How to perform and report a cardiopulmonary exercise test in patients with chronic heart failure.

In the present practice review, we will explain how to perform and interpret a cardiopulmonary exercise test (CPET) in heart failure patients. Specifically, we will explain why cycle ergometer should be preferred to treadmill, the type of protocol needed, and the ideal exercise duration. Thereafter, we will discuss how to interpret CPET findings and determine the parameters that should be included. We will focus specifically on: peak VO2 (absolute value and a percentage of its predicted value), exercise duration, respiratory exchange ratio, peak work rate, heart rate, O2 pulse, end-tidal carbo

[Independence of hyperlactacidemia and hyperventilation threshold work-load exercise in man].

During exercises at constant workload, the hyperlactatemia threshold (continuous lactate production during the whole exercise) (48% VO2 max) and the hyperventilation threshold (57% VO2 max) are not simultaneous. This finding demonstrates that the hyperventilation threshold could not be regarded as an "anaerobic threshold".

Physiological responses during walking in men and women with intermittent claudication.

BACKGROUND: Miyasato et al. show that peak oxygen consumption, walking economy, anaerobic threshold, and cardiovascular responses (heart rate, blood pressure, and rate pressure product) during walking were similar between men and women with peripheral artery disease and intermittent claudication. There were no differences in the physiological responses to walking between men and women with intermittent claudication. Sex per se is not a factor that demands changes in walking prescription for patients with intermittent claudication. OBJECTIVE: Peak oxygen consumption (VO2peak), anaerobic thresh

Effects of movement and work load in patients with congenital central hypoventilation syndrome.

BACKGROUND: Patients with congenital central hypoventilation syndrome lack ventilatory chemosensitivity and depend at least in part on the ergoreceptor function during exercise. In these patients a substantial increase of ventilation has been reported for passive movement during sleep as well as active movement on a treadmill. OBJECTIVES: The aim of the study was to investigate ventilatory response to an increasing work load with constant movement. METHODS AND RESULTS: Eighteen patients and 17 healthy volunteers performed a cardiopulmonary exercise test on a bicycle pedaling at a constant ra

Cardiopulmonary exercise testing applied to respiratory medicine: Myths and facts.

Cardiopulmonary exercise testing (CPET) remains poorly understood and, consequently, largely underused in respiratory medicine. In addition to a widespread lack of knowledge of integrative physiology, several tenets of CPET interpretation have relevant controversies and limitations which should be appropriately recognized. With the intent to provide a roadmap for the pulmonologist to realistically calibrate their expectations towards CPET, a collection of deeply entrenched beliefs is critically discussed. They include a) the actual role of CPET in uncovering the cause(s) of dyspnoea of unknown

Using Multilevel Models to Compare Performance Prediction and Characterization Abilities Between Power-Law and Critical-Speed Models in Middle- and Long-Distance Running.

PURPOSE: Speed-duration relationships are widely used to predict performances and characterize athletes. The critical-speed (CS) and power-law (PL) models share the spotlight for practitioners. Therefore, the present study compares performance prediction accuracy and characterization abilities between PL and CS models using a multilevel model (MLM) for middle- and long-distance runners. METHODS: Data from 184,755 performances by 52,847 athletes from France, Great Britain, Italy, Poland, and Switzerland across 6 events (400-10,000 m running) were computed. MLMs were developed based on the hier

The effect of menstrual cycle and exercise intensity on psychological and physiological responses in healthy eumenorrheic women.

Psychological responses such as affect, mood, motivation, anxiety are important considerations for exercise adherence. A large body of evidence indicates that exercise intensity is a controllable variable capable of increasing positive feelings. Investigations thus far, however, have not considered that the higher incidence of negative psychological responses in women could be due to physiological particularities of the menstrual cycle (MC). This project investigates the effect of MC phases and exercise intensity on psychophysiological responses in fourteen healthy, eumenorrheic and physically

Pulmonary vasodilators and exercise in Fontan circulation: a systematic review and meta-analysis.

OBJECTIVE: In Fontan circulation, pulmonary arterial hypertension (PAH)-targeted therapies could improve the patients' exercise capacity. This study aimed to investigate the effects of PAH agents on different exercise parameters in stable Fontan patients by synthesising evidence of randomised controlled trials (RCTs). METHODS: A systematic search of PubMed, Cochrane Central Register of Controlled Trials and Web of Science databases, as well as of ClinicalTrials.gov, was performed. Primary outcomes were specific cardiopulmonary exercise test parameters: peak oxygen uptake (peak VO2), peak hear

Oxygen uptake is more efficient in idiopathic pulmonary arterial hypertension than in chronic thromboembolic pulmonary hypertension.

