Delayed selective cerebral hypothermia decreases infarct volume after reperfused stroke in baboons.

BACKGROUND: Hypothermia is known to provide neuroprotection from focal ischemia. However, lethal cardiovascular complications resulting from total body cooling have greatly restricted hypothermia as a therapy for stroke. This study determined whether selective cerebral cooling induced after reversible cerebral artery occlusion would decrease the infarct volume. METHODS: Under general anesthesia, 8 baboons were subjected to 1-hour simultaneous occlusion of the left internal carotid artery and anterior cerebral arteries by transorbital surgical approach. Four animals were treated with selective

Effect of oxygen and sodium thiosulfate during combined carbon monoxide and cyanide poisoning.

In a canine model of combined carbon monoxide (CO) and cyanide (CN) poisoning, cardiac output (QT) and oxygen consumption (Vo2) decreased but recovered to baseline values by 15 min after toxic exposure; elevated blood CN and lactic acidosis persisted for at least another 10 min. Given the rapid spontaneous recovery after cessation of toxic exposure, we questioned the efficacy of usual treatment with oxygen (O2) and sodium thiosulfate (Na2S2O3) for CN poisoning. Accordingly, in seven dogs (26 +/- 3 kg, chloralose and urethane anesthesia), we sequentially administered CO by closed circuit inhala

Clinical trial with surgery and intraperitoneal hyperthermic perfusion for peritoneal recurrence of gastrointestinal cancer.

To treat six patients with peritoneal recurrence after radical operation for gastrointestinal cancer, an intraperitoneal hyperthermic perfusion (IPHP), combined with surgical resection of recurrent tumors, intestinal by-pass anastomosis, or both, was carried out. Immediately after complete resection of the intraperitoneal recurrent tumors, a 2- to 3-hour IPHP was performed under hypothermic general anesthesia at about 32 degrees C, using a perfusate containing 10 micrograms/ml or 20 micrograms/ml of mitomycin C (MMC) warmed at the inflow temperature of 46.6 degrees C to 46.9 degrees C. Organizational closure through regenerative signaling as a possible basis for conscious awareness.

Along the lines of neuronal global workspace theories, the paper hypothesizes that active neural signal regeneration in recurrent, re-entrant circuits could constitute an organizational basis for states of conscious awareness. In this view, brains are self-production systems that regenerate their own informational, signal states ("neural autopoiesis"). States of awareness themselves depend critically on sustained regeneration of sets of neural signals in local and global circuits. The set of regenerated signals at each moment determines the specific contents of consciousness. Two hypotheses ar

Anesthesia-Informed Approaches to Insomnia: Shared Neural Circuits, Biomimetic Sleep, and Emerging Interventional Strategies.

BACKGROUND: Insomnia affects approximately 10-30% of adults, burdening quality of life, cognition, and health. Although cognitive behavioral therapy for insomnia (CBT-I) and approved pharmacotherapies remain standard care, limited accessibility, durability, and efficacy in refractory populations have motivated interest in mechanism-based alternatives, including approaches derived from anesthetic pharmacology. OBJECTIVE: This narrative review examines the convergent neural circuits underlying anesthesia-induced and physiological sleep, critically appraises evidence for anesthetic agents-dexmed

Anesthesia of captive African wild dogs (Lycaon pictus) using a medetomidine-ketamine-atropine combination.

Seven captive male African wild dogs (Lycaon pictus) weighing 25-32 kg each, were anesthetized by i.m. injection via hand syringe with a combination of 1.5 mg/kg ketamine, 40 microg/kg medetomidine, and 0.05 mg/kg atropine. Following endotracheal intubation, each animal was connected to a bain closed-circuit system that delivered 1.5% isoflurane and 2 L/min oxygen. Atipamezole (0.1 mg/kg i.v.; 0.1 mg/kg i.m.) was given at the end of each procedure (60 min following injection of medetomidine/ketamine/atropine). Time to sternal recumbency was 5-8 min. Times to standing after atipamezole administ

Reaction of sevoflurane and its degradation products with soda lime. Toxicity of the byproducts.

Sevoflurane previously has been reported to undergo extensive degradation in the presence of soda lime. To more completely characterize the extent and significnce of this reaction, we studied degradation of sevoflurane with and without soda lime, as well as the toxicity and mutagenicity of the degradation products. Two degradation products detected were CF2 = C(CF3)OCH2F (compound A) and CH3OCF2CH(CF3)OCH2F (compound B). During circulation of 1%, 2%, and 3% sevoflurance in a closed anesthesia circuit for 8 h, peak concentrations of compound A were 13.3 +/- 0.27, 30.2 +/- 0.10, and 42.1 +/- 1.0

Reducing kidney motion: optimizing anesthesia and combining respiratory support for retrograde intrarenal surgery: a pilot study.

BACKGROUND: One of the greatest challenges presented with RIRS is the potential for movement of the stone within the operative field associated with diaphragm and chest respiratory excursions due to mechanical ventilation. To overcome this challenge, we propose in this pilot study a new general anesthesia technique combining high frequency jet ventilation (HFJV) with small volume mechanical ventilation (SVMV). Data regarding safety, feasibility and surgeons' impression was assessed. METHODS: Patients undergoing RIRS for kidney stones from November 2017 to May 2018 were prospectively recruited

Developmentally unique cerebellar processing prioritizes self-over other-generated movements.

