Post-tetanic transcranial motor evoked potentials augment the amplitude of compound muscle action potentials recorded from innervated and non-innervated muscles.

BACKGROUND CONTEXT: Transcranial electrical stimulation used to produce motor evoked potentials (TES-MEPs) and subsequent compound muscle action potential (CMAP) recording is widely used to monitor motor function during surgery when there is risk of damaging the spinal cord. Nonetheless, some muscles do not produce CMAP amplitudes sufficient for intraoperative monitoring. PURPOSE: This study aimed to investigate the utility of tetanic stimulation at single and multiple peripheral nerve sites for augmenting CMAP amplitudes recorded from innervated and non-innervated muscles. Longitudinal Characterization of Compound Action Potentials in Chronic Vagus Nerve Recordings in Mice.

The vagus nerve (VN) mediates bidirectional communication between the body and brain to maintain physiological homeostasis; likewise, alterations in ongoing vagal signaling may be indicators of disease and/or contribute to disease pathogenesis. Even though extensively documented in acute experiments, ongoing vagal activity has not been characterized longitudinally, over days or weeks, in mice, a preferred preclinical model. In addition, even though many VN recordings in mice occur during anesthesia, the effects of anesthesia on vagal signaling are unknown. This study uses a chronic implant mou

Diaphragmatic Compound Muscle Action Potentials Correlate With the Clinical Severity of Myasthenia Gravis.

INTRODUCTION/AIMS: Patients with myasthenia gravis (MG) often experience impairment of respiratory function. Some case reports have explored the diaphragmatic compound muscle action potential (DCMAP) amplitudes in patients with MG using single-stimulus phrenic nerve conduction studies. Herein, we aimed to investigate whether the DCMAP amplitude correlated with the clinical severity of MG and whether this improved with treatment. METHODS: We retrospectively evaluated patients with MG who underwent right phrenic nerve conduction studies. We evaluated the patients' clinical characteristics and t

Electrically evoked compound action potentials in cochlear implant users with preoperative residual hearing.

INTRODUCTION: Residual hearing in cochlear implant (CI) candidates requires the functional integrity of the nerve in particular regions of the cochlea. Nerve activity can be elicited as electrically evoked compound action potentials (ECAP) after cochlear implantation. We hypothesize that ECAP thresholds depend on preoperative residual hearing ability. MATERIALS AND METHODS: In a retrospective study, we analyzed 84 adult cochlear implant users who had received a Nucleus® CI632 Slim Modiolar Electrode and who preoperatively had had residual hearing. Inclusion criteria were severe to profound he

Double peak sensory nerve action potentials to single stimuli in nerve conduction studies.

INTRODUCTION: In humans, sensory nerve action potentials (SNAPs) can show 2 separate deflections, i.e., double peak potentials (DPp), which necessarily means that 1 peak is delayed with respect to the other. DPps may have various origins and be due to either physical or physiological properties. METHODS: We review the nature of commonly encountered DPps in clinical practice, provide the most likely interpretations for their physiological origin, and assess their reproducibility and clinical utility. RESULTS: We classified the DPps into 3 categories: (1) simultaneous anodal and cathodal stimu

Factors Influencing Diaphragmatic Compound Motor Action Potentials During Cryoballoon Ablation for Atrial Fibrillation.

INTRODUCTION: The diaphragmatic compound motor action potentials (CMAPs) have been used to predict and prevent phrenic nerve injury (PNI) during cryoballoon ablation of right pulmonary veins. We sought to assess factors that influence the amplitude of the surface CMAP recordings. METHODS AND RESULTS: We analyzed CMAPs from consecutive patients undergoing cryoballoon ablation for paroxysmal atrial fibrillation. CMAP recordings were obtained using electrocardiography electrodes positioned in the "modified lead I" method while stimulating the right PN, until loss of capture (ascertained by palpa

Intact Heart Loose Patch Photolysis Reveals Ionic Current Kinetics During Ventricular Action Potentials.

RATIONALE: Assessing the underlying ionic currents during a triggered action potential (AP) in intact perfused hearts offers the opportunity to link molecular mechanisms with pathophysiological problems in cardiovascular research. The developed loose patch photolysis technique can provide striking new insights into cardiac function at the whole heart level during health and disease. OBJECTIVE: To measure transmembrane ionic currents during an AP to determine how and when surface Ca(2+) influx that triggers Ca(2+)-induced Ca(2+) release occurs and how Ca(2+)-activated conductances can contribu

Androgens block outward potassium currents and decrease spontaneous action potentials in GH3 cells.

Androgens produce nongenomic effects in several cells by different mechanisms, including ion channel modulation. Adenohypophyseal cells express several K(+) channels, including voltage and Ca(2+)-dependent K(+) (BK) channels, which might be the target of androgens to modulate cellular action potentials and hormonal secretion. Androgen effects were studied in GH3 cells (from anterior pituitary rat tumor) by means of the patch-clamp technique. Cells were continuously perfused with saline solution, in the absence or presence of the androgens studied, while applying 40 mV pulses of 400 ms from a h

Different calcium sources control somatic versus dendritic SK channel activation during action potentials.

