The influence of age on adaptation of disparity vergence and phoria.

A paucity of research exists to investigate whether the normal aging process influences the ability to adapt disparity vergence and phoria. Vergence eye movements and dissociated phoria were recorded from 49 healthy subjects (ages 20-70years) using an objective eye movement tracking system. Four-degree vergence responses were modified using a double-step protocol. Dynamics of vergence were quantified via peak velocity. The phoria adaptation experiment measured the magnitude (net change in phoria level) and rate (magnitude divided by the time constant) of phoria adaption during 5min of sustaine

Reversible night blindness associated with intestinal bypass surgery.

A 33-year-old man had a sudden onset of night blindness 3 1/2 years after intestinal bypass surgery for morbid obesity. Ocular examination disclosed a probable Bitot's spot and electrophysiologic testing showed markedly impaired dark adaptation and subnormal electroretinography. After therapy with orally administered vitamin A, the night blindness disappeared and the dark adaptation curve and electroretinogram returned to normal.

Reduced vergence adaptation is associated with a prolonged output of convergence accommodation in convergence insufficiency.

Convergence insufficiency (CI) is a developmental visual anomaly defined clinically by a reduced near point of convergence, a reduced capacity to view through base-out prisms (fusional convergence); coupled with asthenopic symptoms typically blur and diplopia. Experimental studies show reduced vergence parameters and tonic adaptation. Based upon current models of accommodation and vergence, we hypothesize that the reduced vergence adaptation in CI leads to excessive amounts of convergence accommodation (CA). Eleven CI participants (mean age=17.4±2.3 years) were recruited with reduced capacity

Persistent mal de debarquement syndrome: a motion-induced subjective disorder of balance.

Six patients with a distinct syndrome of persistent sensations of imbalance are presented. Common features included onset following a period of motion exposure, symptoms lasting months to years, mild unsteadiness and anxiety, minimal relief from antivertiginous medication, and normal neurologic and quantitative vestibulo-ocular examinations. One patient experienced recurrent episodes. Mal de debarquement refers to sensations of motion experienced on return to stable land after adaptation to motion lasting from hours to days in normal individuals. The presented patients exemplify a syndrome of

Distance vergence adaptation is abnormal in subjects with convergence insufficiency.

It is well known that in convergence insufficiency (CI) prism adaptation is reduced in response to base-out (BO) prisms at near. There have also been some suggestions in the literature that adaptation is reduced at other distances as well. The present data show that in CI adaptation is not only reduced in response to BO at near, but also in response to base-in (BI) at near and for both BI and BO at distance. This raises the interesting question whether distance adaptation becomes reduced because of reduced near adaptation or whether these subjects have a generally reduced horizontal adaptation

Tonic vergence and vergence adaptation.

In the absence of an adequate visual stimulus, the eyes are typically converged by approximately 0.25 to 0.75 meter angles (MA). This vergence response (VR) was believed to reflect the level of tonic innervation to the extraocular muscles, and accordingly has been termed tonic vergence (TV). However, this estimation fails to consider the magnitude of the anatomical position of rest. The true typical value of TV is approximately 23 degrees. This paper will consider various aspects of this parameter, including both clinical and laboratory methods of measurement, and the relationship between TV a

Influence of accommodative and vergence adaptation on binocular motor disorders.

Tait described four categories of binocular disorders including convergence excess, convergence insufficiency, divergence excess, and divergence insufficiency. These disorders are defined by the distance where the largest heterophoria occurs (distance or near), and the amplitude of the accommodative vergence ratio (AC/A). Insufficiency corresponds to a low AC/A ratio, whereas excess corresponds to a high AC/A. The magnitude of the AC/A ratio, which may be influenced by the adaptability of the accommodation and vergence systems, has been shown to be reciprocally related to adaptability of accom

Analysis of tonic and accommodative vergence disorders of binocular vision.

Tonic and accommodative vergence disorders of binocular vision are analyzed with separate methods. The center of symmetry and the associated phoria of forced duction fixation disparity curves are two criteria used for prismatic correction of tonic vergence disorders. The associated phoria is corrected physiologically when patients who have tonic disorders are able to develop prism adaptation with orthoptic exercises. Graphical methods are used to analyze accommodative disorders resulting from mutual interactions between accommodation and convergence. Graphic analysis illustrates that patients

Deficient vergence prism adaptation in subjects with decompensated heterophoria.

