Treating sickle cell disease in resource-limited sub-Saharan Africa: recent strategies and recommendations in addressing the gaps for the provision of evidence-based management.

INTRODUCTION: The management of Sickle cell disease (SCD) in sub-Saharan Africa (SSA) suffers from the lack of universal infant and population screening, inadequate access to standard treatment and poor public health prioritization amidst unstable political systems. AREAS COVERED: The state of evidencebased management of SCD in SSA was investigated including sustainability of international funding agencies. EXPERT OPINION: Current efforts are fragmentary along languages lines; sometimes driven by the funder's objectives and not the national agenda. The review highlighted the role of internal

Pathogenesis of African swine fever in domestic pigs and European wild boar - Lessons learned from recent animal trials.

Over the last decade, African swine fever (ASF) has changed from an exotic disease of Sub-Saharan Africa to a considerable and serious threat to pig industry in Central Europe and Asia. With the introduction of genotype II strains into the European Union in 2014, the disease has apparently found a fertile breeding ground in the abundant wild boar population. Upon infection with highly virulent ASF virus (ASFV), a haemorrhagic fever like illness with high lethality is seen in naïve domestic pigs and wild boar. Despite intensive research, virulence factors, host-virus interactions and pathogenes

Trichinellosis in developing countries: is it neglected?

Trichinellosis is a foodborne zoonosis caused by the parasitic nematode Trichinella, which is characterized by an extremely wide host range and geographical distribution. The aim of the present review is to provide epidemiological information on animal and human trichinellosis occurring in developing countries in the different continents, where cooking habits along with poverty and poor sanitary conditions and lack of veterinary controls may facilitate the occurrence of human trichinellosis outbreaks. Countries have been considered according to the six regions designated by the World Health Or

Consanguinity and rare neurological disease. A five year experience from the Korle Bu Teaching Hospital, Accra, Ghana.

INTRODUCTION: Marriage between close biological kin is not regarded as advantageous in the western world but in other parts of the world, consanguineous unions persist. Consanguineous marriage increases the birth prevalence of individuals with recessive disorders. In Accra, Ghana, consanguinity is beginning to emerge as a significant cause of rare neurological disease at the central referral hospital at Korle Bu in Ghana. METHOD: Documentation of rare neurological and genetic diseases over a five year period resulting from consanguinity (2010-2015) presenting to the Department of Child Health

From aid dependency to market transformation: harnessing international trade and innovative business models to revitalize the assistive technology ecosystem in Africa.

Africa faces a widening and largely unaddressed gap in access to assistive technology (AT). More than 240 million people on the continent require at least one assistive product, yet fewer than 3% of those in sub-Saharan Africa currently have access. The principal causes are four interlocking structural market failures: restrictive trade policies that inflate import costs, chronically underdeveloped local manufacturing capacity, inadequate national health financing for rehabilitation, and the absence of sustainable business models capable of maintaining AT services beyond the lifespan of time-l

Use of intramedullary nailing in poor sanitary conditions: French Military Medical Service experience.

INTRODUCTION: Intramedullary (IM) nailing is the gold standard for treating long bone fractures in developed countries because of its minimally-invasive application and good biomechanical properties. In precarious conditions, external fixation or open plate fixation are often performed because surgeons do not have the means to carry out IM nailing. However, these procedures can lead to infection-related complications and mechanical failures. The aim of our study was to describe the outcomes and postoperative complications of IM nailing of closed, long bone fractures in patients operated in a F

Diagnosis of childhood tuberculosis and host RNA expression in Africa.

BACKGROUND: Improved diagnostic tests for tuberculosis in children are needed. We hypothesized that transcriptional signatures of host blood could be used to distinguish tuberculosis from other diseases in African children who either were or were not infected with the human immunodeficiency virus (HIV). METHODS: The study population comprised prospective cohorts of children who were undergoing evaluation for suspected tuberculosis in South Africa (655 children), Malawi (701 children), and Kenya (1599 children). Patients were assigned to groups according to whether the diagnosis was culture-co

Ancient DNA challenges to admixture narratives: Insights from Notre-Dame Cemetery (1683-1796) in Colonial Montreal, Canada.

Historical narratives of early colonization in Quebec frequently portray French-Indigenous relations as cooperative and characterized by admixture; which are often mobilized in modern debates on false-indigeneity. These narratives have been studied through genealogical, genetic, and bioarchaeological analyses, yet individuals of African descent remain mostly absent from them, while Indigenous peoples are framed largely in relation to colonists. Notre-Dame Cemetery (1683-1796) in Tiohtià:ke-Montréal offers a unique window onto one of the earliest urban colonial populations in Nouvelle France, b

Incidence and characteristics of injuries during the 2011 West Africa Football Union (WAFU) Nations' Cup.

