An asystolic cardiac arrest is reported which occurred at the same time as supramaximal tetanic stimulation over the accessory nerve in order to evoke contractions in the trapezius and sternocleidomastoid muscles. The cause may have been the inadvertent stimulation of one or more of the cranial nerves of the carotid sheath at the base of the skull: the cranial root of the accessory nerve, the vagus, the sino-carotid branch of the glossopharyngeal nerve or the hypoglossal nerve. The most likely culprit, if not the vagus itself, was the cranial root of the accessory nerve which both functionally

We report three cases of ipsilateral spinal accessory nerve palsy complicating carotid endarterectomy. Awareness of this cranial nerve injury, as a complication of this common surgical procedure, can lead to early diagnosis and avoids unnecessary investigations. This should be considered whenever such patients complain postoperatively of ipsilateral shoulder pain/weakness or "aching" about the ear, even if some time has elapsed since surgery.

In a series of 23 patients, the commonest cause of accessory nerve palsy was surgical trauma at the time of lymph node biopsy. The less common causes were penetrating or blunt trauma and a few were of spontaneous onset. There was involvement of adjacent motor sensory nerves in about half of the patients. The prognosis was better following blunt trauma, stretch injuries and after a spontaneous onset. The anatomical relationships of the accessory nerve and aspects of the clinical picture and management are discussed.

INTRODUCTION: The spinal accessory nerve is the external terminal division of the 11th cranial nerve which passes postero-inferiorly in the neck, innervating the sternocleidomastoid and trapezius muscles. The spinal accessory nerve is often in the field of view of neck ultrasound imaging, however, can be overlooked and underappreciated during sonographic neck imaging and ultrasound-guided lymph node biopsies.
METHOD: A scoping review of the literature was conducted to assess current knowledge regarding sonographic imaging of the spinal accessory nerve. The authors' sonographic experience and

OBJECTIVE: Nerve transfers are an alternative in the reconstruction of traumatic brachial plexus injuries. In this study, we report the results of branchial plexus reconstruction using accessory to suprascapular nerve transfer.
METHODS: Thirty-three patients with traumatic brachial plexus injuries underwent surgical reconstruction with accessory to suprascapular nerve transfers. The patients were divided into groups in which surgery was performed either within 6 months after the injury or more than 6 months after the injury. Results were assessed using the Constant score.
RESULTS: There was

Since three-dimensional computed tomography was developed, many researchers have described accessory mental foramina. The anatomical and radiological findings have been discussed, but details of accessory mental nerves (AMNs) have only been researched in a small number of anatomical and clinical cases. For this article, we reviewed the literature relating to accessory mental foramina (AMFs) and nerves to clarify aspects important for clinical situations. The review showed that the distribution pattern of the AMN can differ according to the position of the accessory mental foramen, and the repo

The integrity of the spinal accessory nerve is fundamental to thoracoscapular function and essential for scapulohumeral rhythm. This nerve is vulnerable along its superficial course. This study assessed the delay in diagnosis and referral for management of damage to this nerve, clarified its anatomical course and function, and documented the results of repair. From examination of our records, 111 patients with lesions of the spinal accessory nerve were treated between 1984 and 2007. In 89 patients (80.2%) the damage was iatropathic. Recognition and referral were seldom made by the surgeon resp

BACKGROUND: Inadvertent injury of the spinal accessory nerve during surgical procedures is a cause of significant morbidity with medicolegal repercussions. Surface anatomy is an unreliable guide to the nerve's location. We suggest that ultrasound can be used to map the course of the nerve in the posterior triangle of the neck.
MATERIALS AND METHODS: Fifty healthy subjects (28 females, mean age 37 y) were scanned using a VF13-5 linear probe and a Siemens Sonoline Antares ultrasound machine (Siemens Medical Solutions USA Inc., Malvern, PA). The caliber, course, and distribution of the nerve in

