PURPOSE: To contribute toward assessing the effectiveness of polymerase chain reaction as a rapid method in diagnosis of torpid keratitis caused by opportunistic fungi.
METHODS: Interventional case report. A 50-year-old man with a corneal abscess in the right eye treated for a period of 6 months with different combinations of broad-spectrum antibiotics and steroids was referred to our center. Corneal scraping was taken for microbiological study, including classic cultures and polymerase chain reaction. Amplified DNA was sequenced to identify the pathogen.

Specific immunotherapy is ineffective with unstandardized mold extracts. A double-blind, placebo-controlled study was performed in 24 patients (5 to 56 years of age) only allergic to Alternaria. Primary erythromelalgia (PEM) is a rare neuropathic pain disorder characterized by debilitating burning pain of the extremities relieved by cold exposure. We report a pediatric patient who developed severe, full-thickness lower extremity ulcerations infected with Alternaria alternata and complicated by osteomyelitis following consistent direct skin cooling with window air conditioning. This case demonstrates serious infectious complications that can arise from cooling measures commonly employed for PEM symptom relief.

Alternaria alternata is a dematiaceous mold capable of causing systemic and dermatological infections in immunosuppressed patients. We present a case of a liver transplant patient with cutaneous alternariosis and no response to amphotericin B lipid or surgical debridement of the lesions. Treatment with intravenous voriconazole was initiated, followed by the oral protocol, with complete resolution of the lesions. Voriconazole is an efficient alternative for the treatment of cutaneous lesions produced by A alternata.

Verlamelin A is a macrocyclic antifungal depsipeptide. In this study, a series of derivatives (1a-15a, 17a, 1b- 17b) were developed by replacing the l-Gln residue in verlamelin A with other proteinogenic l-amino acids. In order to determine the prevalence of airborne mould sensitization and the reliability of the in vitro diagnostic techniques in daily practice (antigen-specific IgE and histamine release test), we performed a 3-year study in 2,200 patients diagnosed with rhinosinusitis and/or bronchial asthma.