BACKGROUND AND OBJECTIVE: The responses of oxygen uptake efficiency (OUE) during cardiopulmonary exercise training (CPET) have not been reported in patients with pulmonary hypertension. We aimed to investigate the differences in OUE between patients with idiopathic pulmonary arterial hypertension (IPAH) and chronic thromboembolic pulmonary hypertension (CTEPH). METHODS: Forty-four patients with IPAH and 29 patients with CTEPH were retrospectively enrolled into our study. All patients underwent right-heart catheterization, pulmonary function test and performed the 6-min walk test and CPET. RE

Respiratory gas exchange and metabolic responses during exercise in McArdle's disease.

During normal progressive exercise, the gas exchange anaerobic threshold occurs when CO2 production (VCO2) and ventilation (VE) increase so as to depart from a linear relationship to O2 consumption (VO2). This is thought to represent a gas exchange response to metabolic acidosis due to lactate accumulation. Patients with McArdle's disease have previously been reported to exhibit a steepened ventilatory response relative to VCO2, despite an inability to produce lactate. However, the VCO2 response has not been studied. We therefore investigated the VCO2-VO2 and VE-VO2 relationships in seven McAr

Cardiopulmonary Exercise Testing as a Predictor of Postoperative Outcome in Patients Undergoing Oesophageal Cancer Surgery Following Neoadjuvant Chemotherapy.

OBJECTIVE: Neoadjuvant chemotherapy improves resectability rates of oesophageal cancer, but the process may also take a toll on the patients' exercise capacity and may adversely affect the postoperative outcomes. It can be assessed objectively using cardiopulmonary exercise testing. METHODS: Patients with oesophagus cancer performed a baseline test and a second test after neoadjuvant chemotherapy during the week preceding oesophagectomy. They were followed up for postoperative complications, length of hospital stay, and 30-day mortality. RESULTS: Thirty-three patients completed the study. Th

Contribution of peripheral chemoreceptor drive in exercise hyperpnea in humans.

The peripheral chemoreceptors play a dominant role in the respiratory compensation of lactic acidosis during heavy exercise of humans. Our object was to determine the contribution of peripheral chemoreceptors to exercise hyperpnea during mild to moderate and heavy exercise above the anaerobic threshold. We used a hyperoxic suppression test in six normal male subjects. Inspired gas was abruptly changed without the subject's knowledge from air to pure oxygen for 5 to 6 breaths. The maximal ventilatory depression after O2 breathing was 5.5 +/- 1.7 L/min (BTPS) at mild exercise, and the depression

Role of lactic acidosis in the ventilatory response to heavy exercise.

The purpose of this study was to determine the role of lactic acidosis in the ventilatory response to heavy exercise above anaerobic threshold. Seven subjects ingested either NaHCO3 or CaCO3 at a dose of 300 mg/kg body weight and ran on a motor-driven treadmill at a work load corresponding to 90% of VO2max and above anaerobic threshold for a period of 5 min while minute ventilation and PETCO2 were recorded breath by breath. A total of 10 runs, 5 with CaCO3 and 5 with NaHCO3 in a randomized and blind order, were done in each subject. Statistical analyses of the effects of the chemicals on minut

Submaximal exercise metrics: Looking prior to VO₂max : in Heart Failure.

Maximal oxygen uptake (VO₂max) remains the benchmark for cardiopulmonary exercise testing (CPET) in chronic heart failure (CHF). However, VO₂max and its surrogate, VO₂peak, often fail to capture the full spectrum of functional impairment, especially in patients unable to reach maximal effort. This review delineates submaximal CPET-derived metrics that offer valuable prognostic and diagnostic insights beyond VO₂max and are less dependent on peak effort. Guideline-endorsed parameters such as ventilatory anaerobic threshold (VAT) and the minute ventilation–carbon dioxide production slope (VE/VCO₂

Systematic review of prognostic value of cardiorespiratory fitness in patients with cirrhosis.

BACKGROUND: Cirrhosis is associated with significant risk of comorbidity and early mortality. Low physical function is common in patients with cirrhosis and could predict prognosis. Cardiorespiratory fitness (CRF), determined by maximal exercise with gas exchange measurement, has proven to predict the risk of mortality and disability in other chronic diseases. In patients with cirrhosis, it could help to inform prognostic stratification to improve care and management in clinical practice. This systematic review aims to determine the association between CRF (VO2Peak, percentage of predicted VO2
