A 65-year-old gentleman presented with a history of abdominal distension and difficulty in walking 10 years after a Polya partial gastrectomy. Clinical history and neurological examination suggested an axonal sensory neuropathy. A computed tomographic scan of the abdomen showed a large afferent jejunal loop, and a hydrogen breath test confirmed small-bowel bacterial overgrowth secondary to the blind loop syndrome. Serological tests revealed low copper levels, which are a cause of a myeloneuropathy. The trace element deficiency occurred as a consequence of small-bowel bacterial overgrowth, and

A surgical technic for cardial or juxtacardial ulcers is presented. The surgical principle consists of resection of the distal stomach including the ulcer in a way that the esophageal mucosa remains intact. Reconstruction of the transit is accomplished by a Roux-en-Y esophagogastrojejunostomy in one layer. A variable portion of gastric reservoir is preserved, and alkaline reflux is highly improbable due to the long intestinal loop. This technic has been performed in six patients with excellent postoperative results. Clinical and radiologic follow-up to three years has shown no esophagitis or s

Simultaneous dilation of both the common bile duct and the pancreatic duct (double-duct sign) is usually because of ampullary or pancreatic tumours. Here we report an unusual cause of double-duct dilation; we describe the case of a 49-year-old female who developed afferent loop syndrome after pylorus-preserving pancreaticoduodenectomy: cross-sectional imaging of the abdomen revealed a double-duct sign.

The variety of clinical manifestations operated stomach fits in the sequence of pathological syndromes: dumping and hypoglycemic syndromes, postgastroresectional post gastrectomy anemia, afferent loop syndrome, postoperative dystrophy, reflux esophagitis, a syndrome of "small" stomach, and hypoglycemic syndrome. Aim--to elucidate the role of disorders of motor function of the gastrointestinal tract in postgastroresectional syndromes and their correction. The nutrient administration reduced a motor function level of the stomach, restore motor function of the duodenum and ascending colon intesti

The patient was a 76-year-old man who had 3 times previously undergone laparotomies, including distal gastrectomy with a Billroth Ⅰ operation. In the current case, a total gastrectomy, end-to-side esophagojejunostomy, and a Roux-en-Y anastomosis for adenocarcinoma of the remnant stomach were performed. On postoperative day (POD) 7, he complained of epigastralgia. Abdominal CT revealed a markedly dilated duodenum, and a diagnosis of acute afferent loop obstruction was made. Emergency endoscopy revealed edematous stenosis of the Y-anastomotic site. A nasal endoscope could not pass the stricture,

We report 2 cases of afferent loop obstruction associated with peritoneal dissemination after total gastrectomy. Case 1: A 57-year-old man, who underwent total gastrectomy with Roux-en Y reconstruction for gastric cancer 5 years earlier, experienced lumbago. Computed tomography scans showed a fluid-filled dilated afferent loop. Bypass surgery was performed after inserting a drainage tube into the afferent loop. Case 2: A 61-year-old woman, who underwent total gastrectomy with Roux-en Y reconstruction for a gastric cancer 2 years earlier, experienced abdominal pain. CT and magnetic resonance im

Afferent loop obstruction is a relatively rare but significant complication of Billroth II gastrojejunostomy. We report the imaging findings in a patient in whom obstruction presented acutely and was due to the presence of an enterolith. CT showed dilatation of both the main pancreatic duct and the biliary ducts, and a markedly dilated afferent loop within which a 5-cm mass was present. The lesion had a heterogeneous, laminated appearance and did not show any contrast enhancement. Edema of fatty tissues surrounding the pancreatic tail, which extended to the left pararenal spaces, a small amoun

BACKGROUND: Recurrent malignant afferent loop obstruction (ALO) after pancreaticoduodenectomy poses significant management challenges, with conventional interventions (stents, percutaneous transhepatic cholangio drainage [PTCD], or surgery) having substantial limitations. CASE PRESENTATION: A 78-year-old man with recurrent pancreatic cancer presented with jaundice, fever, and abdominal pain 7 months post-pancreaticoduodenectomy. Imaging confirmed malignant ALO with cholangitis (CA19-9: 6772 U/mL; bilirubin: 116.4 µmol/L). INTERVENTION: An innovative dual-pathway drainage technique was employed

We present percutaneous biliary drainage in a patient with jaundice due to afferent loop obstruction following a pancreatoduodenectomy. Treatment strategy for this type of jaundice is discussed with a review of the literature. The dilated loop of the jejunum was drained percutaneously with 10 F abscess drainage catheter. The obstructive jaundice was improved.

Gastric bypass is now the operation of choice for surgical management of morbid obesity. Prompt and accurate radiological diagnosis in the early postoperative period is critical for proper management of suspected gastrojejunal anastomotic leakage, abscess formation, and acute distension of the excluded stomach and afferent loop. Inadequate weight loss may indicate an increase in the gastric pouch and anastomosis or dehiscence of the staple line. Gastric and jejunal ulceration and anastomotic narrowing also require radiological and endoscopic evaluation.

