INTRODUCTION: The Galway Child and Adolescent Sexual Assault Treatment Service (CASATS) operates within a Barnahus model on site on weekdays from 08.00 to 16.00 h with 24/7/365 out of hours on call availability. The service is localised to West and Mid-West Ireland for "in hours" examinations but covers the whole of Southern Ireland for "out of hours" examinations. Whilst CASATS supports children under 18 years of age, "out of hours" forensic medical examinations (FME) are predominantly children under 14 years of age as the adult sexual assault treatment units (SATU) in Ireland offer acute FME

Each year, British Columbia's Health Care Protection Program reviews up to 20 reported cases of compartment syndrome. This article summarizes some of the key learnings from a review of those reported incidents. Hospital staff who care for orthopedic trauma patients need to be vigilant to the signs and symptoms of compartment syndrome and they must document their findings and interventions. Early intervention can prevent further serious harm to patients who most often have already suffered trauma in a sports or motor vehicle accident.

In Y-stenting, stabilization of the first stent may be problematic as in some cases it migrates during second stent insertion. This report evaluates the safety and effectiveness of the technique and presents the long-term results of hybrid, Y-configured, dual stent-assisted coil embolization in the treatment of wide-necked bifurcation aneurysms. We retrospectively evaluated the patients treated endovascularly due to cerebral aneurysms. Twenty patients treated with hybrid Y-stent-assisted coil embolization were enrolled in the study. In hybrid stenting, an open-cell intracranial stent (Neurofor

BACKGROUND: There is significant variation in the mortality rates of patients with a ruptured abdominal aortic aneurysm (rAAA) admitted to hospital in England. This study sought to investigate whether modifiable differences in hospital structures and processes were associated with differences in patient outcome.
METHODS: Patients diagnosed with rAAA between 2005 and 2010 were extracted from the Hospital Episode Statistics database. After risk adjustment, hospitals were grouped into low-mortality outlier, expected mortality and high-mortality outlier categories. Hospital Trust-level structure

PURPOSE: To minimize cold ischemia time, transplantations with kidneys from deceased donors are frequently performed during the night. However, sleep deprivation of those who perform the transplantation may have adverse effects on cognitive and psychomotor performance and may cause reduced cognitive flexibility. We hypothesize that renal transplantations performed during the night are associated with an increased incidence of pure technical graft failure.
METHODS: A retrospective analysis of data of the Dutch Organ Transplant Registry concerning all transplants from deceased donors between 20

OBJECTIVE: Telephone medicine is part of clinical practice, but there are no published data on the volume, nature, and time allocation of patient-related telephone calls received in a movement disorders center. Such data might provide insights which augment patient care, and may be instructive regarding medical education, since patient-related telephone calls are often addressed by physicians-in-training.
METHODS: Characteristics of patient-related calls to a movement disorders center were prospectively recorded during a 2-month period.
RESULTS: A total of 633 calls were generated by 397 pat

BACKGROUND: UK General Internal Medicine (GIM) registrars are traditionally expected to be able to perform pleural procedures out-of-hours (OOH) if required. Pleural exposure for GIM registrars has declined, with procedures increasingly performed by specialists in-hours. Pleural procedures can cause significant harm. We explored the role of the GIM registrar in performing OOH pleural procedures, with particular focus on competence and confidence.
METHODS: An anonymous survey was distributed to GIM registrars and respiratory consultants across the UK.
RESULTS: Responses were received from 391

OBJECTIVES: Testicular torsion (TT) is an emergency requiring timely surgery to prevent testicular loss. There is a lack of reports on the clinical significance of the time of admission (on-hours vs. off-hours) on the long-term surgical outcome of TT.
METHODS: We retrospectively reviewed all consecutive patients <18 years who were admitted to the hospital and treated for TT during the ten years. Patients were classified according to their admission time: weekday (on-hours), outside working hours, and weekends (off-hours). They were also classified based on their testicular outcome: salvaged a

