Alpha- and beta-adrenergic receptor blocking agents combined with a diuretic in the treatment of essential hypertension.

The antihypertensive efficacy and side effects of the combined therapy with propranolol, phenoxybenzamine, and hydrochlorothiazide were examined in 17 patients with moderate and moderately severe hypertension. Following a control period two to three weeks' duration, propranolol was started in nine patients as the sole antihypertensive agent and together with phenoxybenzamine, in eight. By titrating the dosage against pulse rate and blood pressure response, propranolol was given, in divided doses, from 80 to 160 mg and phenoxybenzamine, from 20 to 50 mg per day. When propranolol was given alone

Response of the urethral smooth muscles to pharmacological agents. I. Cholinergic and adrenergic agonists and antagonists.

We studied the effects of cholinergic and adrenergic agents on the 2 muscle layers of the proximal urethra of the cat. Strips were challenged with bethanechol chloride, phenylephrine and isoproterenol. Bethanechol chloride caused contraction of the longitudinally oriented strips; the circularly oriented strips revealed either minimal or no response to the same doses. The difference between the response of the 2 layers was highly significant (p less than 0.001). Phenylephrine stimulated both layers with a nonsignificant difference. Isoproterenol caused relaxation of both layers with a significa…

beta-Adrenergic receptors and renin release: studies with beta-adrenoreceptor-blocking agents in the conscious rabbit.

1. Plasma renin activity (PRA) and mean blood pressure were studied in conscious rabbits infused with beta-adrenoreceptor antagonists. 2. Oxprenolol and DL-propranolol each significantly reduced PRA and blood pressure, but prindolol, which had a strong blood pressure-lowering effect, increased PRA. 3. When prindolol was given to animals in which PRA and blood pressure had been reduced by DL-propranolol, PRA returned to control values but blood pressure remained low. Thus the increase in PRA caused by prindolol is not mediated by hypotension. These findings, together with the observation that c

Do beta-adrenergic blocking agents increase asthma exacerbation? A network meta-analysis of randomized controlled trials.

Beta-adrenergic blocking agents (abbreviated as beta-blockers) have been used for treating various cardiovascular diseases. However, the potential for asthma exacerbation is one of the major adverse effects of beta-blockers. This study aimed to compare the level of risk for an asthma attack in patients receiving various beta-blockers. We searched for randomized controlled trials (RCTs) of either placebo-controlled or active-controlled design. The current network meta-analysis (NMA) was conducted under a frequentist model. The primary outcome was the incidence of asthmatic attack. A total of 24

Combined effects of adrenergic and intravenous anesthetic agents on inositol monophosphate levels in rat liver prisms.

Combined effects of adrenergic and intravenous anesthetic agents on phosphatidylinositol (PI) turnover were studied using rat liver prisms i … y droperidol. The present results from in vitro studies of liver cell metabolism suggest that alpha-adrenergic agents in combination with barbiturates may potentiate liver cell damage by activation of PI turnover and interrelated intracellular Ca++ accumulation.

Ocular penetration of beta-adrenergic blocking agents. An experimental study with atenolol, metoprolol, timolol and propranolol.

A comparison was made between the ocular penetration of topically applied 14C-atenolol, 14C-timolol, 14C-propranolol and 3H-metoprolol by means of liquid scintillation counting. Only one eye was treated, the fellow eye served as a control. Blood plasma levels were measured as well. We could detect a relationship between the ocular penetration and the degree of lipophilicity of the drugs used, as was to be expected. Drugs with a higher degree of lipophilicity penetrated more readily into the eye, whereas they also achieved higher blood plasma levels. Relatively high concentrations of atenolol w

The effects of alpha and beta adrenergic agents on spleen cell antigen binding in four amphibian species.

Adults of two urodele amphibian species (Triturus cristatus carnifex and Cynops hongkongensis) and two anuran species (Rana temporaria and Xenopus laevis laevis) were immunized with a 25% suspension of sheep or horse erythrocytes. After eight or 14 days, splenic lymphocytes were removed, and their specific red cell-binding capacities tested by immunocytoadherence. Antigen-binding cells were classified as high-dose nonsecretory (S-) or secretory (S+), according to whether they bound a single layer or several layers or erythrocytes. The stimulation of both alpha and beta adrenoreceptors reduced

Oral Adrenergic Agents Produced Ventricular Fibrillation and QT Prolongation in an Elderly Patient Carrying an RYR2 Variant.

Mutant cardiac ryanodine receptor channels (RyR2) are "leaky," and spontaneous Ca2+ release through these channels causes delayed afterdepol … rillation who carried a novel RYR2 variant but no other mutations related to long QT syndrome. Oral adrenergic agents might induce QT prolongation and subsequent ventricular fibrillation in patients carrying an RYR2 variant. Screening for RYR2 could be valuable in patients with suspected drug-induced long QT syndrome.

Application of pH-zone-refining countercurrent chromatography in the chiral separation of two β-adrenergic blocking agents.