Small-conductance calcium-activated potassium (SK) channels play an important role in regulating neuronal excitability. While SK channels at the soma have long been known to contribute to the medium afterhyperpolarization (mAHP), recent evidence indicates they also regulate NMDA receptor activation in dendritic spines. Here we investigate the activation of SK channels in spines and dendrites of rat cortical pyramidal neurons during action potentials (APs), and compare this to SK channel activation at the soma. Using confocal calcium imaging, we demonstrate that the inhibition of SK channels wi

Monophasic action potentials of the right atrium in patients with paroxysmal atrial fibrillation.

To investigate the mechanism of atrial fibrillation (AF), monophasic action potentials (MAPs) from the atrial myocardium were studied in 7 patients with paroxysmal AF (PAF) and in 7 control individuals. The MAPs were recorded using a contact catheter during sinus rhythm and continuous pacing at the high right atrium (HRA) with pacing cycle lengths of 600, 500 and 400 ms. MAPs were obtained from 6 sites in each participant. The MAPD90 was measured from onset to 90% of MAP repolarization. Average, maximal and minimal MAPD90 (avMAPD90, maxMAPD90 and minMAPD90) were obtained from all participants.

Calcium transients evoked by action potentials in the somata of chick ciliary neurons.

The effect of action potentials on the calcium concentration in the somata of chick ciliary neurons ([Ca2+]s) was determined by loading these with the calcium indicator calcium green-1. Following trains of 1-10 impulses (30 Hz) to the postganglionic nerve, the [Ca2+]s increased rapidly and then declined along a single exponential with a time constant of 0.70 +/- 0.04 s (fast phase). After trains of 20 or 50 impulses, the elevated [Ca2+]s declined as the sum of two exponentials, with time constants of 0.78 +/- 0.12 s (fast phase) and 4.0 +/- 0.4 s (moderate phase). After a 600-impulse postgangl

Hearing preservation in acoustic neuroma surgery: value of monitoring cochlear nerve action potentials.

This study reviews the hearing results in 80 consecutive patients who underwent complete removal of histologically proven acoustic neuromas by use of the suboccipital approach. Of these, 56 patients had successful monitoring of cochlear compound action potentials; 20 were not monitored because their surgery predated monitoring; and 4 had unsuccessful monitoring. A significant difference was found in hearing preservation rates between the group in whom compound action potential monitoring was performed and those in whom monitoring was either unavailable or failed (p = 0.02). Overall, 38% (30 of

[Sensory action potentials of ring finger in the diagnosis of carpal tunnel syndrome].

Amplitudes and latencies of sensory action potentials (SAPs) recorded over the index, middle, little and ring fingers by antidromic stimulation of the median or ulnar nerve at wrist were measured in 65 normal adults and 78 patients with clinically and electrophysiologically verified carpal tunnel syndrome. In the normal adults, there was no significant difference among the SAP latencies of the index, middle, ring and little fingers. Among these 65 normal adults, the difference between median and ulnar SAPs of ring finger was less than 0.4 msec in 64 subjects, but 0.7 msec in one. In the patien

[Measurement of single action potentials of the dorsal nerve roots in the human].

The method of recording single unit potentials with wire electrodes in animals has been applied to humans. Using two pairs of special electrodes it was possible to record extracellular single unit potentials at two different places in nerve root filaments in the spinal canal with the dura open and to determine conduction velocities. This method may be used in the future for the identification of afferent fibres in the spinal canal during the operation.

Exploring applications of electrically evoked compound action potentials using the vestibulo-cochlear implant.

OBJECTIVES: Vestibulo-cochlear implants are a potential treatment approach for patients with loss of vestibular function. A critical aspect is the development of reliable and objective outcome measures, to assess their efficacy and optimize their performance. As in cochlear implant care and other neural prostheses, electrically evoked compound action potentials (eCAP) can be used to measure the neural response to implant stimulation. eCAPs might provide information about the presence and misalignment of the electrically evoked vestibulo-ocular reflex, and potential vestibulo-cochlear interacti

Recordings of Superior Laryngeal Nerve Sensory Nerve Action Potentials in a Rat Model.

OBJECTIVE: Superior laryngeal nerve (SLN) function is critical to laryngeal sensation. Sensory dysfunction in the larynx, mediated through the internal branch of the superior laryngeal nerve (iSLN), is thought to occur with aging and neurodegenerative disease. However, objective analysis of iSLN neurophysiology is difficult due to its anatomic location and small diameter. This study measures sensory nerve action potentials (SNAP) from the iSLN in a rat model. METHODS: SNAP data were obtained from two adult rat strains (Sprague-Dawley, SD and Fischer 344 × Brown Norway F1 Hybrid rats, FBN). Ev