INTRODUCTION: Little is known about the management and disease trajectories of heart failure (HF) patients in the pre-hospital setting when experiencing exacerbating symptoms. Shortness of breath (SOB) is a main reason for contacting out-of-hours primary care (OHS-PC). We aim to describe the care trajectories of these patients with HF at the OHS-PC with exacerbating symptom SOB and to assess 6-month outcomes regarding mortality and hospital admissions.
METHODS: We included patients who contacted Dutch OHS-PC for SOB between September 2020 and August 2021. We selected those in whom HF was cons

BACKGROUND: Anesthesiologists providing care during off hours (ie, weekends or holidays, or cases started during the evening or late afternoon) are more likely to care for patients at greater risk of sustaining major adverse events than when they work during regular hours (eg, Monday through Friday, from 7:00 AM to 2:59 PM). We consider the logical inconsistency of using subspecialty teams during regular hours but not during weekends or evenings.
METHODS: We analyzed data from the Anesthesia Quality Institute's National Anesthesia Clinical Outcomes Registry (NACOR). Among the hospitals in the

BACKGROUND: Children with life-limiting conditions often have complex care needs, and their caregivers require expert clinical support after-hours and over weekends to support care at home.
OBJECTIVES: This quality improvement initiative examined usage patterns and impact on caregiving of the nurse-led 24-Hour Clinical Care Line initiated in 2018 by the Canuck Place Children's Hospice, which provides inpatient and community-based pediatric palliative care and respite for children with life-limiting conditions in British Columbia and the Yukon in Canada.

INTRODUCTION: Cauda equina syndrome (CES) develops due to compression of cauda equina nerve roots and requires urgent diagnosis, preferably using MRI. This will allow timely intervention to prevent irreversible neurological problems. On-call services can potentially reduce time taken to diagnose CES outside of standard operational hours. Most acute hospitals in the UK do not have on-call provisions for CES. This study therefore assessed whether introducing a short period of on-call service at an acute hospital significantly reduced the time in diagnosing CES. This can form the decision for fut

PURPOSE: To investigate patient's perception of the severity of their symptoms, reasons for attending an ophthalmic emergency department (ED) out of hours, and to review the prevalence of anxiety and depression.
METHODS: We carried out a prospective analysis of the cases presenting out of hours (8:30 PM to 8:30 AM) over a four-month period. We also conducted two questionnaire studies. First, patient's perception of the severity of their symptoms (graded from 1-10). A score of 7 or above was defined as significant. A second questionnaire study used the Hospital Anxiety and Depression Scale (HA

BACKGROUND: In the healthcare system, in the last 30 years, the prognostically negative value of the so-called Weekend Effect (WE) has been internationally recognized. The WE is regarded as the increased risk a patient might incur when hospitalized during non-working days, of enduring severe complications in comparison to the same hospitalization that occur on working days. The aim of this study was to retrospectively verify whether, once a mistake was made during weekends or on holidays, in comparison to a mistake occurred on workdays, it subsequently implied a higher risk of complications, d

BACKGROUND: Patients newly insured through coverage expansion under the Affordable Care Act (ACA) may have difficulty obtaining timely primary care follow-up appointments after emergency department (ED) discharge. We evaluated the association between availability of timely follow-up appointment with practice access improvements, including patient-centered medical home (PCMH) designations or extended-hours appointments.
METHODS: We performed a secret-shopper audit of primary care practices in greater New Haven, Connecticut. Two callers, posing as patients discharged from the ED, called these p

PURPOSE: Prompt and accurate diagnosis of spinal cord compression (SCC) or cauda equina compression (CEC) is of great clinical importance. The purpose of this study is to evaluate the effect of a standardized reporting template (SRT) on report turnaround time (RTAT) and performance of on-call radiology residents’ preliminary interpretation (PI) for the detection of SCC/CEC on after-hour spine MRI studies. METHODS: We retrospectively reviewed a total of 266 spine MRI studies performed after hours for SCC/CEC, before and after the implementation of a SRT to be used by on-call radiology residents

IMPORTANCE: The phenomenon of a weekend effect refers to a higher potential for adverse outcomes in patients receiving care over the weekend. Few prior studies have comprehensively investigated the effects of postoperative weekend care on surgical outcomes in a generalizable cohort.
OBJECTIVE: To examine differences in short-term and long-term postoperative outcomes of patients undergoing surgical procedures immediately before vs after the weekend.