Vergence prism adaptability was evaluated in subjects with high symptomatic and asymptomatic heterophoria and compared to individuals with a heterophoria within normal range (the control group). A computer haploscope was used to measure phoria values and changes in the eye position after introducing 6 prism diopters base out in front of the right eye. Phoria values were measured with a nonius paradigm every minute for a period of 10 minutes during adaptation. The results showed that subjects with symptomatic heterophoria are characterized by a lower rate of prism adaptation and adapted to a sm

Adaptation to lens-induced heterophorias.

In previous studies we have shown that subjects with normal binocular vision adapt to prism-induced heterophorias. In this study we have continued the work by investigating whether the oculomotor system can also adapt to lens-induced heterophorias. It was found that subjects gradually adapted to the esophoria induced when viewing a distance chart through negative lenses. Adaptation also occurred to lens-induced esophoria and exophoria at near, while at distance, where the visual feedback was unaltered, the phoria remained virtually constant. These findings demonstrate the flexibility of the oc

Cerebellar transcranial direct current stimulation interacts with BDNF Val66Met in motor learning.

BACKGROUND: Cerebellar transcranial direct current stimulation has been reported to enhance motor associative learning and motor adaptation, holding promise for clinical application in patients with movement disorders. However, behavioral benefits from cerebellar tDCS have been inconsistent. OBJECTIVE: Identifying determinants of treatment success is necessary. BDNF Val66Met is a candidate determinant, because the polymorphism is associated with motor skill learning and BDNF is thought to mediate tDCS effects. METHODS: We undertook two cerebellar tDCS studies in subjects genotyped for BDNF V

Visual performance after correcting higher order aberrations in keratoconic eyes.

Keratoconic eyes are affected by an irregular optical blur induced by significant magnitude of higher order aberrations (HOAs). Although it is expected that correction of ocular aberrations leads to an improvement in visual performance, keratoconic eyes might not achieve the visual benefit predicted by optical theory because of long-term adaptation to poor retinal image quality. To investigate this, an adaptive optics (AO) system equipped with a large-stroke deformable mirror and a Shack-Hartmann wavefront sensor was used to correct the aberrations and measure high contrast tumbling E visual a

The effect of orthoptic treatment upon the vergence adaptation mechanism.

This paper is a review of the research work that has been carried out over the past few years investigating the ability of the oculomotor system to adapt to prism-induced heterophoria. Our results show that subjects with normal binocular vision can adapt to horizontal and vertical prism-induced heterophorias whether fixating at distance or near. Further studies have shown that subjects with symptomatic abnormal binocular vision have an abnormal adaptation mechanism. Finally, we have found that when orthoptic treatment results in relief from the symptoms, there is an associated improvement in t

[A case of chronic progressive external ophthalmoplegia with Oguchi's disease].

A 52-year-old case of chronic external ophthalmoplegia accompanied by Oguchi's disease was reported. The patient noticed night blindness and deafness since childhood. Later he developed ocular movement limitation to all directions by the age of 40. He had 20/20 corrected visual acuity and normal anterior segment, as well as normal color vision. Goldmann's perimetry showed generalized constriction of the peripheral field. Golden tapetal reflex without dark adaptation disappeared after 4 hours of dark adaptation, i.e. Mizuo-Nakamura's phenomenon was recognized. Recovery of diminished a and b wav

Probing transsaccadic correspondence with reverse correlation.

The phenomenon of visual stability is classically explained by an internal forward model predicting the sensory consequences of an eye movement based on efference copy. However, this model cannot explain why some object displacements go undetected, a phenomenon that may depend on a passive prior belief that the world is stable. With reverse correlation, we investigated saccadic suppression of displacement and found that transsaccadic correspondence operates differently depending on the position of the postsaccadic visual target relative to the primary landing position; when the signal falls wi

The roles of neural adaptation and sensitization in contact lens discomfort.

PURPOSE: To investigate the roles of neural adaptation and sensitization in contact lens discomfort (CLD). METHODS: Cooling stimuli (20 °C) were applied to the cornea in a group comprising 24 symptomatic and 25 asymptomatic contact lens (CL) wearers as well as 15 non-CL wearing controls, using a computerized Belmonte esthesiometer. The adaptation paradigm consisted of 20 repetitive stimuli at threshold, sub- and supra-threshold levels. The sensitization paradigm involved five levels of suprathreshold stimuli ranging between 1x to 2x threshold. Following each stimulus, participants rated the s
