The burden of gout in acne keloidalis nuchae-Insights from a population-based study.

BACKGROUND: The risk of gout amid patients with acne keloidalis nuchae (AKN) has not been investigated in the past. OBJECTIVE: To assess the risk of developing gout among patients with AKN relative to control subjects. METHODS: A population-based retrospective study followed patients with AKN (n = 2677) and age-, sex-, and ethnicity-matched control subjects (n = 13 190). The incidence of new-onset gout was compared between the two groups. Hazard ratio (HR) for the risk of gout was obtained using a multivariate Cox regression model. RESULTS: The incidence rate of gout was 1.12 (95% CI, 0.68-

Keloid and Hypertrophic Scars Are the Result of Chronic Inflammation in the Reticular Dermis.

Keloids and hypertrophic scars are caused by cutaneous injury and irritation, including trauma, insect bite, burn, surgery, vaccination, skin piercing, acne, folliculitis, chicken pox, and herpes zoster infection. Notably, superficial injuries that do not reach the reticular dermis never cause keloidal and hypertrophic scarring. This suggests that these pathological scars are due to injury to this skin layer and the subsequent aberrant wound healing therein. The latter is characterized by continuous and histologically localized inflammation. As a result, the reticular layer of keloids and hype

Management of severe acne.

Acne is the most common skin disease, affecting up to 95% of adolescents. Severe episodes of acne can cause considerable physical and psychological scarring, and overexpression of transforming growth factor-β can lead to formation of hypertrophic scars and keloids. The severity of acne in adolescence is associated with a positive history of severe acne in first-degree relatives, especially the mother. In most cases acne is a chronic disease, and it is often a component of systemic diseases or syndromes. All forms of severe acne require systemic treatment. The available options include oral ant

Refractory acne keloidalis nuchae treated with radiotherapy.

Acne keloidalis nuchae (AKN) is difficult to treat, and the clinical response is usually poor. Few reports have been published about scarring alopecia treated with radiotherapy (RT). We report a case of AKN that had been refractory to various other treatments but which responded to RT with excellent clinical and cosmetic results.

Acne: treatment of scars.

Acne is a common disease affecting a significant proportion of the population.(1-3) It causes considerable morbidity through soreness, disfigurement, and social handicap due to inflammatory lesions.(4,5) Modern therapy ensures that a considerable proportion of all patients can be offered effective treatment of their disease, but the morbidity is not restricted solely to the inflammatory lesions of acne.(6,7) Despite appropriate and effective primary prevention of scarring, scarring occurs in some degree in 95% of all patients irrespective of the severity of inflammatory acne (although severe a

Acne and Rosacea: Special Considerations in the Treatment of Patients With Latin American Ancestry.

Acne is a common disease among patients with Latin American ancestry. Its presentation is very similar to that in all skin types, but nodulocystic acne is more frequent in patients with oily and darker skin than in white Caucasians. Acne sequelae in patients with Latin American ancestry and with darker skin include postinflammatory hyperpigmentation (PIH) and atrophic and hypertrophic scars or keloids, with PIH being the most common complication affecting the quality of life of patients. Lately, more attention has been paid to rosacea in patients with darker skin. It has been seen that some of

Microneedle delivery of botulinum toxin type A combined with hyaluronic acid for the synergetic management of multiple sternal keloids with oily skin: A retrospective clinical investigation.

BACKGROUND: For the treatment of sternal keloids, corticosteroid therapy has side effects including abnormal sebum secretion and acne. Relapse of keloids is common after corticosteroid injection in patients with oily skin. To reduce side effects and keloid recurrence, we used a combination of botulinum toxin type A (BTX-A) and hyaluronic acid (HA) as synergetic management for multiple sternal keloids in patients with oily skin. METHODS: In total, 58 patients with multiple sternal keloids who received monthly steroid injections were retrospectively included. Thirty-two patients in the interven

Surgical planing of the skin; dichlorotetrafluoro-ethane as a freezing agent.

Surgical skin planing is, in the hands of an experienced operator, a safe and highly effective procedure for treating a number of cutaneous defects, most notably pitted acne scars. The operation is facilitated by the use of a new instrument (jet-spray handpiece) which allows the operator to freeze the skin and plane it almost simultaneously, and by a new freezing agent, dichlorotetrafluoro-ethane, which adds to the safety by eliminating the old hazards of inflammability, explosion, and the toxic inhalation of ethyl chloride. The ability to sharply differentiate between keloid and hypertrophic

Follicular and scarring disorders in skin of color: presentation and management.

