The Impact of African Caribbean Heritage on the Development of Sight-Threatening Diabetic Retinopathy in a Multiethnic Cohort of People With Type 2 Diabetes.

OBJECTIVE: Our aim was to identify risk factors for sight-threatening diabetic retinopathy (STDR) in a multiethnic cohort of type 2 diabetes attending a surveillance digital diabetes eye screening service in South London between 2004 and 2018. RESEARCH DESIGN AND METHODS: A total of 8,529 people who had no retinopathy at baseline were included in this analysis. Female participants made up 49% of the cohort, 38% were White Caucasian, 43.4% were African Caribbean, and 18.6% were of other racial/ethnic groups. STDR was defined as the presence of any moderate to severe nonproliferative or preprol

Rates of cognitive impairment in a South African cohort of people with HIV: variation by definitional criteria and lack of association with neuroimaging biomarkers.

There is wide variation in the reported prevalence of cognitive impairment in people with HIV (PWH). Part of this variation may be attributable to different studies using different methods of combining neuropsychological test scores to classify participants as either cognitively impaired or unimpaired. Our aim was to determine, in a South African cohort of PWH (N = 148), (a) how much variation in reported rates was due to method used to define cognitive impairment and (b) which method correlated best with MRI biomarkers of HIV-related brain pathology. Participants completed detailed neuropsych

Life experiences of people who stutter, and the perceived impact of stuttering on quality of life: personal accounts of South African individuals.

UNLABELLED: The purpose of the study was to investigate the life experiences of a group of South African adults who stutter and the impact of stuttering on their quality of life. Participants were 16 adults with a mean age of 28.9 and ranging from 20 to 59 years. Methods involved individual interviews designed to explore the life domains of education; social life; employment; speech therapy; family and marital life; and identity, beliefs and emotional issues. Main findings of the study indicated that the majority of participants perceived their stuttering to have impacted on their academic per

Diabetogenic elevated childhood total fat in South Asian and Black African/Caribbean people relates to adverse early life growth and low socioeconomic position compared with White people in the UK.

AIMS/HYPOTHESIS: Excess type 2 diabetes mellitus in minority ethnic groups remains unexplained, although greater fat mass makes a strong contribution. We hypothesised that height and weight through infancy in South Asian and Black African/Caribbean subgroups is more adverse than in White populations. These, allied to poor socioeconomic position, determine greater fat mass at age 7 years. METHODS: We report a secondary analysis from the UK Millennium Cohort Study, including 12,280 births of White ethnicity, and 358 of Indian, 650 of Pakistani, 268 of Bangladeshi, 163 of Black Caribbean and 277

Understanding support needs of African and African-Caribbean people living with dementia, their care partners and families, and impacts of delayed support: Identifying inclusive strategies to facilitate social care support: A study protocol.

African and African-Caribbean (AAC) people living with dementia (PLWD) are a population at high risk of inequitable access to health and social care services, and have poor health and wellbeing outcomes. Research suggests they are likely to experience failings in care, be recognised by services late and at points of crisis, and are at increased risk of institutionalisation. This research aims to examine the experiences of AAC PLWD, their care partners and families of accessing social care services and support. Employing an intersectionality theoretical framework, this study will use a multi-me

South African university students' experiences of online group cognitive behavioural therapy: Implications for delivering digital mental health interventions to young people.

Mental disorders are common among university students. In the face of a large treatment gap, resource constraints and low uptake of traditional in-person psychotherapy services by students, there has been interest in the role that digital mental health solutions could play in meeting students' mental health needs. This study is a cross-sectional, qualitative inquiry into university students' experiences of an online group cognitive behavioural therapy (GCBT) intervention. A total of 125 respondents who had participated in an online GCBT intervention completed a qualitative questionnaire, and 1

Evaluating Community Engagement Strategies to Manage Stigma in Two African Genomics Studies Involving People Living with Schizophrenia or Rheumatic Heart Disease.

In global health research and genomics research specifically, community engagement has gained prominence in enhancing ethical conduct, particularly in managing the risk of stigmatization, but there is minimal scientific evidence on how to do this effectively. This article reports on community engagement evaluation strategies in two African genomics studies: the Stigma in African Genomics Research study and the Genomics of Schizophrenia in South African Xhosa People (SAX) study. Within the Stigma in African Genomics Research study, a self-report rating scale and open-ended questions were used t

Even we are important: Sexuality and the degenderisation of people with disabilities in the linguistic landscapes of two South African universities in the Western Cape province.

