Acute cholecystitis in the robotic era: comparative safety and feasibility of robotic and laparoscopic cholecystectomy in an acute care surgery service.

This study aimed to evaluate and compare perioperative outcomes of robotic versus laparoscopic cholecystectomy in patients with acute cholecystitis within an Acute Care Surgery setting. A retrospective cohort study was conducted including patients who underwent cholecystectomy for acute cholecystitis between January 1, 2023, and March 1, 2026. Patients were stratified by operative approach (robotic vs. laparoscopic). Baseline demographics, comorbidities, and postoperative outcomes were analyzed. Continuous variables were compared using Mann-Whitney U test, and categorical variables were assess

Incidence and predictors of postoperative delirium in the older acute care surgery population: a prospective study.

BACKGROUND: Among older inpatients, the highest incidence of delirium is within the surgical population. Limited data are available regarding postoperative delirium risk in the acute care surgical population. The purpose of our study was to establish the incidence of and risk factors for delirium in an older acute care surgery population. METHODS: Patients aged 65 years or more who had undergone acute care surgery between April 2014 and September 2015 at 2 university-affiliated hospitals in Alberta were followed prospectively and screened for delirium by means of a validated chart review meth

Profile of acute kidney injury after open heart surgeries in a tertiary care hospital.

Our objective is to determine the incidence, etiology, risk factors and outcome of acute kidney injury (AKI) after open heart surgery. A prospective study was conducted on 62 patients who underwent open heart surgery and were followed-up for the development of AKI and to determine its incidence, etiology and outcome. Post-operative AKI was considered when the post-operative serum creatinine was >1.5 mg/dL or there was doubling of serum creatinine above the baseline (pre-operative) with a prior normal renal function. The incidence of AKI in the post-operative period in our study was 17.7%. The

Pyloric Exclusion in Modern Acute Care Surgery: A Selective Salvage Strategy for Complex Duodenal Pathology.

BackgroundPyloric exclusion (PEX) was initially described for duodenal fistula and complex ulcer disease. It was adopted in trauma surgery as an adjunct to primary repair of complicated duodenal injuries. However, its use in the non-trauma population has remained uninvestigated in recent years. Its current role as a highly selective operation for complex duodenal pathology remains ambiguous in the acute care surgery population. The aim of this study was to assess indications and outcomes of PEX in the acute care surgery population.MethodsPatients that underwent pyloric exclusion in the NSQIP d

Primary Anastomosis Versus Hartmann's Procedure in Acute Care Surgery: Evidence, Indications, and Barriers to Adoption.

Purpose of ReviewThis review examines the contemporary role of Hartmann's procedure (HP) in acute care surgery, focusing on the two principal disease processes in which it remains commonly utilized: complicated diverticulitis and traumatic colon injury. We review the evolving evidence comparing HP with primary anastomosis (PA), with or without diverting loop ileostomy (DLI), while emphasizing differences in patient populations, operative decision-making, and factors influencing adoption in clinical practice.Recent FindingsIncreasing evidence supports the safety and feasibility of PA in appropr

Shared decision-making within acute care surgery and surgical residency: Identifying deficiencies to increase use.

BACKGROUND: Shared decision-making is integral to patient-centered, goal concordant health care. In this model clinicians and patients work together to make care plans balancing clinical evidence regarding risks, benefits, and expected outcomes with patient preferences and values. While this is an ideal approach to patient care shared decision-making is inconsistently applied. Our aim was to identify surgery attendings', fellows', and residents' perceived barriers to, comfort with, and training experience in shared decision-making. METHODS: Surgery attendings (attendings/fellows) and resident

The new timing in acute care surgery (new TACS) classification: a WSES Delphi consensus study.

BACKGROUND: Timely access to the operating room for emergency general surgery (EGS) indications remains a challenge across the globe, largely driven by operating room availability and staffing constraints. The "timing in acute care surgery" (TACS) classification was previously published to introduce a new tool to triage the timely and appropriate access of EGS patients to the operating room. However, the clinical and operational effectiveness of the TACS classification has not been investigated in subsequent validation studies. This study aimed to improve the TACS classification and provide fu

Do safety-net hospitals provide equitable care after decompressive surgery for acute cauda equina syndrome?

INTRODUCTION: Safety-net hospitals provide care to a substantial share of disadvantaged patient populations. Whether disparities exist between safety-net hospitals and their counterparts in performing emergent neurosurgical procedures has not yet been examined. OBJECTIVE: We used the Nationwide Inpatient Sample (NIS), a national all-payer inpatient healthcare database, to determine whether safety-net hospitals provide equitable care after decompressive surgery for acute cauda equina syndrome (CES). METHODS: The NIS from 2002 to 2011 was queried for patients with a diagnosis of acute CES who

Two rare cases of Acremonium acute endophthalmitis after cataract surgery in a tertiary care hospital.

This report describes two cases of Acremonium sp. endophthalmitis, occurring in two patients who underwent cataract surgery on the same day in the same operating room of our hospital ophthalmology clinic. Diagnosis of fungal endophthalmitis was established by the repeated isolation of the same fungal agent from vitreous washing, acqueous fluid and intraocular lens samples and by its identification on the basis of morphological and molecular features. The cases reported in this study emphasize the need for clinical microbiology laboratories to be prepared to face the diagnosis of uncommon infec

Does post acute care reduce the mortality of octogenarian and nonagenarian patients undergoing hip fracture surgery?

BACKGROUND: With the increasing number of elderly individuals worldwide, a greater number of people aged 80 years and older sustain fragility fracture due to osteopenia and osteoporosis. METHODS: This retrospective study included 158 older adults, with a median age of 85 (range: 80-99) years, who sustained hip fragility fracture and who underwent surgery. The patients were divided into two groups, one including patients who joined the post-acute care (PAC) program after surgery and another comprising patients who did not. The mortality, complication, comorbidity, re-fracture, secondary fractu

What if You Need More Than One? More Acute Care Surgery Procedures Are Associated with Mortality.

Background: It is unknown whether having multiple acute care surgery (ACS) procedures performed in one admission confers additional risk. We hypothesized that having multiple procedures (for example, hernia repair plus bowel resection) would be associated with higher mortality. Patients and Methods: We identified all 2017 National Inpatient Sample admissions with ACS procedures including: colon, small bowel/appendix (SB), hernia, adhesiolysis, peptic ulcer procedures, gallbladder, debridement, other laparotomy, other laparoscopy. The total number of procedures for each admission and common dya

Indocyanine green does not decrease the need for bail-out operation in an acute care surgery population.

BACKGROUND: The use of indocyanine green during laparoscopic cholecystectomy has been postulated to help to define anatomy. Studies have not specifically evaluated patients with acute cholecystitis. We sought to assess whether use of indocyanine green can decrease the rate of bail-out operation (subtotal cholecystectomy or conversion to an open operation) in an acute care surgery population where acute cholecystitis is more frequent. METHODS: Using a retrospective cohort design, we examined all inpatient cholecystectomies performed by the acute care surgery service under urgent or semiurgent
