A histopathologic and immunohistochemical study of small nodules of renal angiomyolipoma: a comparison of small nodules with angiomyolipoma.

Small mesenchymal nodules (SNs) are observed in some cases of renal angiomyolipoma (AML), with or without tuberous sclerosis. They are composed of blood vessels and/or nonvascular smooth muscle cells (SMCs) and/or fat cells. We examined 20 cases of AML, performed detailed histopathologic and immunohistochemical studies of SNs, verified the histologic relationship between SNs and AMLs, and compared the SNs of the tuberous/nontuberous sclerosis groups. Seventy-seven SNs were observed in five cases of AML. The SNs were 0.11 mm to 20.0 mm in diameter. The location of small-sized SNs in the kidney

Crystalloids in angiomyolipoma. 1. A previously unnoticed phenomenon of renal angiomyolipoma occurring at a high frequency.

We present a description of unique crystalloids in renal angiomyolipoma that have not previously been reported. The crystalloids cannot be identified by hematoxylin-and-eosin staining. Detailed observation after diastase treatment followed by PAS staining revealed needle- and rod-like crystalloids, which were clearly seen even by light microscopy, in 11 of 17 patients. Their appearance was characterized by the following phenomena: (a) They appeared mainly in large epithelioid smooth-muscle cells; (b) they appeared at a relatively high frequency at sites where smooth-muscle cells showed diffuse

From angiomyolipoma to malignant epithelioid angiomyolipoma of the kidney, a case report with a history of eight years.

Epithelioid angiomyolipoma (EAML) is a rare subtype of angiomyolipoma (AML) with potential has malign tendency. UP to now, the biological association between EMAL and typical AML is not well known. In the present study, we reported a case of EMAL arose in AML seven years post AML operation. A 34-year-old man with bilateral kidney AML was operated with an open cytoreductive surgery of both sides. After seven years, the left nephrectomy was performed due to a new neoplasm was found in the left kidney. The pathology diagnosis and inmmunohistochemical staining suggested the new neoplasm was EAML.

Renal epithelioid aggressive angiomyolipoma in a patient with tuberous sclerosis and past contralateral benign renal angiomyolipoma.

Twenty-five percent of all aggressive angiomyolipomas (AMLs) are related to tuberous sclerosis. Our patient was a 42-year-old man with tuberous sclerosis and a medical history of a benign AML in the left kidney. Presently, an epithelioid AML was detected in his right kidney and, despite our conducting extensive surgery, a large pulmonary metastasis with mediastinal lymph node involvement followed 20 months later. Close follow-up of the patient is advised when epithelioid cells are observed in angiomyolipoma. Immunohistochemistry for mismatch repair proteins showed that PMS2, MSH2, and MSH6 wer

Pigmented Epithelioid Angiomyolipoma of the Kidney.

Extensive melanin pigment in an epithelioid angiomyolipoma, a potentially malignant and locally aggressive renal tumor, has been rarely reported. A 53-year-old, asymptomatic man with no significant past medical history underwent partial nephrectomy for a right kidney mass discovered on CT scan. Grossly, the mass was 4 cm, well-circumscribed, dark, brown-black. Microscopically, nests of large, clear-to-eosinophilic cells with mildly atypical, vesicular nuclei and prominent nucleoli were separated by striking vascular network. Abundant, brown-black, coarsely granular pigment was noted. The tumor

Epithelioid angiomyolipoma presenting as an urothelial.

OBJECTIVE: Renal angiomyolipoma is a mesenchymal neoplasm composed of dysmorphic blood vessels, smooth muscle, and adipose tissue. Epithelioidangiomyolipoma is an uncommon variant considered potentially malignant. We report a case of renal epithelioid angiomyolipoma suspicious for urothelialcarcinoma of renal pelvis. METHODS: A 50-year-old woman with a 3-day history of macroscopic hematuria and CT-scan showing a tumor arising from the left renal pelvis suspicious for urothelialcarcinoma. Nephrectomy was analysed in multiple slides of hematoxilin-eosin and immunohistochemistry. RESULTS: Left

Imaging features of hepatic angiomyolipomas.

We review the imaging appearances of hepatic angiomyolipomas in patients with and without tuberous sclerosis. Sporadic hepatic angiomyolipomas have a varied appearance because of the inconstant proportion of fat, making confident imaging diagnosis difficult and necessitating biopsy in many cases. In patients with tuberous sclerosis, hepatic angiomyolipomas have a more consistent imaging appearance and, together with other features of the syndrome, can be more easily diagnosed. Preoperative diagnosis helps obviate unnecessary surgery.

CT angiography in renal angiomyolipomas.

Renal angiomyolipomas are fat containing tumours, which are associated with a strong risk of hemorrhage due to the presence of abnormal vessels and pseudoaneurysms. We present three cases of ruptured renal angiomyolipomas with multiple or large pseudoaneurysms diagnosed by CT angiography which enabled emergent endovascular intervention; and propose CT angiography as the diagnostic modality of choice, particularly in emergency situations.

[A case of angiomyolipoma in a horseshoe kidney].

The patient was 52 years old. She had undergone a breast cancer operation 4 years before this visit. On computed tomography (CT), a left renal tumor in a horseshoe kidney was incidentally pointed out. CT scan showed a 1.8-cm enhanced tumor in the upper pole of the left kidney. It was hyperechoic on ultrasonography. Since renal cell carcinoma could not be excluded preoperatively, left partial nephrectomy was performed. Pathological diagnosis was a renal angiomyolipoma. The incidence of horseshoe kidney is 1 in 400. The occurrence of hydronephrosis, infection and calculous disease is not uncommo

[Influence of pregnancy on renal angiomyolipoma].