Animals must distinguish the sensory consequences of self-generated movements (reafference) from those of other-generated movements (exafference). Only self-generated movements entail the production of motor copies (i.e., corollary discharges), which are compared with reafference in the cerebellum to compute predictive or internal models of movement. Internal models emerge gradually over the first three postnatal weeks in rats through a process that is not yet fully understood. Previously, we demonstrated in postnatal day (P) P8 and P12 rats that precerebellar nuclei convey corollary discharge

Ultrasound-guided percutaneous treatment of hepatocellular carcinoma by radiofrequency hyperthermia with a 'cooled-tip needle'. A preliminary clinical experience.

OBJECTIVE: Radiofrequency hyperthermia using the newly-developed 'cooled-tip' needle has recently been proposed as a therapeutic modality for hepatocellular carcinoma (HCC). Herein we report our preliminary results on feasibility and effectiveness of the thermal ablation of mono- or pauci-focal hepatocellular carcinoma with the cooled-tip needle. MATERIALS AND METHODS: We treated 15 cirrhotic patients (mean age 68.8 years; 12 males; 14 HCV-positive; 13 in Child's Class A and 2 in Class B) with 20 hepatocellular carcinoma nodules (mean diameter 28.1 mm; range 10-43 mm; nine lesions with diamet

Airway warming devices in the context of single and multimodal strategies for intraoperative hypothermia: a network meta-analysis.

BACKGROUND: This network meta-analysis (NMA) assessed the relative efficacy of airway warming devices and combined warming strategies for preventing intraoperative hypothermia. METHODS: We searched MEDLINE, EMBASE, CENTRAL, and Google Scholar to identify randomized controlled trials (RCTs) published through November 2025 that compared two or more warming strategies, including airway warming devices, in patients under general anesthesia. The primary outcome was intraoperative core temperature at the end of surgery. To establish the rank order of the evaluated warming strategies, frequentist an

Continuous intratracheal gas suctioning combined with ventilator leak-compensation may enhance carbon dioxide removal efficiency: a proof-of-concept study using a porcine model.

BACKGROUND: Although tracheal gas insufflation (TGI) and aspiration of expiratory gas in the dead space (ASPIDS) have been shown to enhance CO₂ removal by washing out the anatomical dead space, clinical implementation has been limited due to complications including unintended airway pressure increases and mucosal damage, as well as their complexity. We developed a novel respiratory management system combining continuous intratracheal gas suctioning with pressure control ventilation, utilizing the leak compensation function of modern ventilators. The aim of this study was to document the prelim

Mechanism of double potentials recorded during sustained atrial flutter in the canine right atrial crush-injury model.

BACKGROUND: During atrial flutter, double potentials may be recorded at specific sites in the atria. It has been suggested that double potentials represent sequential activations at the center of the reentrant circuit. An alternative hypothesis is that double potentials represent electrical activity in an area of slow conduction. Understanding their mechanism is important because double potentials have been considered a possible indicator of target sites for catheter ablation. METHODS AND RESULTS: We systematically studied double potentials in our canine model of atrial flutter produced by ri

Marked protection by selective cerebral profound hypothermia after complete cerebral ischemia in primates.

Hypothermia has been demonstrated to protect the brain from ischemia or traumatic brain injury. Achieving profound hypothermia has relied on techniques requiring total body cooling, which may result in serious cardiovascular and pulmonary complications. A technique to selectively cool the brain could conceivably exert a marked protection on cerebral structures and provide a relatively bloodless operative surgical field without systemic complications. Accordingly, this approach was tried in 7 rhesus monkeys after induction of general anesthesia. The right internal carotid artery and both intern

Epileptiform activity and behavioral arrests in mice overexpressing the calcium channel subunit α2δ-1.

UNLABELLED: The alpha2delta-1 subunit (α2δ-1) of voltage-gated calcium channels is a receptor for astrocyte-secreted thrombospondins that promote developmental synaptogenesis. Alpha2delta-1 receptors are upregulated in models of injury-induced peripheral pain and epileptogenic neocortical trauma associated with an enhancement of excitatory synaptic connectivity. These results lead to the hypothesis that overexpression of α2δ-1 alone in neocortex of uninjured transgenic (TG) mice might result in increased excitatory connectivity and consequent cortical hyperexcitability and epileptiform activit

Postoperative cognitive dysfunction: spotlight on light, circadian rhythms, and sleep.

Postoperative cognitive dysfunction (POCD) is a neurological disorder characterized by the emergence of cognitive impairment after surgery. A growing body of literature suggests that the onset of POCD is closely tied to circadian rhythm disruption (CRD). Circadian rhythms are patterns of behavioral and physiological change that repeat themselves at approximately, but not exactly, every 24 h. They are entrained to the 24 h day by the daily light-dark cycle. Postoperative CRD affects cognitive function likely by disrupting sleep architecture, which in turn provokes a host of pathological process
