The outcome of ruptured abdominal aortic aneurysm repair was reviewed in 83 consecutive patients with special emphasis on the influence of subsequent laparotomy. The overall 30-day mortality was 47%. Causes of death were exsanguination in six, cardiac failure in 15, uncontrolled hypotension in six, multiple organ failure (MOF) in nine, adult respiratory distress syndrome in one and sepsis in two patients. Thirty-three relaparotomies were performed in 21 patients after a mean interval of 10 days. Suspected intraabdominal haemorrhage was the indication in 15 and sepsis in 18 cases. The preoperat

Subdural hematoma (SDH) was observed in 15 of 484 cases of aneurysmal subarachnoid hemorrhage (SAH). There were four males and 11 females, with ages ranging from 39 to 75 years. The clinical grades (Hunt and Hess) on admission were 11 in three cases, III in two, IV in four, and V in six. The ruptured aneurysms were located in the middle cerebral artery (MCA) in six cases, anterior communicating artery in three, internal carotid artery in two, and distal anterior cerebral artery (ACA) in two, with two cases unconfirmed. A high proportion of aneurysms occurred in the MCA and distal ACA. Aneurysm

An X-ray film of the thorax in a 76-year-old man with recurrent dyspnoea and haemoptysis showed a widened upper mediastinum. Computed tomography demonstrated an aneurysm of an aberrant right subclavian artery as the cause of the symptoms and the mediastinal widening. The patient died 5 days after hospitalization and before a planned operation, another episode of severe haemoptysis having been followed by haematemesis. Autopsy revealed a 13 cm-long aneurysm which had ruptured into the right upper lobe of the lung with bleeding into the bronchial system. No further cases of an aneurysm of an abe

Isolated aneurysms of the iliac arteries are rare and can be difficult to diagnose. The diagnosis is frequently delayed. One patient with a ruptured aneurysm of the left internal iliac artery is presented, and the computed tomography findings are compared to those of a case of a ruptured aortic aneurysm. The hemorrhages had much in common, and we want to stress the importance of including the iliac arteries when no cause of a retroperitoneal hemorrhage is found higher up.

A case of arteriosclerotic aneurysm of the splenic artery with rupture into the transverse colon resulting in secondary perforation of the cecum is presented. Symptomatology, diagnosis and treatment of aneurysms of the splenic artery are briefly illustrated. Aneurysm of the splenic artery must be suspected by the presence of uncharacteristic upper abdominal pain and the annular calcification in the left upper quadrant on roentgenogram. The diagnosis is verified by arteriography. Operation of all symptomatic splenic artery aneurysms is recommended. Asymptomatic aneurysms measuring more than 3 c

INTRODUCTION: Coverage of one or both renal arteries may be required to facilitate endovascular aneurysm repair (EVAR) in patients who are not candidates for open surgery in ruptured abdominal aortic aneurysms (rAAAs). We sought to understand the consequences of renal coverage during these emergent procedures.
METHODS: Using the VQI data set from 2013 to 2018, we selected patients who had undergone EVAR for rAAA. Patients were distinguished by whether they had none, unilateral, or bilateral renal artery coverage. Patients were excluded if they were previously on dialysis or had an interventio

BACKGROUND: Although size is one of the strongest predictors, small aneurysms often rupture. We compared the characteristics of small and large ruptured intracranial aneurysms (RIAs) according to their location to find the factors associated with small RIAs in each location.
METHODS: Patients with subarachnoid hemorrhage due to saccular RIAs were consecutively enrolled. The sizes were dichotomized as small (<5 mm) or large, and the location was classified as paraclinoid-distal internal carotid artery, sidewalls of anterior or middle cerebral artery (MCA; sidewall), MCA-bifurcation, anterior o

We reported on a case of 80-year-old woman who suffered from severe acute respiratory failure. A chest computed tomography (CT) revealed arch aneurysm and innominate artery pseudoaneurysm, which severely compressed main bronchus and trachea. After tracheal intubation in the emergency room, respiratory status improved rapidly. We immediately performed total arch replacement using deep hypothermia, circulatory arrest and the arch first technique. The postoperative course was uneventful, and stenosis of trachea resolved. Arch aneurysm associated with acute trachea occlusion is very rare and emplo

