Alkaline phosphatase (ALP) is abundantly represented in nature, being fundamental for a number of processes. In addition to its fundamental function in skeletal mineralization, its roles in the pathogenesis of other diseases are being explored. The measurement of total ALP activity in serum or in plasma is a useful biomarker in clinical practice. Indeed, routine measurement of serum total ALP is a long-standing established part of initial biochemical evaluation of patients both in the hospital setting and on an ambulatory basis. Raised or reduced values of this enzyme activity are indicative o…

Antibodies to the alkaline phosphatase (AP) of Schistosoma mansoni in infected human and mice sera were evaluated by a direct solid-phase AP immunoadsorption assay (APIA) and by Western blot and immunostaining. APIA consisted of (a) solid-phase capture of immunoglobulins from infected human or mice, (b) immunoadsorption of the enzyme antigen by the antibodies, and (c) detection of the enzymatic activity. By this procedure the appearance of the anti-AP response in mice was detected around 50 days post-infection; this response was not specific for an AP of a given schistosome strain and it was n…

Alkaline phosphatase of the placental type in serum has been suggested as a "marker" for malignancy and pregnancy. We describe a highly sensitive enzyme-linked immunosorbent assay (ELISA) for the measurement of this enzyme in serum and ascitic fluid. The assay will detect as little as 0.4 microgram/L, significantly less than with a radioimmunoassay performed with the same reagents. It is highly specific; it does not measure even above-normal concentrations of the intestinal and liver isoenzymes of alkaline phosphatase. The assay is technically simple and allows the processing of many samples i…

A study using 72 beef cows and their calves was carried out over a one year period in Northern Ontario to examine the serum alkaline phosphatase (SAP) levels of the animals during the onset of hypomagnesaemia. No evidence was found of a fall in SAP corresponding to the dramatic decreases in serum magnesium. At the same time, other factors relating to SAP were considered. The relative SAP levels of the cows were found to be remarkably consistent with respect to time, but those of the calves less so. The SAP levels of the calves were considerably higher throughout than those of the cows, though …

A patient with primary biliary cirrhosis is described. Throughout her illness the serum alkaline phosphatase remained normal and this unique feature emphasizes the importance of histology in diagnosing the disease.

Three patients had subacute thyroiditis and elevated serum alkaline phosphatase, presumably related to the thyroiditis. Concomitant elevation of the serum gamma-glutamyl transferase suggested that the alkaline phosphatase was of hepatic origin. The elevation of the serum alkaline phosphatase could not be definitely related to the degree or duration of elevation of the serum thyroxine. The combination of elevated serum alkaline phosphatase with the systemic symptoms of subacute thyroiditis may obscure the diagnosis, especially if patients have little or no neck pain.

In 302 patients with tumors of the cervix, corpus uteri, and ovaries, assessment by clinical staging (tumors-nodules-metastasis system) (4) and histopathology has been related to the presence of serum heat-stable, placenta-lide alkaline phosphatase (PLAP) activity. Early stages of cervical tumors show the highest incidence of this isoenzyme. In advanced stages of this disease, a decrease in frequency was observed that might be interpreted as the result of gradual dedifferentiation of the tumor cells to a point where synthesis of PLAP became undetectable. The same observation was made in adenoc…

The total alkaline phosphatase levels were determined in 118 patients with adult coeliac disease, 65 of whom had evidence of osteomalacia. The secretor and blood group status, intestinal isoenzyme, and liver alkaline phosphatase played no significant part in the levels of serum alkaline phosphatase found. There was a significant correlation between the levels of serum alkaline phosphatase and the presence of osteomalacia.

Alkaline phosphatase prepared from human placentae shows greater resistance to heat inactivation than any other known alkaline phosphatase of human origin. In the presence of magnesium this enzyme may be heated at 70 degrees C. for 30 minutes without loss of activity whereas other human alkaline phosphatases lose most of their activity on being heated at 56 degrees C. for this period of time. This heat stability is seen in freshly prepared enzyme, in alcohol-fractionated and freeze-dried material, and in the sera of individuals into whom placental alkaline phosphatase has been infused. The cli…
