Enhanced recovery after cesarean (ERAC) delivery is an evidence-based, multi-disciplinary approach throughout pre-, intra-, post-operative period. The ultimate goal of ERAC is to enhance recovery and improve the maternal and neonatal outcomes. This review highlights the role of anesthesiologist in ERAC protocols. This review provided a general introduction of ERAC including the purposes and the essential elements of ERAC protocols. The tool used for evaluating the quality of ERAC (ObsQoR-11) was discussed. The role of anesthesiologist in ERAC should cover the areas including management of peri

Prosthesis-patient Mismatch (PPM) is not uncommon with an incidence reported up to 70% after aortic valve (AV) replacement. Severe forms of PPM are less common (up to 20%); PPM can lead to increased short- and long-term morbidity and mortality. It is important to discriminate PPM from other forms of prosthetic valve dysfunction. Sometimes, prosthetic valve degenerative disease may coexist with PPM. Echocardiography plays an important role in the prevention and diagnosis of PPM. Preemptive strategies to prevent PPM include insertion of newer generation prosthetic valves with better hemodynamic

BACKGROUND AND OBJECTIVES: The new cardiopulmonary resuscitation (CPR) guidelines emphasize the importance of high-quality chest compressions and modify some routines. The objective of this report was to review the main changes in resuscitation practiced by anesthesiologists.
CONTENTS: The emphasis on high-quality chest compressions with adequate rate and depth allowing full recoil of the chest and with minimal interruptions is highlighted in this update. One should not take more than ten seconds checking the pulse before starting CPR. The universal relationship of 30:2 is maintained, modifyi

BACKGROUND: The prevalence of cigarette smoking in China is high. Surgery provides an excellent opportunity for patients to quit smoking, and anesthesiologists can play an important role in tobacco control. However, little is known about the practices, knowledge, and attitudes of Chinese anesthesiologists regarding perioperative tobacco interventions.
METHODS: Chinese anesthesiologists were surveyed at a national meeting in 2009, with written questionnaires distributed to 800 practicing anesthesiologists.
RESULTS: The survey response rate was 60.3%, and 10% of respondents themselves smoked c

BACKGROUND: Although powdered latex surgical gloves are predominantly used in Japanese healthcare facilities, the prevalence of latex sensitization among anesthesiologists has not been investigated.
METHODS: The authors surveyed 60 anesthesiologists of 16 facilities with questionnaires and measurements of specific IgE antibodies against latex and other aeroallergens by AlaSTAT microplate immunoassay. Sensitization was defined as positive if the specific IgE concentration was not less than 0.70 IU x ml(-1).
RESULTS: With surgical gloves 63.3% of anesthesiologists used powdered latex gloves, c

IMPORTANCE: Attending anesthesiologists are not subject to work hour restrictions. Fatigue from long shifts may plausibly contribute to patient harm.
OBJECTIVE: To examine the association between anesthesiologist extended work periods and patient outcomes.
DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used data from the Multicenter Perioperative Outcomes Group registry from January 1, 2010, to August 30, 2020, representing all surgical procedures in patients 18 years or older from more than 50 hospitals across 18 US states. Data analyses were conducted from July 1 through Nov

Innervation of the shoulder joint is complex and remains poorly understood among regional anesthesiologists and chronic pain specialists. Current literature supports the important contribution of the axillary nerve to the total innervation of the shoulder, as well as its blockade for perioperative pain and denervation for chronic shoulder pain. However, a description of the entire course of the axillary nerve, the corresponding optimal targets, and the sonoanatomy pertinent to pain intervention is lacking. This educational article discusses in detail the functional anatomy and sonographic iden

BACKGROUND: The surgical establishment of a secure airway in a cannot intubate-cannot oxygenate situation represents a feared and multidimensionally challenging situation in anesthesiology and emergency medicine teams. Numerous factors can influence the performance of a cricothyroidotomy. In addition to experience, knowledge and practical skills, the materials used and the mindset of the personnel also appear to play a significant role.
AIM OF THE STUDY: This study aimed to provide a comprehensive picture of experiences, attitudes and knowledge on surgical airway management of medical personn

Hemodynamic response to laryngoscopy and intubation is usually transient, but it may be more pronounced and unpredictable in certain group of patients. Bradycardia and asystole during laryngoscopy is usually a rare manifestation compared to hypertension and tachycardia. Anesthesiologists should be more vigilant and take special precautions to avoid such life-threatening complications during laryngoscopy. Here, we report recurrent asystole on multiple occasions during laryngoscopy in a patient with obstructive jaundice.

