Preoperative emobilisation of juvenile nasopharyngeal angiofibroma.

INTRODUCTION: Juvenile nasopharyngeal angiofibroma(JNA) are highly vascular benign tumours originating in the sphenopalatine fossa and may extend to the pterygopalatine fossa, paranasal sinuses, and nasal cavity. The management of JNA has evolved greatly with development of endoscopy. Treatment modality has changed from open approach to endoscopic approach due to various advantages offered by the endoscopic approach. Bleeding during the surgery can compromise the exposure and surgical excision of JNA endoscopically. There have been many techniques to decrease intraoperative bleeding including

[Juvenile nasal angiofibroma significant locoregional involvement].

Juvenile nasal angiofibroma is a benign tumor characterized by abundant vascularization and a tendency to hemorrhage. Despite its benign histology, it can present locally expensive growth that usually presents bone remodeling and the mass effect of adjacent stuctures. We present the case of a 15-year-old patient who came to the emergency department of our center for recurrent epistaxis. After physical examination, it was decided to carry out imaging tests (CT an MRI) that reveal the presence of a large hypervascular mass in the right nostril and maxillary sinus. The patient undergoes surgery a

Frontoethmoidal Extranasopharyngeal Angiofibroma With Orbital Pyocele.

Significance StatementExtranasopharyngeal angiofibromas (ENA) are rare vascular tumors that do not conform to the clinical characteristics of typical nasopharyngeal angiofibromas. We present the management of an angiofibroma in a rare site, within the frontal sinus with a concomitant orbital pyocele, which was completely excised via an endoscopic approach. ENAs should be considered as a differential diagnosis in patients with sinonasal mass and epistaxis. Awareness of this rare entity will avoid radical surgery thus decreasing postoperative morbidity.

[Juvenile nasopharyngeal angiofibroma with orbital extension].

CLINICAL CASE: The case is presented of a 21 year-old male with a history of left proptosis and diplopia of two weeks of onset. The MRI showed an ethmoid-orbital vascular lesion with anterior skull base invasion and orbital extension. Biopsy of the ethmoid confirmed fibrovascular tissue, which supported the diagnosis of angiofibroma. DISCUSSION: It is a benign neoplasm with local characteristics of malignancy due to its ability to invade adjacent areas. In this case, the debut presented with manifestations of orbital extension. A broad and multidisciplinary approach is needed in order to impr

Extranasopharyngeal angiofibroma: clinical and radiological presentation.

Nasopharyngeal angiofibroma (NA) is a rare, vascular tumor affecting adolescent males. Due to aggressive local growth, skull base location and risk of profound hemorrhage, NA is a challenge for surgeons. Angiofibromas have been sporadically described in extanasopharyngeal locations. We review ten cases of extranasopharyngeal angiofibroma (ENA) and discuss the incidence, clinical presentation and management of this pathology. The group consisted of 4 males and 5 females aged 8-49. There were 7 patients with nasal angiofibroma, 1 patient with laryngeal angiofibroma, 1 patient with oral angiofibr

Extragenital subcutaneous cellular angiofibroma. Case report.

Cellular angiofibroma (CAF) is a rare distinctive mesenchymal neoplasm that occurs almost exclusively in the genital area. We report the case of a 38-year-old woman who presented with an asymptomatic subcutaneous mass, 3.5 cm in diameter, located in the left hypochondrium, which had progressively enlarged during the previous 6 months. The lesion was completely excised. No recurrence was observed 3 months after the excision. A review of the literature--and including the present report--revealed five cases (three men and two women) with location outside the urogenital tract. Mean age was 57 (ran

Surgical approaches to juvenile nasopharyngeal angiofibroma.

INTRODUCTION: Juvenile nasopharyngeal angiofibromas are highly vascular, non-encapsulated tumours affecting predominantly young males. These lesions are benign histologically but they may become life-threatening with excessive bleeding or intracranial extension. MATERIAL AND METHODS: The surgical approaches to 22 male patients with nasopharyngeal angiofibromas are reviewed. A modification of midfacial degloving performed without rhinoplasty incisions and lateral osteotomies is described. RESULTS: The patients' ages ranged between 9 and 26 years (mean 14.9). Three stage I tumours, 8 stage II

[Nasopharyngeal angiofibroma. Our experience (1990-2001)].

We present our experience in the treatment of nasopharyngeal angiofibromas, tumours not very common, with a benign histology but with the risk of severe epistaxis or possible involvement of vital structures. In a period of 12 years (1990-2001) we have treated 6 males with this pathology, operating 5 of them. We describe the clinical manifestations, the tumour stages following the Fisch classification, the diagnostic and surgical techniques used, the evolution of the patients and their postoperatory. We review also the literature about this subject.

Middle turbinate angiofibroma in an adolescent boy.

Bleeding nasal mass in adolescent boys has customarily been attributed to Juvenile nasopharyngeal angiofibroma. However, little is known regarding the extranasopharyngeal origin of angiofibroma, as highlighted in this case report of a 15-year-old boy who presented with recurrent epistaxis and nasal obstruction. On constructing a working diagnosis of nasal haemangioma, the patient was taken up for endoscopic excision under general anaesthesia. Intraoperative endoscopic findings and histopathological examination revealed the diagnosis of middle turbinate angiofibroma, which is an extremely rare

Nasal Angiomyolipoma (AML) Mimicking Juvenile Nasopharyngeal Angiofibroma.