BACKGROUND: This study focuses on the positioning of gender, sexual orientation and people with disabilities in the linguistic landscapes of two selected South African universities, which are located in the Western Cape province. OBJECTIVES: This study aims to answer the question: How are power relations depicted through linguistic landscaping in the universities? METHODS: Given that there is minimal empirical data in this field, the researcher approached this question by exploring the way in which sexual orientation and people with disabilities are perceived, via the modal resources used in

Delivery of health care for cardiovascular and metabolic diseases among people living with HIV/AIDS in African countries: a systematic review protocol.

BACKGROUND: People living with HIV (PLHIV) in African countries are living longer due to the rollout of antiretroviral drug therapy programs, but they are at increasing risk of non-communicable diseases (NCDs). However, there remain many gaps in detecting and treating NCDs in African health systems, and little is known about how NCDs are being managed among PLHIV. Developing integrated chronic care models that effectively prevent and treat NCDs among PLHIV requires an understanding of the current patterns of care delivery and the major barriers and facilitators to health care. We present a sys

Associations of common variants in the BST2 region with HIV-1 acquisition in African American and European American people who inject drugs.

OBJECTIVE: The bone marrow stromal cell antigen 2 (BST2) gene encodes a host restriction factor that acts as an innate immune sensor of HIV-1 exposure and suppresses release of HIV-1 particles. We aimed to identify associations of variants in the BST2 gene region with HIV-1 acquisition and disease progression. DESIGN/METHODS: Using HIV+ cases and HIV- controls from the Urban Health Study (n=3136 African Americans and European Americans who inject drugs), we tested 470 variants in BST2 and its flanking regions for association with HIV-1 acquisition and log-transformed viral load. RESULTS: We

Engagement with services in Black African and Caribbean people with psychosis: The role of social networks, illness perceptions, internalized stigma, and perceived discrimination.

OBJECTIVES: Research and policies in the United Kingdom have repeatedly highlighted the need to reduce ethnic disparities and improve engagement with mental health services among Black African and Caribbean people with psychosis. The aim of this study was to examine the role of social network characteristics and psychological factors in engagement with services in Black people with psychosis. METHODS: A cross-sectional study was conducted with 51 Black African and Caribbean adults with non-affective psychosis and currently receiving care from mental health services in England. Measures were c

Symptom Clusters in People Living with HIV Attending Five Palliative Care Facilities in Two Sub-Saharan African Countries: A Hierarchical Cluster Analysis.

BACKGROUND: Symptom research across conditions has historically focused on single symptoms, and the burden of multiple symptoms and their interactions has been relatively neglected especially in people living with HIV. Symptom cluster studies are required to set priorities in treatment planning, and to lessen the total symptom burden. This study aimed to identify and compare symptom clusters among people living with HIV attending five palliative care facilities in two sub-Saharan African countries. METHODS: Data from cross-sectional self-report of seven-day symptom prevalence on the 32-item M

'Find Your Float': A Public Health Approach to Addressing Drowning Inequities in People of African, Caribbean and Asian Heritage: A Cross-Sectional Study.

Drowning is a largely preventable global public health problem. Many unintended drownings are linked to the initial responses to cold-water immersion (cold shock), which includes a rapid involuntary cardio-respiratory response that increases the risk of aspirating water within the first two minutes of immersion. Floating rather than struggling to swim can help survival in this circumstance. In light of early research and subsequent changes in anthropometry, we are re-examining whether bone mineral density (BMD) in people from African, Caribbean, and Asian communities influences their float. Social determinants of health and long-term conditions in people of Black African and Black Caribbean ethnicity living with HIV in London: A qualitative study.

BACKGROUND: People living with human immunodeficiency virus (HIV) are disproportionately impacted by socioeconomic deprivation and are at increased risk of developing other long-term conditions (LTCs). These illnesses require transformative action to tackle the adverse effects on their health. Data on lived experiences of LTCs among people living with HIV of Black African and Black Caribbean ethnicities are sparse, and how people with LTCs are impacted by social determinants of health (SDoH). METHODS: Through a phenomenological study design this qualitative study, conducted in 2022, comprised