Multivariable regression analysis is a powerful statistical tool in biomedical research with numerous applications. While linear regression can be used to model the expected value (ie, mean) of a continuous outcome given the covariates in the model, quantile regression can be used to compare the entire distribution of a continuous response or a specific quantile of the response between groups. The advantage of the quantile regression methodology is that it allows for understanding relationships between variables outside of the conditional mean of the response; it is useful for understanding an

PURPOSE: Ebola virus disease (EVD) is a viral hemorrhagic fever that is highly transmissible and all too often rapidly fatal. Recent outbreaks in West Africa reveal that this infection has the potential to be transmitted worldwide. Anesthesiologists and intensivists, due to their training in the management of the critically ill, may be called upon to assist in the management of these patients. The focus of this brief review is on the epidemiology, pathogenesis, and management of patients with EVD.
SOURCE: Review of the current literature.
PRINCIPAL FINDINGS: Ebola virus disease causes severe

Trauma-informed care continues to follow the 4R principles (realize the widespread nature of trauma, recognize what unresolved trauma looks like, respond by changing practices and protocols, and resist retraumatization) but has expanded to include 6 core guiding principles that include safety, trust, peer support, mutuality, empowerment, and the understanding of an individual's cultural context. Additionally, it is now recommended that trauma-informed practices be offered as universal precautions to all individuals, regardless of trauma history, focusing on trauma prevention. Several situations in anesthesia practice exist where medications are mixed; for instance, local anesthetics-additive mixtures to increase block duration, propofol-lidocaine mixture to reduce pain on injection, the sequential injection of different substances into a peridural catheter, or the mix of different intravenous sedatives and analgesics before application. This article recommends a cautious, evidence-based approach to medication mixing in anesthesia practice, with careful consideration of available alternatives. Future research should focus on establishing compatibility standards, imp

INTRODUCTION: PHACES syndrome is a neurocutaneous syndrome that consists of posterior fossa brain malformation, hemangiomas of the face, arterial cerebrovascular malformation, cardiovascular anomalies (Coarctation of the aorta), eye anomaly, and sternal defect or supraumbilical raphe. Most of these manifestations would interfere with anesthesia and hence need major considerations.
CASE PRESENTATION: A 2.5-month-old female weighing 2100 gr was a candidate for laser therapy due to retinopathy of prematurity. She was diagnosed with PHACES syndrome. Her anesthesia was induced and maintained with

Cerebral palsy (CP) refers to a spectrum of nonprogressive neurological disorders with disturbances in posture and movement, resulting from perinatal intrauterine insult to developing infant brain. Many conditions associated with CP require surgery. Such cases pose important gastrointestinal, respiratory, and other perioperative considerations. Anesthetic management in these cases is delicate. Intraoperative complications including hypovolemia, hypothermia, muscle spasms, seizures, and delayed recovery might complicate the anesthetic management. A thorough preanesthetic evaluation allows for a

Abusive head trauma (AHT) is the most common cause of severe traumatic brain injury (TBI) in infants and the leading cause of child abuse-related deaths. For reasons that remain unclear, mortality rates after moderate AHT rival those of severe nonintentional TBI. The vulnerability of the developing brain to injury may be partially responsible for the poor outcomes observed after AHT. AHT is mechanistically more complex than nonintentional TBI. The acute-on-chronic nature of the trauma along with synergistic injury mechanisms that include rapid rotation of the brain, diffuse axonal injury, blun