INTRODUCTION: Angiomyolipoma (AML), a benign mesenchymal tumor that commonly arises from the kidney, may be associated with tuberous sclerosis complex and perivascular epithelioid cell tumors (PEComas). Nasal angiomyolipoma is very rare and usually occurs in elderly individuals with epistaxis and nasal obstruction. CASE REPORT: We report a rare case of nasal angiomyolipoma in a young male. To the best of our knowledge, this is the first documented case of angiomyolipoma originating from the posterior end of the inferior turbinate, clinically mimicking juvenile nasopharyngeal angiofibroma (JNA

Transnasal microscopic approach for juvenile nasopharyngeal angiofibroma.

INTRODUCTION: Juvenile nasopharyngeal angiofibroma (JNA) is a rare, benign, vascular tumor originating in the nasopharynx. The treatment of choice for JNA is surgical excision. In recent years, surgical management has been greatly influenced by the use of the transnasal endoscopic technique. THE AIM: The aim of the study was to present our experience with the transnasal microscopic removal of JNA. MATERIAL AND METHODS: Ten patients with JNA aged 12-17 underwent diagnostics imaging and transnasal microscopic tumor excision. Medical records of patients were retrospectively reviewed. The main o

(68)Ga-DOTATATE PET in juvenile angiofibroma.

BACKGROUND: As somatostatin receptors (SSTRs) may be overexpressed in rapidly growing vessels, the aim of this study was the analysis of in vivo and in vitro SSTR2A expression in juvenile angiofibroma (JA). MATERIAL & METHODS: A group of six male adolescents with a diagnosis of primary, recurrent/residual JA was enrolled in the study. All patients underwent (68)Ga-DOTATATE PET/computed tomography (CT) followed by immunohistochemical staining for SSTR expression. RESULTS: (68)Ga-DOTATATE PET/CT showed accumulation in areas matching the pathologic tissue in the nasopharynx of all patients stud

Middle turbinate angiofibroma in an elderly woman.

BACKGROUND: Angiofibromas are histologically benign vascular tumors, originating from the nasopharynx, near by the area of sphenopalatine foramen. These neoplasms occur typically in male adolescents. Reports of primary extranasopharyngeal angiofibromas have appeared sporadically in the literature in English. We present the first case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma. CASE REPORT: A 63-year-old female presented with left-sided nasal obstruction and epistaxis. Endoscopic evaluation revealed a polypoid mass arising from the anteroinferio

[Juvenile nasopharyngeal angiofibroma: stage and surgical approach].

Juvenile nasopharyngeal angiofibromas (JNAs) are benign tumors that occasionally invade the pterygopalatine fossa, infratemporal fossa, or middle cranial fossa. Several surgical approaches have been used based on the location of the tumor, including transpalatal, transmaxillary, and lateral rhinotomy, midfacial degloving, and Le Fort type I osteotomy. We reviewed 4 cases of JNA that had been treated by resection in the Department of Otolaryngology of Kagoshima City Hospital and 31 cases of JNA reported in the Japanese literature between 1990 and 2003. We analyzed the outcome of the surgical tr

Giant cell angiofibroma of the nasolacrimal duct.

PURPOSE: To describe clinical and histologic features of the first case, to our knowledge, of giant cell angiofibroma located in the nasolacrimal duct region in a 28-year-old woman. METHODS: Interventional case report. A left nasolacrimal duct tumor was excised en bloc by lateral rhinotomy. Histopathologic examination was performed with the use of light microscopy. Immunohistochemical staining included S-100 protein, muscle-specific actin, desmin, myoglobin, vimentin, and CD34. RESULTS: The lesion was characterized by haphazardly arranged oval to spindled cells, a myxoid and collagenous stro

Solitary periungual angiofibroma. An unusual case report.

Solitary periungual angiofibromas can often be challenging in the diagnosis and treatment of such common lesions. Because of the various histologic presentations of fibrous skin tumor, a complete differential diagnosis would include hemangioma, seborrheic keratosis, fibroma, and digital fibrokeratoma. Treatment by total sharp excision can often be accomplished, depending on the morphological appearance of the lesion. An incisional biopsy of a more expansile lesion may be warranted.

Juvenile Nasopharyngeal Angiofibroma: An Aberrant Case Report.

Juvenile nasopharyngeal angiofibroma (JNA) is a very uncommon condition. We are presenting a case of a teenage boy with painless nasal blockage and fullness of bilateral ears for two to three months. On nasal endoscopy, a firm proliferative mass obstructing the right nasal cavity was noted. It was bleeding on touch. On contrast-enhanced computed tomography of the paranasal sinuses (CECT PNS), a diagnosis of the JNA was made based on the evidence of bony erosion and intense post-contrast enhancement. He was pre-operatively locally embolized followed by surgical resection. On follow-up, the pati
