Long-Term Follow-Up in Children with Anisocoria: Cocaine Test Results and Patient Outcome.

BACKGROUND: Evaluation of anisocoria including pharmacological testing for Horner's syndrome in the pediatric population is challenging in view of potential serious underlying disease. We describe cocaine test results, outcome of systemic investigation, and long-term follow-up in children with anisocoria. METHODS: Retrospective review of medical records and phone interview of consecutive pediatric patients (<18 years old) who underwent cocaine testing from August 2007 to July 2015 at a tertiary referral centre. RESULTS: A total of 35 patients were included with a positive, negative, or incon

[Prolonged toxic coma and anisocoria secondary to doxepin, lorazepam and phenobarbital poisoning--case study].

Doxepin is a tricyclic antidepressant from the group of dibenzoxepines. Apart from the antidepressant effect, it has also the sedative and anxiolytic effect, so it is used in the treatment of anxiety disorder in the course of psychosis, organic diseases and alcoholism. Doxepin increases concentration of norepinephrine and serotonin in the brain by preventing inactivation and blocking their reuptake. In addition, the drug has an antagonistic effect on receptors in CNS (muscarinic M1, histamine H1, alpha-1adrenergic, serotonergic 5-HT2) and also blocks sodium and potassium channels in cardiomyoc

Acute anisocoria: learning from a case of accidental exposure to nebulised medicines in the emergency department.

Anisocoria (unequal pupil size) occurring acutely can be indicative of serious neurological pathology. However, in healthcare professionals who handle nebulised medicines, acute anisocoria is often caused by benign conditions such as mydriasis (abnormal pupil dilation) triggered by exposure to certain drugs and occurring unilaterally. This article reports on the case of an emergency nurse who developed acute anisocoria while on shift. It describes the initial assessment and further investigations, outlines the differential diagnosis, and discusses the implications for emergency nursing practic

The Degree of Anisocoria in Pediatric Patients With Horner Syndrome When Compared to Children Without Disease.

PURPOSE: To study the magnitude of anisocoria in pediatric patients using the plusoptiX A08 (plusoptiX GmbH, Nuremberg, Germany) photoscreener as compared to a literature review of pediatric patients with known Horner syndrome to determine if anisocoria alone should raise suspicion for the diagnosis. METHODS: The medical records of 592 consecutive patients, neonates to 9 years old, were collected and analyzed. All patients had complete ophthalmic examinations that included photoscreening with the plusoptiX A08. Data included age, pupil sizes, and anisocoria. A complete literature search of do

Prediction of coma and anisocoria based on computerized tomography findings in patients with supratentorial intracerebral hemorrhage.

OBJECTIVES: Coma and anisocoria are the two common signs of a crucial state of neurological dysfunction. The ability to forecast the occurrence of these conditions would help clinicians make clinical risk assessments and decisions. PATIENTS AND METHODS: From October 2006 to September 2008, 118 patients with supratentorial intracerebral hemorrhage (SICH) were enrolled in this retrospective investigation. Patients were distributed into 3 groups according to occurrence of the signs of coma and/or anisocoria in the observation unit during a 30-day period. Group 1 included 52 patients who had norm

An Unusual Case of Low Vision and Anisocoria Considered a Neurological Finding in the Emergency Department: Ocular Siderosis.

We present the case of a patient who came to the emergency department with a significant decrease in vision and dilated pupil in the left eye. Since neurological pathologies were primarily considered, diffusion brain magnetic resonance imaging (MRI) and brain computed tomography (CT) were requested. After the results were reported as normal, we were consulted. On examination, the anterior segment was normal but we detected shiny pearl-like formations in the anterior vitreous, condensation at the inferior of the posterior vitreous, and a scar in the macula. When we evaluated the orbital section

Report of Two Cases: Altered Mental Status and Anisocoria as Presenting Symptoms in Acute Basilar Artery Occlusion.

INTRODUCTION: A posterior circulation stroke at the level of the basilar artery can cause ischemia to the brainstem, cerebellum, and occipital lobes. Posterior circulation strokes are notoriously more difficult to clinically diagnose than anterior circulation strokes, with a variety of presenting symptoms including altered mental status, dizziness, vision changes, nausea, and vomiting. Anisocoria has been reported to occur in rare cases. CASE REPORT: We present two cases where patients had an acute episode of altered mental status with a key exam finding of anisocoria, or unequal pupil sizes.

Anisocoria without extraocular muscle impairment due to moderate traumatic brain injury with midbrain contusion: a case report.

