A hybrid acromioclavicular joint fracture-dislocation in which an intra-articular fracture of the distal clavicle coexisted with a coracoclavicular ligament injury is described. The proposed mechanism of injury is a fall on the point of the shoulder that simultaneously drives the scapula both anteriorly and inferiorly. Forced anterior scapular protraction caused a displaced horizontal plane fracture of the inferior half of the distal clavicle, which was separated from the remainder of the clavicle. The inferiorly acromioclavicular ligament was still attached to this distal clavicle fragment. T

Synovial chondromatosis involving the acromioclavicular joint is very rare. We report a case of synovial chondromatosis of the acromioclavicular joint associated with synovial cyst formation. Arthrography of the acromioclavicular joint revealed that the loose bodies lay within a synovial cyst which was in communication with the acromioclavicular joint. Synovectomy including removal of the synovial cyst and the loose bodies relieved symptoms completely. The patient was still pain- and recurrence-free at 4 years' follow-up.

Synovial chondromatosis involving the acromioclavicular joint occurred in a 20-year-old woman. Synovectomy including removal of loose bodies and resection of the distal end of the clavicle relieved symptoms. Synovial chondromatosis is a relatively rare disorder and seems not to have been reported in the acromioclavicular joint.

Tracking movement in the scapular plane and electromyograms from the upper trapezius, rhomboideus, middle deltoideus, and pectoralis major muscles were analyzed during the movement in ten subjects who had grade II dislocation of the acromioclavicular joint. The purpose of this study was to clarify whether or not the dislocation of the acromioclavicular joint affects the rhythmic movement of the shoulder joint, and to evaluate objectively fatigue on the shoulder girdle of the dislocated side. The dislocation of acromioclavicular joint affects the fast rhythmic movement of the shoulder joint fro

BACKGROUND: Acromioclavicular joint (ACJ) space narrowing has been considered to be an important diagnostic image parameter of ACJ osteoarthritis (ACJO). However, the morphology of the ACJ space is irregular because of osteophyte formation, subchondral irregularity, capsular distention, sclerosis, and erosion. Therefore, we created the ACJ cross-sectional area (ACJCSA) as a new diagnostic image parameter to assess the irregular morphologic changes of the ACJ.
AIM: To hypothesize that the ACJCSA is a new diagnostic image parameter for ACJO.
METHODS: ACJ samples were obtained from 35 patients

UNLABELLED: Septic arthritis of native joints is uncommon, but the condition can be threatening to life and limb if left untreated.1 Septic arthritis of the acromioclavicular (AC) joint of the shoulder is particularly rare and has only appeared sparsely in medical literature, mainly through individual case reports. Early recognition and treatment of the condition is vital, but diagnosis of septic AC arthritis can be difficult due to its presentation with vague symptoms and nonspecific laboratory findings. This case report describes the care of a patient with poorly managed diabetes who present

BACKGROUND: Suspension stabilization is a commonly used surgical method for acute acromioclavicular (AC) joint dislocation. However, a failure rate of around 26% caused various emerging surgical techniques and recommendations. We aimed to identify the significance of postulated factors relevant to the radiographic success of AC joint stabilization.
METHODS: The data of patients with acute AC joint dislocation treated with suspension stabilization in our institution from January 2012 to December 2022 were reviewed. Zanca views x-ray films of both AC joints were reviewed including the preoperat

Background: Optimal treatment for high-grade acromioclavicular (AC) joint dislocations is still not unanimous. Improving horizontal AC stability has been emphasized in recent years. Biomechanical studies and computer simulations have demonstrated that adequate horizontal stability could be restored with an additional AC fixation. We aim to prospectively investigate if AC augmentation leads to better clinical and radiological results. Methods: A total of 70 patients with a mean (± SD) age of 42 ± 11 years with acute AC joint dislocation Rockwood type IIIb and V were prospectively randomized int

BACKGROUND: Acromioclavicular (AC) joint (ACJ) dislocation can lead to superior clavicular instability when the AC and coracoclavicular (CC) ligaments are torn. No previous study has assessed the effects of combined AC-CC ligament resections in fresh-frozen cadavers with preserved soft tissues around the thorax and shoulder girdle. This study aimed to develop such an ACJ dislocation model and evaluate stability following ligament resections.
METHODS: Nine fresh-frozen cadaver shoulders (mean age, 86.6 years) without clavicular fractures or ACJ osteoarthritis were used. Each specimen included

OBJECTIVES: We evaluated the long-term clinical results of acute complete acromioclavicular dislocations treated with micro-movable and anatomical acromioclavicular plate.
METHODS: Open reduction and internal fixation was performed using the MAAP in 16 patients (10 males, 6 females; mean age 36 years; range 16 to 63 years) with acute complete acromioclavicular joint dislocation. Radiographic evaluations were routinely conducted every 3 weeks until 3 months postoperatively. The MAAP were removed under local anesthesia after 3 months postoperatively. We evaluated the functional results by using