INTRODUCTION: A limited number of drugs are available for the treatment of functional dyspepsia and irritable bowel syndrome. The efficacy of STW 5 (Iberogast) was previously shown in clinical trials. Since visceral hypersensitivity seems to be the prime pathomechanism of functional gastro-intestinal disorders, the aim of this study was to explore whether STW 5 reduces intestinal afferent sensitivity in the upper gastrointestinal tract.
METHODS: Two groups of male Wistar rats were pretreated with either the herbal preparation STW 5 or its vehicle (30.8% ethanol). Then, after 2h, general anest

Experiments on dogs demonstrated the role of chronic disorders of duodenal patency (ChDDP) in the development of immediate and late-term postoperative complications of gastric and duodenal ulcer, like incompetence of the duodenal stump, reflux gastritis, progression of duodenostasis existing before the operation, the afferent loop syndrome, postresection cholecystitis and pancreatitis, the dumping syndrome. The formation of valvar anastomoses and early recognition and correction fo ChDDP in resection of the stomach (554 patients) made it possible to reduce significantly both the mortality rate

The Roux-en-Y gastrojejunostomy has become an increasingly popular technique for gastrointestinal tract reconstruction since it is purported to obviate many of the classic complications of the Billroth II gastrojejunostomy. In a review of over 900 Roux-en-Y gastrojejunostomies, seven patients with complications mimicking those seen with Billroth II reconstruction were identified. These complications included duodenal stump blowout, proximal blind loop or afferent loop syndrome, and bile-reflux gastritis. These complications resulted from technical problems in construction of the Roux-en-Y. Onc

To facilitate the percutaneous drainage of an afferent-loop small-intestine obstruction, we used sonography to detect visceral adhesions and select a safe puncture route. The portion of the small intestine that was fixed to the anterior abdominal wall was sonographically identified by using a high-frequency transducer to locate the area of restricted visceral sliding. The needle was then inserted into the intestine. In 3 cases, we have found that this technique improves the confidence of the physicians who perform the percutaneous drainage and may help to minimize the risks associated with the

BACKGROUND: Restorative proctocolectomy with ileal pouch-anal anastomosis can be associated with a variety of complications, including floppy pouch complex (FPC). FPC is defined as the presence of pouch prolapse, afferent limb syndrome, enterocele, redundant loop, and folding pouch on pouchoscopy or contrasted pouchogram. The main symptoms of patients with FPC are dyschezia, incomplete evacuation, and bloating. The aims of the study were to evaluate the relative frequency of each disorder of FPC and to characterize its risk factors.
METHODS: This case-control study included all eligible patie

A considerable diagnostic value of endoscopy for the detection of recurrent ulcers and other postoperative syndromes was shown on the basis of the experience with endoscopies in 21 patients after gastroenterostomies , 281 patients after gastric resection and 96 patients after vagotomies performed for the ulcer disease of the stomach and duodenum. The methods of examining and the endoscopic picture after different operations are found to be of particular character. To study the state of the afferent loop after Bilroth -II resection of the stomach it is expedient to associate endoscopy with rent

Four hundred and forty-seven patients who had undergone various surgical procedures for stage II-III cancer of the distal part of the stomach and 35 operated cases of chronic gastric ulcer were followed for three years. The study showed distal subtotal resection of the stomach using transversal gastroenteroanastomosis with a long loop and additional transversal enteroenteric anastomosis to offer functional advantage in cancer patients. These valve anastomoses prevented reflux of duodenal contents to the gastric stump, assured a stepwise emptying of the resected stomach and precluded passing of

If recurrent peptic ulceration follows partial gastrectomy with Billroth II reconstruction, retained antrum on the duodenal stump may be the culprit. Moderate hypergastrinemia and a high basal acid output (BAO) to maximal acid output (MAO) ratio on gastric analysis should alert the clinician. Careful filling of the afferent loop on barium meal or technetium 99m scanning may verify the diagnosis. The secretin provocative test may be helpful in distinguishing retained antrum from the Zollinger-Ellison syndrome by eliciting a decrease in serum gastrin levels in patients with retained antrum and a

In this article, we examine phantom limb syndrome to gain insights into how the brain functions as the mind and how consciousness arises. We further explore our previously proposed consciousness model in which consciousness and body schema arise when information from throughout the body is processed by corticothalamic feedback loops and integrated by the thalamus. The parietal lobe spatially maps visual and non-visual information and the thalamus integrates and recreates this processed sensory information within a three-dimensional space termed the "3D default space." We propose that phantom l

After gastrectomy and Billroth II anastomosis, if the afferent loop is too long, it may kink and herniate, resulting in obstruction. If it is too short, it may produce acute angulation with obstruction. This condition often simulates acute pancreatitis when the amylase level in urine and serum is increased. Three cases of afferent loop obstruction are reported. The patient who did not undergo laparotomy died; the other two were operated on and survived. The diagnosis, etiology and treatment of these complications are discussed, and similar cases reported in the literature are cited.