BACKGROUND: New-onset anisocoria is an important clinical clue to life-threatening intracranial injury. Anisocoria alone without impairment of extraocular muscles is a rare presentation of moderate traumatic brain injury (TBI). CASE PRESENTATION: A 79-year-old woman was transported to hospital soon after falling off a bicycle. Glasgow Coma Scale score on arrival was 11 (E3V3M5). On examination at admission, she was found to be drowsy. Bruising was seen around the right eye and pupil diameters differed (right, 4.5 mm; left, 3.0 mm; both reactive to light). Computed tomography of the head revea

An unusual case of anisocoria in a child with bleeding aneurysm of posterior communicating artery and idiopathic thrombocytopenic purpura (ITP).

BACKGROUND: ITP can give rise to bleeding episodes in different parts of the body including the central nervous system with various systemic manifestations. CASE: An eight-year old female child, diagnosed as a case of chronic ITP for last two years, developed intense headache and vomiting for a few days before admission. This was associated with right sided mid dilated pupil with brisk reaction to light. MRI-angiography showed a bleeding aneurysm of posterior communicating artery. The features of raised intra cranial tension subsided with conservative management but the anisocoria persisted.

Unilateral Ocular Siderosis Bulbi Due to Missed Metallic Intraocular Foreign Body Masquerading as Anisocoria of Neurological Origin: A Case Report.

BACKGROUND Ocular siderosis is an uncommon cause of vision loss due to a retained ferrous intraocular foreign bodies (IOFB) that cause iron deposition in ocular tissues. The most common manifestations are cataract formation, diffuse pigmentary changes of the retinal pigment epithelium, iris heterochromia, dilated pupils, secondary glaucoma, iritis, and cystoid macular edema. CASE REPORT We report a case of 38-year-old man who presented with a left dilated pupil and visual field defect. Neurological examination results were normal. Brain magnetic resonance imaging revealed a gross artifact at t

[Photo- and videographic determination of the dilatation deficit in differential diagnosis of Horner syndrome].

Physiological anisocoria, a very frequent condition among normal subjects, must be differentiated from Horner's syndrome. Aside from a history including evaluation of older photographs, pharmacological pupil testing with cocaine eye drops has been the "gold standard". Measurement of dilation lag, occurring in Horner's syndrome, is much less common and normal values are missing. Therefore, we evaluated various photographic parameters for testing for the best parameter to discriminate Horner's syndrome (22 patients) from physiological anisocoria (16 patients). Additionally, we tested whether a c

Critical evaluation of the cocaine test in the diagnosis of Horner's syndrome.

We evaluated the effectiveness of the cocaine test for diagnosing Horner's syndrome. The test was administered to 119 patients with a diagnosis of Horner's syndrome and to 50 normal subjects. We compared the cocaine-induced anisocoria in the two groups by measuring photographs of the pupils. We found the cocaine test to be highly effective in separating normal subjects from patients with Horner's syndrome. The chances of having Horner's syndrome increased with the amount of cocaine-induced anisocoria. Through the use of logistic regression analysis, we determined the odds ratio of having Horne

Effect of 0.5% apraclonidine on ptosis in Horner syndrome.

To demonstrate the effect of apraclonidine on anisocoria and ptosis in Horner syndrome, one drop of 0.5% apraclonidine was instilled in both eyes of 3 patients who presented with acute Horner syndrome, and the effect on ptosis and anisocoria was documented. As reported in the literature, one drop of 0.5% apraclonidine reverses the anisocoria of Horner syndrome. In addition, 0.5% apraclonidine leads to a complete resolution of the ptosis associated with Horner syndrome, a finding reported once in the literature. Apraclonidine is a safe and readily available alternative to cocaine for the diagno

Internal Carotid Artery Dissection Presenting with Transient or Subclinical Horner Syndrome.

INTRODUCTION: The most frequently encountered symptoms in internal carotid artery dissection (ICAD) are head or neck pain and cerebral ischemia. Ocular symptoms or signs have been reported as the presenting feature in up to 50% of patients, with (painful) Horner syndrome being the most frequently associated. Horner syndrome is part of the classic triad that depicts the characteristic presentation of ICAD and that consists of pain in the ipsilateral neck, head and orbital regions, (partial) Horner syndrome, and cerebral or retinal ischemia. All patients presenting with painful Horner syndrome s

Brief, transient Horner's syndrome can be the hallmark of a carotid artery dissection.

We describe a 41-year-old woman in whom the diagnosis of carotid artery dissection was suspected based on a recent history of anisocoria and ipsilateral ptosis that lasted 2 days. She had a normal neurologic examination, including no clinical evidence of anisocoria or ptosis. Subsequently, a cocaine test demonstrated pharmacologic Horner's syndrome. MRI confirmed the carotid dissection. This patient illustrates that a history of transient pupillary and eyelid abnormalities can lead to the diagnosis of a carotid dissection. Specific questioning about transient anisocoria and ptosis should be co
