Calcitonin is used in the treatment of osteoporosis and several adverse effects, such as rash, antibody presence, hypocalcemia, etc have been reported with the therapeutic use of large quantities of this hormone. The results found in this paper show that 7 h after administration of large doses of calcitonin to osteoporotic patients on hormone treatment, the acid-base status shifts to metabolic alkalosis. This represents another adverse effect of the use of pharmacological doses of calcitonin.

The physiology of respiratory control of acid-base balance is reviewed. The pathophysiological mechanisms during hypercapnia and hypocapnia are discussed in the light of the causes and clinical manifestations of these disturbances. In addition to the role of the kidney in the compensatory processes of these disturbances, renal functional changes during acute and chronic pulmonary acid-base derangement is discussed. Some of the difficulties encountered in the patient with chronic renal disease in whom respiratory abnormalities may be present are also discussed.

Studies have been carried out in human volunteer subjects to evaluate the role of aldosterone in the development, maintenance, and correction of metabolic alkalosis induced by selective depletion of hydrochloric acid. During the first phase of our study the rate of aldosterone secretion was measured before the induction of alkalosis (while the subjects were on a low salt diet) and again after a steady state of metabolic alkalosis had been established.
