To Trim or Not to Trim: Revisiting Acetabuloplasty During Hip Arthroscopy for Femoroacetabular Impingement and Acetabular Overcoverage.

PURPOSE OF REVIEW: Acetabuloplasty is a common component of hip arthroscopy for pincer-type and mixed femoroacetabular impingement (FAI), yet the optimal degree of correction remains controversial. Historically, rim trimming has often been guided by radiographic normalization, particularly of the lateral center-edge angle (LCEA). The purpose of this review is to synthesize current evidence regarding acetabular morphology, radiographic assessment, clinical outcomes related to the degree of acetabular correction, and intraoperative strategies that support optimal correction during acetabuloplast

Analysis of osteonecrosis following Pemberton acetabuloplasty in developmental dysplasia of the hip: long-term results.

BACKGROUND: The favorable results of Pemberton acetabuloplasty in children with developmental dysplasia of the hip have been well reported. We reviewed our long-term results related to osteonecrosis of the femoral head after this surgery, especially with regard to the effect of excessive inferior displacement of the femoral head. METHODS: From 1993 to 1997, we performed 167 Pemberton acetabuloplasties in patients with developmental dysplasia of the hip who were eighteen months of age or older. Patients who had had prior treatment or developmental dysplasia of the hip due to neuromuscular dise

[New results of a modified acetabuloplasty after Lance combined with detorsion-varus-osteotomy (author's transl)].

The results of 254 Lance-acetabuloplasties in a modified technique combined withe detorsion-varus-osteotomy have been analysed. The children were operated upon in the years 1970-1974. Regarding the acetabulum the results were as follow: 1. In children between 1 and 6 years a normal acetabular index (as compared with the Normal Values of Tönnis and Brunken) was achieved in 78.3%, a slightly dysplastic index in 14.8%. In 6.6% a severe dysplasia remained. 2. In children between 6 and 10 years a normal acetabular index was found in 59.4%, a slightly dysplastic index in 24.3% and an index as classi

Comparison of Lateral Shelf Acetabuloplasty in the Treatment of Legg-Calvé-Perthes Disease in Older Children.

BACKGROUND: In older children with Perthes disease, where containment surgery is often indicated, the comparative effectiveness of the 2 principal shelf acetabuloplasty techniques, Staheli and Tectoplasty, remains unclear. METHODS: A total of 26 children (23 males and 3 females; mean age at surgery, 8.55±0.95 y) underwent the Staheli technique, and 20 children (18 males and 2 females; mean age at surgery, 8.65±0.89 y) were treated with the Tectoplasty technique. Clinical data and radiographs were retrospectively reviewed. Radiographic parameters, including the center-edge angle, Sharp angle,

Shelf acetabuloplasty in children diagnosed with osteonecrosis of the femoral head secondary to acute lymphoblastic leukaemia.

BACKGROUND: Shelf acetabuloplasty (SA) is a pelvic augmentation procedure often described in treatment of children diagnosed with Perthes disease of hip. This retrospective study is the first to report outcome in osteonecrosis of the femoral head (ONFH) secondary to acute lymphoblastic leukaemia (ALL). PATIENT AND METHODS: All patients treated with SA in duration between 2017 and 2024 were included except for those who had follow-up less than one year following surgery. Patients selected for SA were those aged between 6 and 13-y with stage B or C collapse of the femoral head according to later

Arthroscopic reduction and acetabuloplasty for the treatment of dislocated hips in children of walking age: a preliminary report.

INTRODUCTION: This paper presents our initial findings on the use of arthroscopic hip reduction in conjunction with acetabuloplasty to treat patients of walking age who had congenital dislocation of the hip. MATERIALS AND METHODS: Arthroscopic reduction with acetabuloplasty was performed on nine children of walking age with nine dislocated hips. The average age of the patients was 21.4 months (14-41 months). Arthroscopic reduction was performed using a two-portal method with a 2.7-mm mini cannulated arthroscopic instrument. We used a subadductor portal and a high anterolateral portal. The obs

Quality of life in patients following percutaneous PMMA acetabuloplasty for acetabular metastasis due to carcinoma.

Pain is the main symptom of acetabular osteolysis from metastatic carcinoma, and it is frequently associated with pathologic fractures. Radiotherapy alone usually is not effective for pain control, and it does not allow weight bearing. Surgical procedures requiring an aggressive approach and consequently high morbidity are rarely indicated in patients with multiple metastases. Polymethylmethacrylate (PMMA) has been widely used in oncologic and spinal surgery as a bone filler. We evaluated the functional results of percutaneous injection of PMMA in 20 patients (24 acetabula) with acetabular ost

Combined acetabuloplasty and varus derotation osteotomy in congenital dislocation of the hip. Long-term results.

Twenty-two patients (33 hips) with congenital dislocation of the hip were examined at an average of 26 years after combined acetabuloplasty and intertrochanteric varus derotation osteotomy. Seventeen patients had no problems despite physically demanding jobs and sporting activities in some cases. The CE angle was improved to the lower end of the normal range. The shaft-neck angle was corrected by 31 degrees to 120 degrees. Restoration to the normal valgus subsequently occurred and we found no evidence of subcapital coxa valga. Although there was an increase in the neck epiphysis angle 5 years

Total hip arthroplasty with shelf acetabuloplasty in dysplastic coxarthrosis, mean follow-up of 7 years.