Two β-adrenergic blocking agents, 1-[(1-methylethyl)amino]-3-phenoxy-2-propanol (1) and 1-[(1-methylethyl)amino]-3-(3-methylphenoxy)-2-propanol (2; Toliprolol), were enantioseparated by pH-zone-refining countercurrent chromatography. A two-phase solvent system composed of chloroform containing 0.10 mol/L of di-n-hexyl l-tartrate/0.10 mol/L of boric acid aqueous solution (1:1, v/v) was selected, in which 20 mmol/L triethylamine was added in the organic phase as a retainer and 2 mmol/L HCl was added in the aqueous phase as an eluter. Fifty milligrams of each racemate was completely enantiosepara

A case of life-threatening lactic acidosis after smoke inhalation - interference between beta-adrenergic agents and ethanol?

A 49-year-old male developed bronchospasm and severe lactic acidosis after exposition to fire smoke. The correction of lactic acidosis following beta-adrenergic agents withdrawal, and the transitory increase in lactate after salbutamol reintroduction are consistent with hypersensitivity to salbutamol. However, the plasma lactate concentration (32.6 mmol/l) that we observed 9.5 h after admission is far above those currently seen after administration of beta-adrenergic agents. We searched for causes able to potentiate the adverse effects of these drugs and we noticed that our patient had a high …

Effect of some beta-adrenergic blocking agents on tissue guanylate cyclase and cyclic nucleotides in the rat.

Pretreatment of rats with the beta-adrenergic blocking agents, atenolol, practolol, pronethalol and propranolol, at a dosage of 150 mg/kg/day for 5 days, produced marked increases in guanylate cyclase activity in the liver, gastric and intestinal mucosae, but with concomitant decreases in cyclic GMP levels. The ratios of cAMP/cGMP in each of the tissues were not changed significantly from control values after pretreatment with these drugs. These observations indicate that any oncogenicity seen with beta-blocking agents is not due to the common pharmacological action of beta-adrenoreceptor bloc

Beta-adrenergic blocking agents. 20. (3-Hydroxyprop-1-enyl)-substituted 1-(aryloxy)-3-(alkylamino)propan-2-ols.

The synthesis of a series of (3-hydroxyprop-1-enyl)-substituted 1-(aryloxy)-3-(alkylamino)propan-2-ols is described. These compounds were investigated for their beta-adrenoreceptor blocking properties and their selective of action. Among the o-(hydroxypropenyl)-substituted derivatives we have found some potent noncardioselective beta-adrenoreceptor blocking agents which have a greater blocking action on the beta 2 receptor, thus resembling propranolol. The p-(hydroxypropenyl)-substituted analogues were generally less potent and tended to be cardioselective. The structure-activity relationships

An in vitro approach to investigate ocular metabolism of a topical, selective β1-adrenergic blocking agent, betaxolol.

1. Topical glaucoma treatments have often been limited by poor absorption and bioavailability. Betaxolol, a selective β1-blocker, has been well studied for its pharmacokinetics and disposition. Limited ocular, betaxolol metabolism data is available despite a growing number of novel ocular treatments. 2. In vitro ocular fractions indicated the formation of an active metabolite, across rat, rabbit and human, which was only observed historically in the liver. 3. Ocular metabolic profiles of preclinical toxicology species, rat and rabbit, were not predictive of human in vitro ocular data. M1 was s

Effects of beta-adrenergic agents on systolic and diastolic myocardial function in patients with and without heart failure.

Cyclic AMP plays a central role in modulating systolic and diastolic myocardial function. Consequently, the adrenergic nervous system, acting through myocardial beta-adrenergic receptors, very likely plays a significant role in regulating left ventricular systolic and diastolic function. Theoretically, beta-adrenergic agonists and antagonists may further modify myocardial systolic and diastolic function. Although the effects of beta-adrenergic agents are clearly demonstrable in vitro, it has been more difficult to evaluate the direct myocardial effects of beta-adrenergic agents in patients. Th…

Effects of alpha- and beta-adrenergic agents and dopamine on gastric motor activity stimulated by acetylcholine or physostigmine.

The inhibitory actions of phenylephrine, adrenaline, isoprenaline and dopamine on the antral and fundic pressure responses to exogenous acet … the former inhibited physostigmine more than acetylcholine in the antrum. It may be concluded that adrenergic agents and dopamine exert their inhibition of gastric motor activity partly on intramural cholinergic neural pathways. Dopaminergic as well as alpha-adrenergic influence appear to be more effective than beta-adrenergic. The pronounced effect on phasic antr …

Potentiation by steroids of the beta-adrenergic agent-induced stimulation of cyclic AMP in isolated mouse thymocytes.

There is an increasing amount of evidence suggesting that glucocorticoids may modulate the responsiveness of various cell types to beta-adrenergic agents. In some systems, it has been shown, in addition, that steroids potentiate the elevation of cAMP induced by catecholamines. Little is known however of the mechanism underlying steroid action. We have studied this 'permissive action' in isolated thymocytes which have specific receptor sites for both glucocorticoids and beta-adrenergic agents. The glucocorticoid compound dexamethasone did not alter intracellular cAMP level but markedly enhanced…