PURPOSE: Optimal treatment of chronic unstable acromioclavicular (AC) joint dislocations (stage 3-5 according the Rockwood classification) is still debated. Anatomic coracoclavicular (CC) reconstruction is a reliable option in terms of two-dimensional radiographic reduction, clinical outcomes, and return to sports, but there remain concerns regarding anterior-posterior stability of the AC joint with CC ligament reconstruction alone. The aim of the present study was to describe the mid-term results of a new hybrid technique with CC and AC ligament reconstruction for chronic AC joint dislocation

Injuries to the acromioclavicular (AC) joint are common and comprise ∼12% of shoulder injuries. Stability to the AC joint depends on the coracoclavicular (CC) ligaments for vertical stability and AC ligaments and capsular structures for horizontal stability. Injuries to the AC ligaments can lead to horizontal instability of the AC joint. There is no gold standard technique for treating these injuries surgically, and many of the described procedures focus on vertical instability rather than horizontal instability. This article describes an open AC ligament reconstruction with semitendinosus all

Acromioclavicular joint (ACJ) dislocations are a common cause of pain in patients of any age. Athletes who participate in contact sports, such as hockey, football, rugby, and soccer, are particularly susceptible to such injuries. The ACJ has an important role in the function of the upper limb, and its complexity of movement makes it susceptible to acute injuries and chronic dysfunction with debilitating effects that must be treated appropriately and promptly to preserve function. Recently, ACJ has received increasing attention due to the development of new surgical techniques for the restorati

Multiple papers have described aspects of treatment of acromioclavicular (AC) joint injuries. Most have emphasized aspects of surgical treatment, and some papers have addressed non-operative treatment. Few papers have highlighted the specific pathoanatomy of an AC joint injury or have described methods of evaluating the 3-dimensional pathomechanics resulting from the pathoanatomical injury. This paper is based on 3 observations: (1) AC joint injuries exist and present on a spectrum of pathoanatomy; (2) The effect of the pathoanatomy on normal AC joint mechanics to produce pathomechanics is dep

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2019eng
PURPOSE: To report the outcomes of all-arthroscopic coracoclavicular (CC) ligament reconstruction and simultaneous diagnosis and treatment of glenohumeral pathologies in patients with symptomatic, chronic (>6 weeks), complete (Rockwood type III-V) acromioclavicular joint (ACJ) separations.
METHODS: We prospectively followed up 57 consecutive patients treated arthroscopically for chronic Rockwood type III (n = 11), type IV (n = 19), and type V (n = 27) ACJ dislocations. Previous ACJ surgery failed in 11 (19%). The mean delay between injury and surgery was 39 months (range, 6 months to 17 years

An unusual case of a patient presenting with a large infected haematoma following a traumatic grade II acromioclavicular joint dislocation is reported. Diagnosis of this rare complication, of an otherwise common self-limiting injury, was delayed until 19 days postinjury despite several presentations during this time with worsening swelling and pain. The patient was found to have significant tissue destruction by the time washout was performed and required multiple procedures to treat the infection. This case highlights the need for a high index of suspicion for complications, even following co

BACKGROUND AND PURPOSE: Surgical treatment of chronic acromioclavicular joint dislocations is challenging, and no single procedure can be considered to be the gold standard. In 2010, the GraftRope method (Arthrex Inc., Naples, FL) was introduced in a case series of 10 patients, showing good clinical results and no complications. We wanted to evaluate the GraftRope method in a prospective consecutive series.
PATIENTS AND METHODS: 8 patients with chronic Rockwood type III-V acromioclavicular joint dislocations were treated surgically using the GraftRope method. The patients were clinically eval

AC (acromioclavicular) joint dislocations are a common injury seen by physicians. Symptoms range from minor discomfort with activity to complete disability of the extremity. Although most orthopaedic surgeons agree on how to treat either mild (type 1-2) or severe (type 4-6) injuries, there is no consensus for treatment of type 3 injuries. This article reviews the relevant literature pertaining to the anatomy of the injury, evaluation of the patient, pertinent imaging as well as the controversial management of type 3 AC joint dislocations. With improvement in surgical techniques over the past 3

PURPOSE: We present a new technique of arthroscopic-assisted AC-hook plate fixation for acromioclavicular joint dislocation with all the advantages of minimally invasive surgery and the possibility to treat concomitant pathologies.
METHODS: Initially a glenohumeral arthroscopy is performed to address concomitant intra-articular injuries. Under subacromial visualisation the drill hole for the hook of the plate can be exactly positioned in the acromion. The hook plate is put in place under visual control.
RESULTS: The initial results (n = 3) are promising with good to excellent results in the
