a period of three months in a large district general hospital in the UK. Our study demonstrated the waiting time for an open access echocardiogram to be shorter than the waiting time for the outpatient clinic, which allowed identification of clinically significant cardiac disease sooner, leading to early advice on patient management. Specialist referral was avoided by the inclusion of management comments by a cardiologist in t

Oregon primary care providers as a frontline defense in the War on Melanoma™: improving access to melanoma education.

Melanoma is one of the deadliest forms of skin cancer but is typically cured with surgical excision when detected early. As an access point to medical care, primary care providers (PCP) play an integral role in early skin cancer detection. However, limited time for examinations and dermatologic training may present barriers to effective skin examination in the primary care setting. As a facet of Oregon Health & Science University's War on Melanoma™ (WoM), our multi-pronged outreach initiative aims to provide PCPs across Oregon with free, convenient, and effective melanoma education. The WoM PC

Most primary care physicians are aware of prescription drug monitoring programs, but many find the data difficult to access.

State prescription drug monitoring programs are common tools intended to reduce prescription drug abuse and diversion, or the nonmedical use of a prescribed drug. The success of these programs depends largely upon physicians' awareness and use of them. We conducted a nationally representative mail survey of 1,000 practicing primary care physicians in 2014 to characterize their attitudes toward and awareness and use of prescription drug monitoring programs. A total of 420 eligible physicians (adjusted response rate: 58 percent) returned completed surveys. Among all physicians surveyed, 72 perce

What high-risk women are telling us about access to primary and reproductive health care and HIV prevention services.

Focus group discussions on barriers to health care and attitudes toward family planning, reproductive health services, and condom use were conducted with 63 women at high risk for HIV due to their own injection drug use, sex with injection drug users, sex industry work, or a history of multiple sexually transmitted diseases. Barriers identified include the high cost of health care, perceived poor quality of care and experiences of discrimination and stigmatization, geographic accessibility, fear of legal/social services punitive actions, misperceptions about the efficacy of birth control metho

Geographic Access to Primary Care and Racial and Ethnic Disparities in the Prevalence of Maternal Chronic Conditions 2020-2024.

INTRODUCTION: Unequal access to preventive health care before and between pregnancies contributes to risk for adverse perinatal outcomes. Few studies have examined access to primary care as a contributor to racial and ethnic disparities in chronic disease prevalence among women having a live birth. METHODS: We linked 2020-2024 natality data with Health Resources and Services Administration's Area Health Resource Files and used spatial Bayesian conditional autoregressive models to estimate county-level associations between geographic access to primary care and prevalence of chronic conditions …

A randomized controlled non-inferiority trial of primary care-based facilitated access to an alcohol reduction website (EFAR Spain).

BACKGROUND: Brief interventions (BI) for risky drinkers in primary healthcare have been demonstrated to be cost-effective but they are still poorly implemented. Digital BI seems to be a complementary strategy to overcome some barriers to implementation but there is a scarcity of studies in clinical environments. We present the results of a randomized controlled non-inferiority trial which tests the non-inferiority of facilitated access to a digital intervention (experimental condition) for risky drinkers against a face-to-face BI (control condition) provided by primary healthcare professionals
