Comparative efficacy of akathisia treatments: A network meta-analysis.

OBJECTIVE: Akathisia, a common side effect of psychotropic medications, poses a significant challenge in neuropsychiatry, affecting up to 30% of patients on antipsychotics. Despite its prevalence, akathisia remains poorly understood, with difficulties in diagnosis, patient reporting, and treatment efficacy. This research aimed to shed light on effective interventions to improve akathisia management. METHODS: A systematic review following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines was conducted, encompassing controlled trials in English and Italian l

Exaggerated psychomotor response to intravenous bolus dose administration of ketamine.

The case of a 40-year old female patient with exaggerated psychomotor response to intravenous bolus dose administration of ketamine is presented. The mechanism of action and some of the untoward effects of the drug are discussed. The need for proper evaluation of underlying psychiatric history, especially in the elderly, before the administration of the drug is stressed.

Postburn delirium associated with use of intravenous lorazepam.

A case of delirium with persecutory delusions is reported, which occurred three days after a previously healthy male suffered burns over 30% of his total body surface area. Routine evaluation was negative for other etiology of delirium, and the symptoms spontaneously remitted within seven hours with no sequella. We found that the benefits and potential side effects of intravenous lorazepam are similar to but of longer duration than those of intravenous diazepam. The effects of intravenous lorazepam should be recognized in the differential diagnosis of postburn delirium in patients given this m

Placebo-level incidence of extrapyramidal symptoms (EPS) with quetiapine in controlled studies of patients with bipolar mania.

OBJECTIVES: To evaluate extrapyramidal symptoms (EPS), including akathisia, with quetiapine in patients with bipolar mania. METHODS: Data were analyzed from four similarly designed, randomized, double-blind, 3- to 12-week studies. Two studies evaluated quetiapine monotherapy (up to 800 mg/day) (n = 209) versus placebo (n = 198), with lithium or haloperidol monotherapy as respective active controls. Two studies evaluated quetiapine (up to 800 mg/day) in combination with a mood stabilizer (lithium or divalproex, QTP + Li/DVP) (n = 196) compared to placebo and mood stabilizer (PBO + Li/DVP) (n =

[Paraesthesia in the legs].

Paraesthesia in the legs can have numerous causes. In addition to the restless legs syndrome, other primary causes include venous insufficiency in the leg, propriospinal myoclonus, nocturnal leg cramps, peripheral polyneuropathy that affects mostly the legs or neuroleptic drug-induced akathisia. Through detailed questioning of the patient, restless legs syndrome can be specifically distinguished from the other named differential diagnoses.

Restless legs syndrome probably induced by risperidone treatment.

BACKGROUND: According to some reports, patients treated with risperidone may develop akathisia. Restless legs syndrome (RLS), which shares some clinical features with akathisia, is a distinct movement and sleep disorder that may be induced by various drugs that act on the CNS. METHODS: We studied a 31-year-old patient suffering from a schizoaffective disorder including auditory hallucinations who developed symptoms typical of the RLS during treatment with risperidone, using polysomnography to objectify sleep disturbances and associated periodic leg movements during sleep (PLMS). RESULTS: Aft

Neuroleptic-associated tardive syndromes.

We have briefly reviewed the literature on late-onset akathisia, dystonia, and Tourette-like syndrome in patients on long-term neuroleptic treatment. To date, there is no satisfactory epidemiologic or other evidence directly implicating neuroleptics in the etiology of these so-called tardive syndromes. Similarities between these disorders and tardive dyskinesia, however, make them worthy of some consideration.

Carisoprodol-induced myoclonic encephalopathy.

CASE REPORT: A 39-year-old man ingested 35 g carisoprodol. He developed agitation, tachycardia, myoclonus, and coma. The blood carisoprodol was 71 micrograms/mL; the meprobamate was 26 micrograms/mL. DISCUSSION: Carisoprodol overdose is thought to induce simple central nervous system depression. This case demonstrates a severe overdose with symptoms more consistent with myoclonic encephalopathy. A review of cases presenting to the San Francisco Division of the California Poison Control System during 1997 suggests that carisoprodol is more commonly associated with agitation and bizarre movemen

Effects and risks associated with novel psychoactive substances: mislabeling and sale as bath salts, spice, and research chemicals.