Lesions of the spinal accessory nerve are usually iatrogenic, especially after lymph node extirpation on the neck. Between 1994 and 2003, 31 patients were operated on in the Neurosurgical Department of the University of Ulm for iatrogenic lesions of the XIth cranial nerve. Of 31 patients, 22 had undergone a previous lymph node extirpation, 2 had been injured during a selective peripheral denervation for spasmodic torticollis, and the other 7 patients by different causes. The neurosurgical intervention was performed 0-19 months after trauma (mean 7.2 months). All patients showed paresis/atrophy

BACKGROUND: Meningiomas arising from cranial nerves with no dural attachment are exceedingly rare. The authors present a patient with bilateral meningiomatous lesions originating symmetrically from both spinal accessory nerves.
CASE REPORT: A 61-year old woman presented with a one-year history of spinal ataxia and minimal left-sided motor impairment. Magnetic resonance imaging demonstrated two extrinsic lesions dorsolaterally of the medulla. Surgical exposure via a midline suboccipital approach with C1 laminectomy revealed the lesions arising from the spinal accessory nerves and in direct con

The spinal accessory nerve (nerve XI) constitutes a unique and readily encountered hazard in cutaneous and subcutaneous surgery of the head and neck. Presented is a simple method of using pinprick hyperaesthesia to accurately identify the course of this nerve. Delineating this nerve prior to performing surgery in the lateral neck and shoulder region is crucial to avoid the potentially disastrous complication of severing this nerve.

The paper deals with the pathogenesis of torticollis spastica and potential modes of their treatment. The findings indicate that changes in the root of nerve XI are essential in the pathogenesis of this abnormality. Despite comparatively small materials, much higher efficiency of micro-decompression of the accessory nerve is shown.

Microsurgical decompression of the accessory nerve was conducted in 10 patients with spasmodic torticollis (tonic and tonic-clonic forms). Compression of the roots of the accessory nerve of the posterior inferior cerebellar and the vertebral arteries was discovered. Decompression was accomplished by division of the arachnoid adhesions, anastomosis with sensory roots of C1 and C2, and placing a layer of muscular tissue between the nerve and the vessel. The results of treatment of spasmodic torticollis are analysed and the efficacy of neurovascular decompression is emphasized.

INTRODUCTION: In this study we sought to determine whether standard electrophysiological testing of the spinal accessory nerve (SAN) accurately identifies patients who would benefit from surgical repair.
METHODS: Sixteen consecutive patients sent for surgical evaluation of unilateral SAN injury were studied clinically and electrophysiologically.
RESULTS: All patients demonstrated a low-amplitude SAN compound muscle action potential (CMAP) that required a higher stimulus intensity to obtain it than on the unaffected side. Upper trapezius needle electromyography showed dense fibrillation poten

OBJECTIVE: To describe a new method of accessory nerve defect reconstruction with sternocleidomastoid muscle-great auricular flap.
METHODS: Thirty-four cases receiving traditional radical neck dissection were divided into two groups: single neck dissection group (n = 19) and accessory nerve reconstruction group (n = 15). Surgical procedure of the reconstruction was described in detail. Postoperative shoulder functions were compared between the two groups.
RESULTS: Accessory nerve reconstruction group experienced much better shoulder function recovery than that in single neck dissection group

There has been the controversy surrounding the cranial root (CR) of the accessory nerve. This study was performed to clarify the morphological characteristics of the CR in the cranial cavity. Fifty sides of 25 adult cadaver heads were used. The accessory nerve was easily distinguished from the vagus nerve by the dura mater in the jugular foramen in 80% of 50 specimens. The trunk of the accessory nerve from the spinal cord penetrated the dura mater at various distances before entering the jugular foramen. In 20% of the specimens there was no dural boundary. In these cases, the uppermost cranial

It was recently reported that the internal branch of the accessory nerve not only comprises the cranial root but also various combinations of the cranial root, spinal root, and the vagal component of the vagus nerve. The aim of this study was to demonstrate the anatomical distribution of each component of the internal branch of the human accessory nerve. Ten half-heads and necks of adult cadavers were used. The internal branch of the accessory nerve had three courses: the pharyngeal branch, the descending branch to the thorax, and the recurrent laryngeal nerve. The pharyngeal branch of the int
