Hypnosis associated with local anesthesia versus local anesthesia alone during bone marrow biopsy: a randomized, controlled, multicenter study, QUALHYBOM.

PURPOSE: Bone marrow biopsy (BMB) is a common procedure for the diagnosis, staging, and evaluation of several hematological diseases. BMB often causes anxiety and pain even when local anesthesia (LA), premedication, or an equimolar mixture of oxygen and nitrous oxide (EMONO) are used. Hypnosis has emerged as a promising non-pharmacological method to reduce anxiety and pain during invasive procedures. QUALHYBOM, a prospective, multicenter, randomized, open-label study was conducted to assess whether adding hypnosis to LA (LA + H) could improve patient comfort during BMB compared to LA alone. M

A randomized controlled trial of minor hand surgeries comparing wide awake local anesthesia no tourniquet and local anesthesia with tourniquet.

INTRODUCTION: Epinephrine and sodium bicarbonate (NaHCO3) used in wide-awake local anesthesia no tourniquet (WALANT) affect many areas such as hemostasis, injection pain, anesthetic effect and others. However, few clinical trials have focused on injection pain and the duration of anesthetic effect, and no prospective studies have reported the benefits of WALANT post operation. This study compared WALANT with conventional local anesthesia with tourniquet in minor hand surgeries, and aimed to answer following questions: (1) Does WALANT have enough benefits for injection pain and duration of anes

Clinical efficacy of general anesthesia versus local anesthesia for percutaneous transforaminal endoscopic discectomy.

OBJECTIVE: Local anesthesia (LA) is recommended for percutaneous transforaminal endoscopic discectomy(PTED), but satisfactory pain management is not mostly achieved. The goal of this study was to examine the clinical efficacy of PTED for lumbar disc herniation when performed under local anaesthetic vs. general anesthesia (GA). METHODS: From August 2018 to August 2020, the clinical data of 108 patients treated with PTED were retrospectively evaluated and separated into two groups: LA and GA based on the anesthesia method. General information and clinical outcomes of patients were included. Vis

Stepwise Rostrocaudal Brainstem Anesthesia as a Complication of Local Anesthesia: A Case Report.

Iatrogenic cranial nerve palsies can rarely complicate neurosurgical, oral maxillofacial, and otolaryngological procedures. Among the most serious complications of cranial nerve palsy is upper airway obstruction, which is life threatening. We present a case of multiple cranial nerve palsies evolving rapidly in a rostrocaudal stepwise fashion after infiltration of lidocaine to repair a cerebrospinal fluid leak in a patient postoccipital craniectomy. This led to hypoxic respiratory failure requiring mechanical ventilation before resolving spontaneously. This is the first known case of accidental

[Neuraxial anesthesia after local anesthesia for management of percutaneous vertebroplasty complication during vertebroplasty].

Percutaneous vertebroplasty is a relatively safe, simple and commonly performed interventional procedure for the management of vertebral compression fractures. However, serious complications are rarely reported in the procedure. Those are pulmonary embolism, severe infection, paraplegia and an occurrence of a new fracture in an adjacent vertebra after vertebroplasty. Acute complications are generally associated with the procedure. We present the case of neuraxial anesthesia, developed after local anesthesia with 8mL of 2% prilocaine, in a 68-year-old woman who underwent percutaneous vertebropl

[Hypersensitivity to tumescent local anesthesia in dermatosurgical procedures].

Tumescent local anesthesia (TLA) offers many advantages in major and minor dermatosurgical procedures, especially in elderly and multimorbid patients in whom general anesthesia is sometimes associated with major risks. LA intolerances are rare. In our experience, TLA is a safe method despite the mixture of different LA. Monitoring of the patient during TLA is necessary. Clarification of a history of LA intolerance prior to surgical treatment is useful; however, a negative test does not reliably rule out an allergic reaction.

Submental liposuction under local anesthesia - About 695 patients.

UNLABELLED: There is more and more non-invasive treatment proposed in face rejuvenation. However, there are only a few treatments that decrease volume of the tissue efficiently. Submental liposuction is not classically considered as a non-invasive treatment. MATERIAL AND METHOD: We reviewed from October 2020 until August 2022; 695 patients were operated by the first author under local anesthesia. RESULTS: Complications exist but are minimal. The most important is irregularity that can be managed by massage and ultrasound treatment. DISCUSSION: With our experience of over 600 cases, we belie

Single-Port Thoracoscopic Pericardial Window Under Local Anesthesia.

