BACKGROUND: Atherosclerotic peripheral arterial disease (PAD) can lead to disabling ischemia and limb loss. Treatment modalities have included risk factor optimization through life-style modifications and medications, or operative approaches using both open and minimally invasive techniques, such as balloon angioplasty. Drug-eluting balloon (DEB) angioplasty has emerged as a promising alternative to uncoated balloon angioplasty for the treatment of this difficult disease process. By ballooning and coating the inside of atherosclerotic vessels with cytotoxic agents, such as paclitaxel, cellular

Treatment of in-stent restenosis (ISR) is associated with a high incidence of recurrence. This study evaluated the clinical safety and 6-month efficacy of excimer laser coronary angioplasty (ELCA) before scoring balloon dilatation for the treatment of ISR. Twenty-three patients with ISR were included and treatment strategy of ISR was dependent on each operator. Twelve patients among those were treated with ELCA before scoring balloon dilatation (ELCA group) and 11 patients were treated with scoring balloon alone (non-ELCA group). Acute procedural results were evaluated by quantitative coronary

The relative safety and thrombogenicity of pulsed excimer and thermal laser angioplasty systems were compared in 20 normal coronary artery segments in a total of seven pigs. Using similar over the wire catheter systems and laser delivery periods of 3 to 5 s, thermal laser angioplasty was achieved with a 1.3 mm metal probe heated with 10 W of continuous argon laser energy and excimer laser angioplasty was performed with a 4.5F excimer laser catheter consisting of 13 concentrically arranged 200 microns fiber optics delivering 35 to 40 mJ/mm2 of xenon chloride (308 nm) excimer laser irradiation a

Hemothorax is a rare complication following coronary angioplasty. We report a case of a young adult who presented with anterior wall myocardial infarction, underwent coronary angioplasty, and subsequently developed massive right-sided hemothorax. It was suspected clinically and a chest radiograph and contrast-enhanced computer tomography (CECT) of the thorax confirmed the diagnosis. Injury to the right subclavian and right internal mammary arteries was subsequently ruled out by selective angiography. Despite our best efforts, we could not find any evidence of perforation or any bleeding site;

Bradycardia, a condition commonly managed conservatively and, subsequently, with pacemaker implantation presents a unique challenge when coexisting with coronary artery disease (CAD). This case report delves into an unusual scenario where bradycardia and its related symptoms resolved following coronary angioplasty. Our goal is to contribute valuable data to the argument for a comprehensive evaluation of bradycardic patients for underlying CAD prior to considering pacemaker implantation. This approach aims to prevent unnecessary pacemaker implantations and offers insights into the optimal manag

BACKGROUND AND AIMS: We devised a technique to treat peripheral arterial disease (PAD) with laser, i.e. percutaneous transluminal laser angioplasty (PTLA). Considerable good results were obtained with PTLA, but it is apparently considered obsolete as a technique to treat occlusive arterial disease of peripheral arteries, perhaps because of the development and improvement of stents and the ease of their use compared to the somewhat intricate technique required for PTLA. Although the author admits the usefulness of stents, they are foreign to a human body. PTLA does not use a foreign body and co

Every percutaneous coronary intervention carries risk for acute and long-term complications. This is also true of chronic total occlusion (CTO) interventions, which can also have complications specific to specialized techniques, such as retrograde crossing and dissection/reentry techniques. Acute CTO intervention complications can be coronary artery-related, cardiac noncoronary, or noncardiac. In the long term, CTO interventions can be complicated by in-stent restenosis, stent thrombosis, or coronary aneurysm formation. Understanding of the pathogenesis of possible CTO intervention complicatio

Beginning with simple balloon angioplasty, minimally invasive revascularization techniques have progressed to the use of metallic stents for improved immediate and long-term results. Stent-supported angioplasty now offers a therapeutic option for those individuals ineligible for surgical revascularization of stenotic atherosclerotic lesions and who have failed maximal medical therapy. However, the clinical equivalence, or possibly even superiority, of angioplasty of the extracranial carotid and vertebral arteries in atheromatous occlusive disease over surgical revascularization has yet to be d

OBJECTIVE: Although anterior circulation disease has both medical and surgical treatment options, management of vertebrobasilar disease has predominantly had only medical options. Some patients remain symptomatic despite medical treatment, and angioplasty has been demonstrated to relieve critical stenoses. However, the elative safety and effectiveness of medical and surgical treatments is not clearly known. This report reviews the clinical characteristics, indications, and procedural risks of intracranial angioplasty in a series of patients with symptomatic posterior circulation ischemia.

