Pulmonary artery pressure and the acute chest syndrome in homozygous sickle cell disease.

OBJECTIVE: To investigate whether attacks of acute chest syndrome affected pulmonary artery pressure in patients homozygous for sickle cell disease. MAIN OUTCOME MEASURES: Pulmonary artery pressure, assessed by non-invasive echocardiographic techniques. PATIENTS: 20 patients with homozygous sickle cell disease with a history of at least six episodes of acute chest syndrome and in 20 age, sex, and height matched controls with homozygous sickle cell disease without a history of acute chest syndrome. RESULTS: There was no difference in any of the echocardiographic or Doppler indices between th

Renin-Angiotensin Blockade Reduces Readmission for Acute Chest Syndrome in Sickle Cell Disease.

Rationale Acute chest syndrome (ACS) is a life-threatening complication of sickle cell disease (SCD). Current treatment is supportive-supplemental oxygen, transfusions, and antibiotics. Prevention of ACS may reduce morbidity and mortality in patients with SCD. Acute chest syndrome appears similar to pulmonary fat embolism (PFE), a complication of severe skeletal trauma or orthopedic procedures from pulmonary micro-vessel blockage by bone marrow fat. Vascular obstruction and bone marrow necrosis occur in PFE and ACS.  Pulmonary fat embolism rat models have shown that angiotensin-converting enzy

Inhaled nitric oxide for acute chest syndrome in people with sickle cell disease.

BACKGROUND: Acute chest syndrome has been defined as a new infiltrate visible on chest radiograph associated with one or more symptoms, such as fever, cough, sputum production, tachypnea, dyspnea, or new-onset hypoxia. Symptoms and complications of this syndrome, whether of infectious or non-infectious origin, vary quite widely in people with sickle cell disease. Lung infection tends to predominate in children, whilst infarction appears more common in adults. However, these are often interrelated and may occur concurrently. The differences in clinical course and severity are suggestive of mult

Addison's disease presenting as acute chest syndrome: case report and review of literature.

BACKGROUND: Addison's disease is due to primary adrenal failure. It is an uncommon condition with equal prevalence in both males and females. The onset of symptoms is gradual and manifestation is non specific, hence diagnosis is easily missed without a high index of suspicion. METHODS: The medical records of a patient who presented with acute chest pain to the cardiac unit of the University of Port Harcourt Teaching Hospital were reviewed. A review of the literature using manual library and Medline search on Addison's disease was also done. RESULT: A 48 years old male presented in our medica

Severe acute respiratory syndrome: prognostic implications of chest radiographic findings in 52 patients.

PURPOSE: To retrospectively assess prognostic implications of radiographic findings in severe acute respiratory syndrome (SARS). MATERIALS AND METHODS: Radiographic findings were reviewed by two radiologists for 52 patients with SARS. On each radiograph, each lung was separated into upper, middle, and lower zones. A four-point scale was used to score extent of SARS-related lesions in each zone; points from all zones were added for a cumulative score. Patient sex, age, comorbidities, duration of developing lesions, lesion score for each radiograph, need for mechanical ventilation, and percenta

Acute chest syndrome of sickle cell disease: new light on an old problem.

The pulmonary findings of acute chest syndrome of sickle cell disease have been well characterized in numerous studies. Whereas a third of patients have a documented infection associated with this syndrome, and fat embolism from necrotic marrow is the etiologic factor in another approximately 10%, no cause is discovered in the majority of patients. In most patients, however, the underlying pathophysiology is the presence of a hypoxia-driven, adhesion-related occlusive event in the pulmonary microcirculation. This may be accompanied by a decrease in the levels of normal cytoprotective and anti-

Secretory phospholipase A(2) predicts impending acute chest syndrome in sickle cell disease.

Acute chest syndrome (ACS) is the leading cause of death in sickle cell disease. Severe ACS often develops in the course of a vaso-occlusive crisis (VOC), but currently there are no predictors for its development. Secretory phospholipase A(2) (sPLA(2)), a potent inflammatory mediator, is elevated in ACS, and previous work suggests that sPLA(2) predicts impending ACS. We prospectively evaluated sPLA(2) concentration during 21 admissions for VOC; 6 of these patients went on to develop ACS. Elevation of sPLA(2) was detected all 6 patients 24 to 48 hours before ACS was clinically diagnosed. Adding

Diagnostic importance of admission platelet volume indices in patients with acute chest pain suggesting acute coronary syndrome.

OBJECTIVE: Acute coronary syndrome (ACS) is a challenging issue in cardiovascular medicine. Given platelet role in atherothrombosis, we sought to determine whether platelet indices can be used as diagnostic tests for patients who suffered from an acute chest discomfort. METHODS: We prospectively enrolled 862 patients with an acute chest pain and 184 healthy matched controls. They were divided into four groups: 184 controls, 249 of non-ACS, 421 of unstable angina (UA), and 192 of myocardial infarction (MI) cases. Blood samples were collected at admission to the emergency department for routine

Natriuretic peptides in acute chest pain and acute coronary syndrome: from pathophysiology to clinical and prognostic applications.