There are numerous surgical approaches for the treatment of pericardial effusions but no clear consensus of best management. We present a 44-year-old woman with metastatic breast cancer presenting with a new 2-cm pericardial effusion on ultrasound. In light of the patient's palliative condition and the urgent need for chemotherapy, careful consideration was made for her surgical drainage of the pericardial effusion. Because of the patient's medical comorbidities, a general anesthetic was deemed not to be in the patient's best interest. Furthermore, the invasive subxiphoid or thoracotomy approa

Tetrodotoxin-bupivacaine-epinephrine combinations for prolonged local anesthesia.

Currently available local anesthetics have analgesic durations in humans generally less than 12 hours. Prolonged-duration local anesthetics will be useful for postoperative analgesia. Previous studies showed that in rats, combinations of tetrodotoxin (TTX) with bupivacaine had supra-additive effects on sciatic block durations. In those studies, epinephrine combined with TTX prolonged blocks more than 10-fold, while reducing systemic toxicity. TTX, formulated as Tectin, is in phase III clinical trials as an injectable systemic analgesic for chronic cancer pain. Here, we examine dose-duration re

Radio-guided minimally invasive parathyroidectomy under local anesthesia.

BACKGROUND: Despite the high success rate of the complete bilateral neck exploration to treat primary hyperparathyroidism, less invasive alternatives have been emerging. In an attempt to reduce operative time and decrease perioperative morbidity, we reported our experience with radio-guided minimally invasive parathyroidectomy (RMIP) under local anesthesia. STUDY DESIGN: A retrospective chart review was carried out to study 55 consecutive patients, in an adult tertiary care hospital (Montreal General Hospital), who underwent RMIP under local anesthesia over a 30-month period. Charts were revi

Tetrodotoxin for prolonged local anesthesia with minimal myotoxicity.

Conventional local anesthetics such as bupivacaine cause considerable myotoxicity and neurotoxicity, whereas tetrodotoxin (TTX) does not. Tetrodotoxin combined with bupivacaine or vasoconstrictors produces long-duration nerve blockade. To assess whether these prolonged blocks can be produced without increased myotoxicity, Sprague-Dawley rats were injected with bupivacaine, TTX, and both, or TTX plus epinephrine. Median durations of thermal nociceptive blockade were, respectively, 188, 401, 882, and 972 min. On dissection 4 days later, all tissues appeared macroscopically pristine. Muscle injur

A simplified endoscopic third ventriculostomy under local anesthesia.

The aim of this study is the analysis of our experience with awake endoscopic third ventriculostomy (ETVS) in hydrocephalic patients. From September 1994 to December 2001, 24 neuroendoscopic procedures were performed under local anesthesia. Local infiltration was administered using a bupivacaine and lidocaine mixture. Analgesics were titrated to the effect. A free-hand technique with a flexible endoscope was adopted in 24 patients with primitive and secondary (neoplastic) hydrocephalus. ETVS was performed successfully in all cases. No procedure needed to be discontinued due to seizures, bleedi

Avoiding complications in local anesthesia induction: anatomical considerations.

BACKGROUND: Dentists administer thousands of local anesthetic injections every day with few reports of serious complications. However, misjudging the anatomy involved during local anesthetic administration can result not only in inadequate or incomplete anesthesia, but in other complications such as paresthesia, bleeding or hematoma formation, or in serious systemic complications. OVERVIEW: The authors discuss anatomical considerations that dentists should keep in mind when administering local anesthetic injections. In particular, they discuss ways in which dentists can minimize the risk of n

Biopsy of anterior mediastinal masses under local anesthesia.

Treatment of most primary mediastinal tumors is based on positive histological diagnosis; we describe a variation of anterior mediastinoscopy under local anesthesia. After infiltration with local anesthetic, a transverse skin incision is made in the second, third, or fourth interspace. The endothoracic fascia is incised vertically adjacent to the periosteum and a mediastinoscope is inserted in the mediastinum. Between 1995 and 2001, we have employed this technique in 46 patients with anterior mediastinal tumors. Histological diagnosis was obtained in all patients. Pneumothorax (2 patients) was