BACKGROUND: The number of newly reported psychoactive substances in Europe is now higher than ever. In order to evade legal restrictions, old and novel psychoactive substances from medical research and their derivatives are commonly mislabeled as "not for human consumption" and offered for sale on the Internet and elsewhere. Such substances are widely taken by young people as "club drugs." Their consumption must be considered in the differential diagnosis of psychiatric, neurological, cardiovascular, or metabolic disturbances of unclear origin in a young patient. METHOD: Selective review of p

Acute dyskinesia, myoclonus, and akathisa in an adolescent male abusing quetiapine via nasal insufflation: a case study.

BACKGROUND: Although the benefits of antipsychotic pharmacotherapy can be pronounced, many patients develop unwanted adverse effects including a variety of movement disorders. Compared with the traditional antipsychotics, the atypical antipsychotics have a decreased risk for associated movement disorders. Drug-induced movement disorders can occur, however, and the risk of adverse events can increase significantly when medications are abused. CASE PRESENTATION: We describe the case of a 13-year-old male who presented to an emergency department with acute movement disorders after nasal insuffla

[Psychiatric symptoms due to theophylline overdose; the necessity of blood level monitoring].

Two patients presented with severe side effects of theophylline. Due to the theophylline intoxication patient A developed ischaemic hepatitis, which made haemodialysis treatment necessary to accelerate the elimination of the drug from the body. The intoxication caused immobilisation and total dependence with respect to the daily activities of patient B. In neither case was the association between mood disorders--like depression, agitation and anxiety--and an overdose of theophylline recognised. Administration of theophylline should be monitored with regular measurements of the serum theophylli

Akathisia: a life-threatening side effect of a common medication.

The authors describe the case of a 38-year-old man with a history of schizoaffective disorder, who attempted suicide following the recent starting of a neuroleptic agent that resulted in the development of intolerable akathisia. He survived the attempt, and following changes in his medications the akathisia resolved with no further suicidal ideation.

Carbamazepine-induced hyperammonemia.

PURPOSE: A case of carbamazepine-induced hyperammonemia is presented. SUMMARY: A 26-year-old man with bipolar disorder, seizures, and mild mental retardation secondary to a traumatic brain injury began treatment with carbamazepine for aggression and seizure control. After three weeks of carbamazepine therapy, the patient arrived at the emergency department (ED) with severe agitation and aggressive behavior. His oral medications included topiramate, carbamazepine, olanzapine, quetiapine, guanfacine, and desmopressin acetate. The patient's medications had been stable for at least six months exc

[The serotonin syndrome: review of the literature and description of an original study].

In animals the occurrence of a behavioural syndrome consisting of hyperactivity, stereotyped movements and increase of temperature has been induced by MAOIs, 5-HT precursors (L-tryptophan) and 5-HT reuptake inhibitors. Most of these manifestations were specifically blocked by a pretreatment with an inhibitor of serotonin synthesis. In humans, the association of myoclonus, diarrhea, confusion, hypomania, agitation, hyperreflexia, shivering, incoordination, fever and diaphoresis, when patients are treated with serotoninergic agents, could constitute a "serotonin syndrome". Such cases of serotoni

[General anesthesia with continuous-infusion ketamine].

The importance of Ketamine in continuous perfusion is described on the basis of a studied carried out in 30 patients. The clinical data are compared with results predicted by the mathematical model. Special study was made of the behaviour of the cardiocirculatory apparatus, particularly in reference to myocardial contractility and electroencephalographic activity. It is concluded that the drug can be advantageously used in continuous perfusion.

Toxicity induced by multiple high doses of vitamin B12 during pernicious anemia treatment: a case report.

Context: The clinical consequences of excess vitamin B12 induced by multiple oral doses of cyanocobalamin are not well-known.Case details: A young woman was treated with multiple daily doses of 1 mg of cyanocobalamin for severe pernicious anemia. After a total dose of 12 mg, she developed acne, palpitations, anxiety, akathisia, facial ruddiness, headache, and insomnia. She improved two weeks after stopping the drug. There were no sequelae nor complications.Discussion: Although these symptoms of cobalamin toxicity were unexpected and unusual, the case reminds us that the administration of any d