The aim is to perform a literature search on the role of pregnancy in the rupture of renal angiomyolipoma. Articles published from 1952 to 2004 in the Medline database were searched using the keywords renal angiomylipoma and pregnancy. Pathologies associated with angiomylipoma (lymphangioleiomyomatosis and Bourneville tuberous sclerosis) were taken into account. Seventy-two cases of association of renal angiomyolipoma and pregnancy were found, out of which 58 presented a haemorrhage. In only 26% of the cases, renal angiomyolipoma had been documented prior to pregnancy. Clinical presentation wa

Angiomyolipoma in the knee--a case report.

Extrarenal angiomyolipoma is an uncommon disease and this may be the first report of angiomyolipoma in the knee, mimicking a malignant sarcoma. A 38-year-old man without tuberous sclerosis presented with a history of increased mass in the knee joint. Computerized tomography and magnetic resonance imaging demonstrated a CS x CS x CR cm subcutaneous tumor in the knee joint. The tumor was widely excised. Histologically, the tumor was, well circumscribed, and composed of smooth muscle, vascular spaces, connective tissue, and mature fat. There were no signs of recurrence at one year and eight month

Angiomyolipoma of the liver: case report.

Hepatic angiomyolipoma is a rare benign mesenchymal tumor of the liver. Most multiple hepatic angiomyolipomas have appeared in patients with renal angiomyolipoma and tuberous sclerosis. A 38-year-old female patient without chronic hepatitis B or C was hospitalized because of epigastric fullness for 2 months. Radiologic studies showed a large solid tumor with a small daughter nodule in the right hepatic lobe. Upon intravenous bolus injection of contrast medium, both tumors showed weak heterogeneous enhancement in the delayed phase. Although hepatocellular carcinoma was suspected by the findings

Angiomyolipomas of the liver in tuberous sclerosis.

Although the association of tuberous sclerosis and renal angiomyolipomas is well established, the occurrence of hepatic hamartomatous lesions in this disease is less well documented. We describe two cases of tuberous sclerosis with associated multiple intrahepatic angiomyolipomas, and we review the literature on this subject. Radiologically, angiomyolipomas can range from entirely lipomatous to completely solid, features which are present in our cases. A characteristic finding of the hepatic involvement in this disease appears to be the multiplicity of the lesions. We suggest that in the setti

Angiomyolipoma of the liver: a collective review.

This review summarizes the clinical and pathological findings of 52 cases of hepatic angiomyolipoma to discern and establish the most pertinent clinical and pathologic characteristics of the tumor. The disease was symptomatic in 60% of the patients. Abdominal pain or distress was the most common symptom, appearing in 37% of the patients, followed by malaise and upper abdominal mass or hepatomegaly. Of the 52 patients, only three (5.8%) showed associated tuberous sclerosis. Antemortem diagnosis of the tumor has been made with increasing frequency with the recent advent of computed tomography (C

Hepatic Epithelioid Angiomyolipoma: A Case Report.

Hepatic epithelioid angiomyolipoma (HEAML) is a rare mesenchymal tumor belonging to the family of perivascular epithelioid cell tumors (PEComas), a group of mesenchymal neoplasms characterized by the proliferation of distinctive perivascular epithelioid cells expressing both melanocytic and smooth muscle markers. Because of its hypervascular nature and nonspecific imaging features, HEAML can closely mimic other hypervascular hepatic lesions, including hepatocellular carcinoma, hepatic adenoma, and focal nodular hyperplasia, making accurate preoperative diagnosis challenging. A 42-year-old woma

Hepatic angiomyolipoma, misdiagnosed as hepatocellular carcinoma.

Perivascular epithelioid cell neoplasm (PEComa) is a rare type of tumor, and hepatic PEComa is even rarer. Its preoperative diagnosis is difficult, given the absence of specific clinical manifestations, often constituting an accidental finding, and the lack of a gold standard for identification using imaging studies. Instead, the diagnosis of hepatic PEComa is based on morphological and immunohistochemical features. We describe a case of an asymptomatic hepatic PEComa, angiomyolipoma type, which appeared in a middle-aged woman with chronic liver disease, during her follow-up and screening. Giv

Invasive Renal Angiomyolipoma With Cytologic Atypia.

Renal angiomyolipoma (AML) is a benign neoplasm of the kidney arising sporadically in an idiopathic manner, or syndromically as a component of tuberous sclerosis complex. Although the classic AML has no malignant potential, and is the most common mesenchymal tumor of the kidney, variant AML cases with epithelioid morphology have demonstrated aggressive or invasive behavior. Classic AML, on the other hand, can occasionally display focal histology concerning for sarcomatous transformation, but in the absence of invasive features, it is easy to distinguish from a malignancy. In this article, we d

Extrarenal retroperitoneal angiomyolipoma with unusual evolution.

The aim of this paper is to present the case of an extrarenal retroperitoneal angiomyolipoma with unusual evolution, due to the herniation through the inguinal canal, determining an extraperitoneal hernia. A ureteral duplicity and associated hydronephrosis contributed to the peculiarity of the case. The case was operated (en block tumor and right kidney removal), the postoperative evolution being favorable at seven years after the surgery.