Skin of color, also known as ethnic skin, is described as skin of individuals of African, Asian, Hispanic, Native-American, Middle Eastern, and Pacific Island backgrounds. Differences in hair morphology, hair grooming, cultural practices, and susceptibility to keloid scarring exist within these populations and have been implicated in hair, scalp, and skin disorders. Acne keloidalis (AK), central centrifugal cicatricial alopecia (CCCA), dissecting cellulitis of the scalp (DCS), pseudofolliculitis barbae (PFB), traction alopecia (TA), and keloids are the most prevalent follicular and scarring di

Treatment of acne scarring.

Acne scarring is a common and expected result of moderate to severe acne vulgaris. Given the clinical variety of acne scars and the plethora of treatment options available, management of cutaneous scarring from acne can be challenging and confusing. This article discusses the pathophysiology of acne and acne scarring to better understand its biologic and structural nature. A simple, yet practical classification schema is presented, allowing caregivers to better organize their assessment of acne scarring and develop useful management strategies from this model. This article highlights the vario

The management of postacne scarring.

BACKGROUND: Therapeutic intervention for postacne scarring has historically been limited by the considerable morbidity of most treatments for only marginal disease improvement. Within the past decade, however, a greater understanding of the pathogenesis of acne scarring has led to the development of techniques that offer more favorable risk-benefit profiles. OBJECTIVE: The aims of this article are to highlight a number of newer techniques and to assign their appropriateness to particular grades of acne scarring. MATERIALS AND METHODS: Current modalities are discussed as they relate to diseas

[Advancement in phodynamic therapy for non-neoplastic skin diseases].

Photodynamic therapy is mainly used in dermatology to treat skin tumors, precancerous lesions, and condyloma acuminatum. Due to its excellent tissue selectivity, easy operation and good cosmetic effect, it has been gradually applied to the treatment of various non-neoplastic skin diseases, such as verruca acuminata, acne, rosacea, chronic skin ulcer, fungal diseases, keloid, and so on. Here is the review on the advancement in photodynamic therapy for non-neoplastic skin diseases.

Keloid Formation and Any Skin Complications in Patients Treated With Isotretinoin and Undergone Any Skin-Related Procedures.

BACKGROUND: Isotretinoin is widely used for moderate to severe acne vulgaris. Despite its broad application, isotretinoin carries a risk of permanent scarring and keloid formation following various skin procedures. As a result, a delay of at least 6-12 months after completing or discontinuing isotretinoin treatment is commonly recommended before undergoing skin procedures. AIMS: This study aims to evaluate the necessity of delaying skin procedures performed concurrently with or soon after isotretinoin treatment at different dosages in patients with acne vulgaris, based on the dermatological s

Cross-Sectional Study of Beliefs, Perceptions, Knowledge of Treatment Modalities and Willingness to Pay for Acne Scar Treatment in Acne Patients.

BACKGROUND AND AIMS: There are limited data on the use of different treatment modalities for acne scar in Nigeria despite their widespread availability. Cost of treatment may limit the number treated despite treatment being desired by patients. Also, it is not known, what the Nigerian patient is willing to pay for these treatments. The aim of this study was to assess: perceptions and attitude towards acne scars, determine awareness that acne scars can be treated, determine the knowledge of acne scar treatment modalities and the willingness to pay for these treatments. METHODOLOGY: Cross secti

Acne in ethnic skin: special considerations for therapy.

Acne vulgaris occurs in people of all ethnicities and races. Although the pathophysiology and treatment options are similar in all skin phototypes, darker-skinned patients have higher incidence rates of two sequelae of acne: postinflammatory hyperpigmentation and keloidal scarring. Postinflammatory hyperpigmentation may also be triggered by skin irritation. In choosing therapies for patients of color, therefore, clinicians must find a balance between aggressive early intervention to target inflammatory acne lesions, and gentle treatments to increase tolerability and avoid skin irritation. Acne keloidalis nuchae: another cutaneous symptom of metabolic syndrome, truncal obesity, and impending/overt diabetes mellitus?

Acne keloidalis nuchae (AKN) is an inflammatory disease related to the spectrum of keloidal scarring alopecias. No association with internal disease has been described so far. We describe a case series of four Indian patients who showed an association of AKN with acanthosis nigricans, a skin marker for the metabolic syndrome (in particular obesity). Well known risk factors for AKN such as curly hair or wearing of a helmet were not present in these patients. AKN shows some similarities to acne inversa. We propose that AKN may be considered a cutaneous marker for the metabolic syndrome but case-