Treatment paradigms for elective repair of asymptomatic abdominal aortic aneurysms (AAA) have evolved during the past 2 decades, with endovascular aneurysm repair as the preferred treatment modality. The patient care strategy for emergent treatment for ruptured AAA is not as straightforward, due in part to surgeon expertise and stent-graft availability at the institution. Although most reports have extrapolated elective endovascular aneurysm repair feasibility data to the ruptured AAA patient and the aneurysm anatomy, these expectations should be interpreted with caution. In the absence of lev

Late aortic rupture following successful endovascular repair of abdominal aortic aneurysm still does occur. It represents the ultimate failure of endovascular aortic repair of abdominal aortic aneurysm (EVAR) and subjects patients to equivalent risk of death as de novo rupture. Unfortunately, it is difficult to identify patients at risk for post-EVAR rupture as many present with aortic rupture in the absence of any endograft-related complications. Continued surveillance and timely intervention are of paramount importance to assure rupture-free survival, the ultimate goal of any aneurysm treatm

Renal artery aneurysms constitute one per cent of all aneurysms. To our knowledge, this is the first reported case of a calcified saccular arteriosclerotic renal artery aneurysm rupturing. Because the patient was in hemorrhagic shock, nephrectomy was performed. The postoperative course was complicated by acute renal failure, pneumonia, and small-bowel fistula, which responded to hemodialysis, antibiotics, and central hyperalimentation, respectively. This case and review of the literature emphasizes the need for elective revascularization of renal artery aneurysms by primary repair or bypass gr

This manuscript presents a rare case of ruptured sinus of Valsalva aneurysm, which broke into the peri-membranous part of interventricular septum, forming a contained hematoma and protruded into the left ventricular outflow tract with fungal infective endocarditis. Echocardiography and computed tomography played a crucial role in the diagnosis of this complex case.

BACKGROUND: Subclavian steal occurs due to stenosis or occlusion of the subclavian artery or innominate artery proximal to the origin of the vertebral artery. Often asymptomatic, the condition may be unmasked due to symptoms of vertebrobasilar insufficiency triggered by strenuous physical exercise involving the affected upper limb. The association of vertebrobasilar junction (VBJ) aneurysms with subclavian steal syndrome has been rarely reported. Hereby, we present a case of VBJ aneurysm associated with subclavian steal treated successfully with endovascular coiling.

A patient presented to the emergency department with undifferentiated shock 4 days after discharge from a hospitalization for a lower gastrointestinal bleed. The patient fulfilled 4/4 of the Systemic Inflammatory Response Syndrome criteria and 3/3 of the quick Sequential Organ Failure Assessment criteria on presentation to the emergency department, notably, without a localized source of infection and no localizing symptoms. After admission, the patient's hemoglobin was found to have dropped more than expected after intravenous (IV) fluid administration, suggesting a potential alternative or co

A ruptured thoracoabdominal aortic aneurysm is an urgent situation requiring emergency surgery during which it is necessary to decrease the scope of the intervention in a patient at an extremely high surgical risk. A combination of surgical and endovascular techniques makes it possible to minimize surgical injury, blood loss, to shorten the duration of internal organs ischaemia, to reduce the risk of paraplegia. Presented herein are the results of emergency hybrid intervention for a ruptured thoracoabdominal aortic aneurysm, accompanied and followed by description of the technical aspects of p

Massive haemoptysis is associated with high mortality. Among various diseases presenting with massive haemoptysis, a ruptured bronchial artery aneurysm (BAA) is a rare entity. We report the case of a ruptured BAA as small as 3 mm in diameter associated with massive haemoptysis in a previously healthy 77-year-old woman. She had suddenly developed massive haemoptysis and was intubated and admitted to the intensive care unit. A CT scan and repeated bronchoscopy could not reveal the cause. Because haemorrhage continued, she underwent bronchial arteriography (BAG) twice. We finally detected a BAA w