BACKGROUND: Neurological deterioration in eclampsia is considered to be secondary to cerebral vasospasm. Magnesium sulfate therapy improves symptoms and controls seizures, possibly related to its vasorelaxive effects in spastic arteries. Some cases, however, are refractory to magnesium therapy. To our knowledge, there is no report of angioplasty for vasospasm from eclampsia in the literature.
METHODS: A 27-year-old woman presented 10 days postpartum with severe mental status changes and left arm and bilateral leg weakness that were refractory to magnesium therapy. Cerebral angiography demonst

Eleven patients with Takayasu Arteritis (TA) underwent angioplasty and stent placement in aorta, renal, carotid, subclavian and coronary arteries. Five wall stents were deployed in aorta in four patients. Indications for angioplasty and stent placement in aorta included hypertension in four patients and claudication and erectile impotence in one patient each. Post-procedure the peak systolic pressure gradient across the stenotic segment in the aorta disappeared. Six patients underwent angioplasty and stent placement in carotid arteries. Indications were syncope in 6 patients, loss of vision, s

PURPOSE: Endovascular treatment of carotid stenoses.
MATERIAL AND METHOD: 259 angioplasties of carotid stenosis are reported. The stenosis was an inflammatory stenosis or a restenosis post endarterectomy in 71 cases, atherosclerotic stenosis in 188 cases. Angioplasty was performed without cerebral protection in 123 cases including a group of 38 with atherosclerotic bifurcation stenosis. A cerebral protection (triple coaxial catheter) was used in 136 atherosclerotic bifurcation stenoses.
RESULTS: There has been no complication related to the procedure in the 71 cases of nonatherosclerotic ste

Thrombolysis is the most widely used method of coronary reperfusion in the acute phase of myocardial infarction. The indications of angioplasty after thrombolysis have been subject of considerable controversy over the last few years. Three randomised trials (TIMI 2, TAMI, ECSG) have shown that it is not desirable to perform systematic immediate angioplasty after intravenous thrombolysis with rt-PA. Angioplasty may be carried out as a "salvage" procedure in cases of failure of thrombolysis. The validity of this approach was confirmed recently by the "RESCUE" trial in anterior myocardial infarct

BACKGROUND: Although surgical endarterectomy or bypass is regarded as the treatment-of-choice for extracranial cerebral vascular stenosis, percutaneous transluminal angioplasty (PTA) has its role as an alternative procedure for symptomatic patients refractory to medical therapy and/or for whom surgical treatment is contraindicated.
METHODS: Occlusive blood vessels were first identified by neurologic, ultrasonic and angiographic studies. Patients were premedicated with heparin and corticosteroid. From a transfemoral approach, double lumen balloon dilatation catheter was guided across the area

Mesenteric angina is a difficult diagnosis and surgical treatment carries a significant morbidity and mortality. In patients with stenotic mesenteric vascular disease angioplasty offers an alternative method of treatment. In six patients who underwent mesenteric angioplasty relief of symptoms was achieved in five. Recurrence of symptoms associated with recurrent stenosis occurred in two patients and was remedied by re-dilation in one. Mesenteric angioplasty is a safe alternative to surgical revascularisation for mesenteric stenosis causing chronic intestinal ischaemia.

In 91 patients suffering from peripheral arterial occlusive disease (Fontaine stage IIb-IV) 125 percutaneous transluminal angioplasties (PTA) of crural arteries were performed. Eighty-six of the dilatations were done in combination with a recanalization procedure (PTA, laser angioplasty, fibrinolysis) of a femoropopliteal obstruction in order to improve outflow. PTA was performed with 5 F balloon catheters 2.5 to 4 mm in diameter in combination with steerable guide wires. A primary technical success was achieved in 41 of 42 (97.6%) vessels with a single stenosis, in 64 of 68 (94.1%) vessels wi

Percutaneous transluminal balloon angioplasty (PTBA) was performed in 87 patients for 111 stenotic lesions due to Takayasu arteritis. Of the lesions attempted for dilatation, 35 were in the aorta, 64 in renal arteries, 9 in subclavian, and 3 in common iliac arteries. The stenosis of aorta could be successfully dilated in 33 of 35 (94.3%) patients with fall in peak systolic pressure gradient (PSG) from 77.7 +/- 28.4 mmHg to 26.4 +/- 20.6 mmHg (P < 0.001) and increase in luminal diameter from 4.7 +/- 2.4 mm to 10.1 +/- 4.1 mm (P < 0.001). Percutaneous transluminal angioplasty was performed in 3 patients with stenotic portosystemic shunt (one splenorenal, one mesocaval, and one arterialized splenoportal shunt). In the first two cases, the procedure was successful, followed immediately by widening of the shunt and a decrease of the splenorenal and mesocaval pressure gradient. Shunt patency was confirmed at long term as the patients showed no evidence of esophageal varices or recurrent variceal bleeding. In the third case, results were disappointing as thrombosis of the shunt occurred at the end of the procedure. Nevertheless, we

The consequences of restenosis after angioplasty were evaluated in 466 patients who had coronary angiography 3 to 12 months after successful coronary angioplasty and were followed long term. The 236 subjects with restenosis resembled the 230 without restenosis with respect to age, sex, presence of multivessel disease, mean ejection fraction, prior myocardial infarction, prior coronary artery bypass grafting, and completeness of revascularization. The 5-year relative risk of revascularization for patients with restenosis markedly exceeded that for patients without restenosis. The relative risk