BACKGROUND: Plasma levels of natriuretic peptides (NPs) appear to have an important prognostic value in many clinical settings including coronary artery disease and provide additional information about cardiovascular risk. CONTENT: The aim of this study was to review the literature on the role of NPs in the prognosis and management of patients with acute chest pain and acute coronary syndrome (ACS). Several trials have assessed the prognostic value of NPs as biomarkers of myocardial injury for risk stratification in patients with acute chest pain and ACS. The additional prognostic information

Treating a Patient with Your Hands Tied: Acute Chest Syndrome in a Jehovah's Witness.

Acute chest syndrome (ACS), a vaso-occlusive crisis in patients with sickle cell anemia, is a life-threatening condition and a leading cause of death in these patients. It is treated with analgesics, antibiotics, intravenous fluid, supplemental oxygen (or ventilatory support in severe cases) with simple or exchange transfusion, being the mainstay of therapy. We report a young Jehovah's Witness (JW) patient with sickle cell anemia who presented with ACS. Her religious beliefs precluded the use of blood products. Despite concomitant hemolytic and aplastic crisis and a hemoglobin nadir of 3.1 g/d

Controversies in Diagnostic Imaging of Patients With Suspected Stable and Acute Chest Pain Syndromes.

There has been a tremendous growth quantity of high-quality imaging evidence in the area of acute and stable ischemic heart disease (SIHD). A number of recent comparative effectiveness trials have spurned significant controversies in the field of cardiovascular imaging. The result of this evidence is that many health care policies and national guidelines have undergone significant revisions. With all of this evidence, many challenges remain and the optimal evaluation strategy for evaluation of patients presenting with chest pain remains ill-defined. This paper enlisted the guidance of numerous

Diagnostic efficacy of myeloperoxidase to identify acute coronary syndrome in subjects with chest pain.

BACKGROUND: Early diagnosis of acute coronary syndrome (ACS) is frequently a challenging task. AIMS: To assess the role of novel biomarkers to identify ACS. METHODS: Concentrations of lipids, lipoproteins, oxidized LDL (oxLDL), high-sensitivity C-reactive protein (hsCRP), paraoxonase-1 (PON1), secretory phospholipase A2 (sPLA2), and myeloperoxidase (MPO) were measured in 703 patients (mean age 65.5 ± 11.2 years; 422 men, 281 women) without diabetes mellitus assigned to coronary angiogram. The subjects were divided into three groups: ACS (n = 242), stable angina pectoris (SAP) (n = 242), and

A fatal case of acute chest syndrome in a patient with undiagnosed sickle cell trait.

We report a fatal case of acute chest syndrome in an African-American male. The patient was hospitalized for respiratory distress, fevers, and pulmonary infiltrates after working in an attic space on a summer day. An extensive work-up failed to reveal an etiology for his respiratory failure. He died of respiratory failure two weeks after admission. Autopsy findings suggested the patient had clinically unrecognized sickle cell trait exacerbated by working in the heat, causing acute chest syndrome with a fatal outcome. Clinicians should consider acute chest syndrome and sickle cell trait in the

Thrombus Precursor Protein and the measurement of thrombosis in patients with acute chest pain syndrome.

The major limitation of current diagnostic methods for the rapid diagnosis of an acute myocardial infarction (AMI) involves the measurement of the effects of ischemic or necrotic processes on myocardial function rather than the detection of the precipitating thrombotic event. The utility of an assay was investigated for thrombus precursor protein (TpP) in the diagnosis of 115 patients with symptoms consistent with myocardial ischemia or infarction of less than six hours in duration. Samples from patients were drawn at 0, 1, 2, 4, 8, 16, and 24 hours post presentation, and creatine kinase, CK-M

Biomarkers for the diagnosis and phenotyping of acute chest syndrome in patients with sickle cell disease.

INTRODUCTION: Acute Chest Syndrome (ACS) is a serious and often fatal complication of Sickle Cell Disease (SCD). In the absence of specific biomarkers that indicate the development of ACS, diagnosis can be nebulous and difficult. In this narrative literature review, we examine the current evidence for novel biomarkers associated with acute and long-term propensity toward ACS. AREAS COVERED: A search focused on terms related to 'acute chest syndrome,' 'sickle cell anemia,' and 'biomarkers' was created and run by a medical librarian in MEDLINE (Ovid), Google Scholar, and Embase (Ovid). Four-hun

Bedside ultrasound as a predictive tool for acute chest syndrome in sickle cell patients.

BACKGROUND: Acute chest syndrome (ACS) is the leading cause of death for patients with sickle cell disease (SCD). Early recognition of ACS improves prognosis. OBJECTIVE: Investigate the use of bedside lung ultrasound (BLU) in identification of early pulmonary findings associated with ACS in SCD patients. METHODS: Prospective, observational study of a convenience sample of SCD patients presenting to the Emergency Department (ED) for a pain crisis. BLU interpretations were made by an emergency physician blinded to the diagnosis of ACS, and were validated by a second reviewer. The electronic me

The Association of Electrocardiographic Abnormalities and Acute Coronary Syndrome in Emergency Patients With Chest Pain.

OBJECTIVES: The electrocardiograph (ECG) is an essential tool in initial management and risk stratification of patients with suspected acute coronary syndrome (ACS). A six-point reporting criterion has been proposed to facilitate standardized clinical assessment of patients presenting to the emergency department (ED) with suspected ACS. We set out to evaluate the efficacy of these criteria in identifying patients with major adverse cardiac events (MACE), Type 1 myocardial infarction (T1MI), Type 2 myocardial infarction (T2MI), and 1-year mortality in a cohort of emergency patients with chest p