UNLABELLED: Developmental dysplasia of the hip is the cause of approximately one third of secondary coxarthrosis. Anatomy alterations make it difficult to place a total hip prosthesis in its anatomical position and for it to be stable in the long term; there are several techniques to achieve this goal. In the present work, we used autograft of the femoral head (shelf graft or reinforced roof), to improve the coverage of the acetabular component with favorable results. MATERIALS AD METHODS: Sixteen cases were included in 14 patients with a diagnosis of developmental dysplasia of the hip (13 wo

Natural history of shelf gap and paleo-acetabular depth after shelf acetabuloplasty for Perthes disease.

The purpose of this study was to evaluate the morphologic evolution of the shelf gap, acetabulum and femoral head after shelf acetabuloplasty in patients affected by Perthes disease. 54 patients operated for Perthes disease with shelf acetabuloplasty were retrospectively reviewed regarding the radiographic results. Three pelvic antero-posterior radiographs have been studied for each patient, one at 2 postoperative months, one at 1 postoperative year and one at the latest clinical follow up (mean 76 postoperative months). The shelf gap decreased from 108% at 2 months to 104% at the last follow-

Comparison of acetabular anterior coverage after Salter osteotomy and Pemberton acetabuloplasty: a long-term followup.

BACKGROUND: The Salter osteotomy and Pemberton acetabuloplasty are common procedures for a deficient acetabulum in patients with developmental dysplasia of the hip. However, the degree of increasing retroversion and anterior acetabular coverage of these two procedures remains unanswered. QUESTIONS/PURPOSES: The purpose of this study is to show the change in anterior coverage and relevant parameters in measuring pain and function among patients who have undergone either a Salter osteotomy or Pemberton acetabuloplasty. METHODS: Forty-two patients who underwent either a Salter or Pemberton proc

[Long-term results following acetabuloplasty using cortico-spongiosa banked chips in hip dysplasia and dislocation].

Analysed are altogether 119 acetabuloplasties correcting acetabular fossae using corticospongiose bankchips. The operations of the years from 1975-1983 have been used for this analysis. Examined are by clinical progress and serial roentgenography the compatibility, the resistance to mechanical stress as well as building in and transformation of the corticospongiose bankchips. Finally is determined the progress of the socket by using the socket-roof- and centre-corner-angle postoperative on an average of 10 years.

Excessive external rotation of the ipsilateral iliac wing following Pemberton acetabuloplasty: a three-dimensional computed tomography study.

BACKGROUND: Emerging evidence suggests that developmental dysplasia of the hip involves not only localized acetabular dysplasia but also internal rotation of the entire innominate bone. Pemberton acetabuloplasty is widely used to improve acetabular coverage in walking-age children and generally yields favorable radiographic and clinical outcomes. However, the effect of this procedure on the global rotational alignment of the innominate bone has not been systematically investigated. METHODS: This retrospective comparative study included patients with a primary diagnosis of developmental dyspla

Shelf acetabuloplasty for Perthes disease in patients older than eight years of age: an observational cohort study.

This is a retrospective review of 24 patients with late-onset Perthes disease treated with shelf acetabuloplasty. The mean age of the patients was 9.8 years. The medial joint space ratio improved from 1.66 to 1.47 (P<0.05) and the acetabular cover ratio improved from 0.87 to 1.18 (P<0.001). At maturity, five hips were Stulberg II, 13 were Stulberg III and six were Stulberg IV. The study suggests that shelf acetabuloplasty favourably affects long-term Stulberg grading and therefore outcome compared with historic nonoperative treatment regimens. We believe that the procedure deserves further eva

Balloon acetabuloplasty for cartilage impaction reduction in traumatic acetabular fractures: a case series and technical note.

PURPOSE: Acetabular fractures with cartilage impaction are associated with a high risk of post-traumatic osteoarthritis, frequently requiring total hip arthroplasty (THA). Conventional open reduction and internal fixation (ORIF) often fail to restore joint surface congruity. This case series evaluates the radiological and clinical outcomes of a novel minimally invasive technique using balloon acetabuloplasty to reduce dome impaction in traumatic acetabular fractures. METHODS: This retrospective case series included patients with acetabular fractures involving cartilage impaction. Balloon acet

Influence of shelf acetabuloplasty on the outcomes of total hip arthroplasty in hips with dysplasia: a case-control study.

INTRODUCTION: Total hip arthroplasty (THA) in young adults, especially in those with hip dysplasia, is affected by wear and acetabular fixation problems. Shelf acetabuloplasty is performed to delay THA in patients with acetabular dysplasia. Thus, we conducted a retrospective, continuous case-control study at a single healthcare facility to analyze (1) the influence of prior shelf acetabuloplasty on the survival of a subsequent THA and (2) the functional outcomes. MATERIALS AND METHODS: We evaluated 105 patients (124 THA) who underwent THA due to hip dysplasia: 54 patients (61 THA) were includ