BACKGROUND: Prospective studies on football injuries and their risk factors in the African setting are sparse. Such studies are needed to understand the peculiarities of injuries and hence proffer appropriate intervention for injury prevention in the region. OBJECTIVE: To evaluate the incidence and characteristics of match injuries during the 2011 West Africa Football Union (WAFU) cup. METHODS: Team physiotherapists from the participating teams documented all newly incurred injuries on standardised injury report forms using the Federation of International Football Association's (FIFA) Medica

Searching the Silver trail: Charles van Onselen, Joe Silver and Jack the Ripper.

OBJECTIVE: South African historian Charles van Onselen's discovery of the identity of Jack the Ripper is described. The author was consulted by van Onselen during the writing of the latter's book, "The Fox and the Flies: Joe Silver, Racketeer and Psychopath", which suggested the identity of Jack the Ripper and was first published earlier this year. CONCLUSIONS: van Onselen's investigations into the life of Joe Silver, a Polish Jewish brothel owner in the early days of Johannesburg, revealed an extraordinary criminal career starting in London and covering the Atlantic zone before ending on the

Mpox in Sierra Leone and the Mano River Union: an epidemic unfolding in real time.

The 2025 mpox outbreak in Sierra Leone represents an unprecedented public health crisis, escalating rapidly from minimal prior cases to over 4400 confirmed infections within 6 months. This surge challenges previous assumptions about mpox epidemiology in West Africa and highlights critical vulnerabilities in disease surveillance, diagnostics and health system preparedness-challenges shared across the Mano River Union (MRU) region, including Liberia, Guinea and Côte d'Ivoire. Despite global commitments to equitable vaccine access, Sierra Leone and its neighbours have faced significant delays and

Multicountry genomic analysis underscores regional cholera spread in Africa.

Cholera remains a significant public health burden in many countries in sub-Saharan Africa, though the exact mechanisms of bacterial emergence and spread remain largely undefined. We generated genomic data from 763 Vibrio cholerae O1 isolates predominantly collected between 2019-2024 to create the largest dataset of V. cholerae genomes sequenced locally in Africa. This dataset enabled us to interrogate recent patterns of spread, including the rapid circulation of the AFR15 lineage associated with unusually large outbreaks in Southern Africa. We provide evidence for the movement of the AFR15 li

Plasmodium falciparum sexual conversion rates can be affected by artemisinin-based treatment in naturally infected malaria patients.

BACKGROUND: Artemisinins (ART) are the key component of the frontline antimalarial treatment, but their impact on Plasmodium falciparum sexual conversion rates in natural malaria infections remains unknown. This is an important knowledge gap because sexual conversion rates determine the relative parasite investment between maintaining infection in the same human host and transmission to mosquitoes. METHODS: The primary outcome of this study was to assess the impact of ART-based treatment on sexual conversion rates by comparing the relative transcript levels of pfap2-g and other sexual ring bi

Gender-based violence against young women: A comparative analysis of cross-sectional surveys of 11 sub-Saharan African countries.

Despite several national and international strategic efforts against gender-based violence, the problem persists, particularly in sub Saharan Africa (SSA), where thirty percent of women report the experiences of physical or sexual violence in their lifetime. The objective of this study was to determine the prevalence and determinants of gender-based violence among young women aged 15 24 in eleven sub-Saharan African (SSA) countries. A total of 68,186 young women aged 15-24 were pooled from the Demographic and Health Surveys (DHS) data of eleven SSA countries on the basis of availability of nat

The 2018-2020 Ebola Outbreak in the Democratic Republic of Congo: A Better Response Had Been Achieved Through Inter-State Coordination in Africa.

The Ebola virus disease, formerly known as Ebola hemorrhagic fever, is a severe and often fatal zoonosis in humans. The 2013-2016 West African Ebola outbreak had distinctive characteristics, and it was the largest and most complex epidemic since the virus discovery in 1976. Although the 2018-2020 Ebola outbreak in the Democratic Republic of the Congo had many similarities, there were additional challenges due to the presence of armed rebel groups at the epicenters of the epidemic. Despite these challenges, the extraordinary commitment of the World Health Organization (WHO) regional office for

The CEO of Anglo American on getting serious about safety.

When Carroll became the EO of Anglo American, in 2007, the company had suffered 200 worker fatalities over the previous five years. Some company veterans insisted that deaths were inevitable at such a large mining company, given the dangerous nature of the business. Carroll fundamentally rejected that assumption. She undertook a tour of operations in Australia, Chile, Colombia, Venezuela, and South Africa. The platinum business in South Africa had the worst track record in terms of safety, and Carroll was troubled by her conversations with its local managers. Immediately after her visit, anoth
