Thermosensitive in situ gels for ocular drug delivery: Advances in polymer-based formulations.

Ocular drug delivery presents significant challenges due to the unique anatomical and physiological barriers of the human eye, with the rapid precorneal elimination, limiting the bioavailability of conventional eye drops. Thermosensitive in situ gels have emerged as a promising delivery system to overcome these limitations by undergoing a reversible sol-to-gel transition upon contact with ocular surface temperature, which facilitates ease of administration as a liquid and subsequent transformation into a gel, thereby enhancing precorneal residence time, prolonging the drug release, and improvi

Optimizing instilled drug delivery: a scoping review of microdrops in ophthalmology.

Eyedrop instillation constitutes the most commonly used ocular drug delivery method that serves for both diagnostic and therapeutic purposes. Ocular disposition and bioavailability of instilled drugs depend on the anatomy and physiology of the ocular surface as well as the physicochemical properties of the active agent. Intraocular bioavailability is positively associated with the amount of drug available onto the ocular surface and the precorneal residence time. Concerns are raised regarding systemic absorption of the instilled drugs intraocularly, percutaneously, via the conjunctiva, through

Elucidating acceptance and clinical indications to support the rational design of drug-eluting contact lenses.

The advent of drug-eluting contact lenses (DECLs) has opened up new avenues for the treatment of eye diseases. DECLs is expected to partially overcome the shortcomings of eye drops due to single-dose packaging, accurate dosing, prolonged drug elution behavior, and simplified dosing procedures. Currently, a significant proportion of the DECLs design effort has been directed towards enhancing the compatibility of contact lenses with drugs. The appropriate elution time for the drug remains unclear. Additionally, it is ambiguous for which ophthalmic diseases DECLs offers the greatest therapeutic a

An Ocular Exposure Prediction for Topical Atropine in Human Using Physiologically Based Pharmacokinetic Modeling.

Developing a mathematical model to predict the distribution and bioavailability of atropine in human eyes is an insight approach for clinical practice. This study aims to develop a human ocular physiologically based pharmacokinetic (PBPK) modeling for the ophthalmic drug atropine and explore possible mechanisms by which atropine reduces myopia progression in children. The Ocular Compartment Absorption and Transit (OCAT™) model was employed to describe the ocular distribution of atropine following administration at different dosages in both rabbits and humans. The PBPK model enables the extrapo

[Bradyarrhythmia and topical timolol : highlighting the need for medication reconciliation].

Ophthalmic beta-blockers such as timolol can exert clinically significant systemic effects due to high transnasal and conjunctival absorption. We describe a 94-year-old woman who developed symptomatic bradyarrhythmia, including sinus pauses and atrial fibrillation with slow ventricular response, attributed to topical timolol. Symptoms resolved following discontinuation and substitution with non-beta-blocker therapy. This case underscores the importance of comprehensive medication reconciliation, explicitly addressing non-oral routes of administration. In elderly, polymorbid patients, unrecogni
