The production and characterization of anti-bothropic and anti-crotalic IgY antibodies in laying hens: a long term experiment.

Ophidian accidents represent a great public health problem in developing countries. Recent studies have shown that antibodies produced in la … antigens with molecular masses of 14 kDa and 30 kDa. An increase in the antigens recognized by the antivenins was observed during the experimental period. Samples of bothropic IgY antivenin antibodies presented an efficiency of 290 μl/3 DL50, a potency of 0.307 mg/ml and a specific activity of 0.230. Samples of anti-crotalic IgY antibodies presented an effi …

An Unexpected Case of Black Mamba (Dendroaspis polylepis) Bite in Switzerland.

Mambas (genus Dendroaspis) are among the most feared venomous African snakes. Without medical treatment, mamba bites are frequently fatal. First-aid treatment includes lymphatic retardation with the pressure immobilization technique. Medical management comprises continuous monitoring, securing patency of the airway, ensuring adequate ventilation, symptomatic measures, and administration of specific antivenin. We report an unusual case of a snake breeder bitten by a black mamba in Switzerland, report the clinical course, and review the lifesaving emergency management of mamba bites. This case h

Protobothrops mangshanensis bite: first clinical report of envenoming and its treatment.

AIM: This case report presents envenoming by the Chinese pit viper Protobothrops mangshanensis (formerly Zhaoermia) and its treatment. METHODS: A 38 year old snake breeder suffered two-fang bites to elbow by a Chinese pit viper Protobothrops mangshanensis resulting in local edema of the affected arm. No other signs of envenoming appeared. On the 5(th) day following the bite a hematoma developed on the other arm which had been mechanically injured 14 days before. Laboratory testing revealed severe coagulopathy with hypofibrinogenemia and immeasurably prolonged coagulation times. RESULTS: As s

Failure of electric shock treatment for rattlesnake envenomation.

The use of high-voltage electric shock therapy for the treatment of snake venom poisoning has recently gained popularity in the United States. We present a case that documents the dangerous, ineffective application of electric shock to the face of a patient envenomated by a Great Basin rattlesnake (Crotalus viridis lutosus). The successful use of antivenin in this critically ill, antivenin-allergic patient is described.

Black widow spider bite.

These two cases illustrate many classical features of latrodectism. A subclinical bite followed by extensive systemic signs and symptoms in a recognizable pattern is strongly suggestive of the diagnosis. Pain and muscle spasms are refractory to standard therapy but are relieved by intravenous calcium. Finally, there should be complete resolution of the symptoms after only one vial of antivenin is administered. The common features of these cases form a recognizable constellation of signs and symptoms known as latrodectism. The specifics of these cases highlight the uncertainty of therapy. The o

[Adder bites in Turku region in 2000 to 2010].

Adder bites cause dozens of hospital visits and complications in Finland each year Some of the patients also suffer from troublesome late symptoms. We established the number of adder bites, use of antivenin, and degree of severity of the bites in child and adult patients treated at TYKS in 2000 to 2010. Antivenin was given to 9.6% of the bitten adults and 17.3% of the children. The number of adder bites was highest in July. Accidental stepping on an adder or hitting an adder-inhabited tussock with the berry picker's hand were the most common cases. Severe cases were rare.

Exotic snakebite: envenomation by an African puff adder (Bitis arietans).

We report an envenomation by the African puff adder (Bitis arietans), an exotic snake in the United States. The patient developed swelling and ecchymoses in the affected extremity, and cutaneous necrosis of the envenomated fingertip. There was no significant coagulopathy. He received 20 vials of specific antivenin (Schlangengift-Immunserum Behring Zentralafrika, Behringwerke, Marburg, Germany) and debridement of devitalized finger tissue. The only permanent sequelae were cutaneous scarring and permanent loss of the fingernail on the envenomated finger. Exotic snakebite is a rare presenting pro

Venomous snakebite in Thailand. II: Clinical experience.

We reviewed a total of 2,525 snakebite patients in Bangkok. Of these, 1,415 were bitten by venomous snakes, 91 by neurotoxic snakes of genus Naja or Bungarus and 1,324 by snakes of family Viperidae or Crotalidae. Seventy-one percent of bites were on the lower extremity. There were two fatal cobra bites; both patients were dead on arrival at the hospital. Bites from vipers caused morbidity but no deaths. Species-specific antivenins are effective in reversing respiratory failure from cobra bites and coagulopathies from bites by Viperidae and Crotalidae snakes. However, early respiratory and woun…

Anaphylactoid reaction to rattlesnake envenomation.

The clinical manifestations of an anaphylactoid reaction are identical to true anaphylaxis; however, a previous exposure to the offending agent is not needed to manifest these symptoms. We present a case of an anaphylactoid reaction in a 62-y-o female following a first-time envenomation by a rattlesnake. The patient required s.c. epinephrine and i.v. diphenhydramine, methylprednisolone, and ranitidine. She had not been envenomated by a rattlesnake previously or received any horse-derived antivenins in the past.

Stenosing flexor tenosynovitis following a rattlesnake bite.

Snakebite victims have been described previously in orthopedic literature in regard to complications such as compartment syndrome and carpal tunnel syndrome. We introduce a previously unreported complication of stenosing flexor tenosynovitis in a patient bitten by a rattlesnake. After being bitten in her right forearm, the patient experienced mild systemic symptoms of fever and nausea and was assessed at an outside hospital, where it was determined that she did not suffer from envenomation and therefore did not require antivenin therapy. She presented to our institution 1 week later with signs

Characterization of Chicken-Derived Single Chain Antibody Fragments against Venom of Naja Naja Atra.

Traditional, horse-derived antivenin is currently the most efficient treatment against snake bites. However, it is costly and has unpredictable side effects. Thus, alternative, cost-effective strategies for producing antivenin are needed. In this study, we immunized hens with inactivated NNA venom proteins from the cobra Naja naja atra (NNA). Purified yolk IgY antibodies showed specific anti-NNA binding activity comparable to that of the equine-derived antivenin. We used phage display technology to generate two antibody libraries containing 9.0 × 10⁸ and 8.4 × 10⁸ clones with a short or long l

Comparative analysis of newborn and adult Bothrops jararaca snake venoms.

Different clinical manifestations have been reported to occur in patients bitten by newborn and adult Bothrops jararaca snakes. Herein, we studied the chemical composition and biological activities of B. jararaca venoms and their immunoneutralization by commercial antivenin at these ontogenetic stages. Important differences in protein profiles were noticed both in SDS-PAGE and two-dimensional electrophoresis. Newborn venom showed lower proteolytic activity on collagen and fibrinogen, diminished hemorrhagic activity in mouse skin and hind paws, and lower edematogenic, ADPase and 5'-nucleotidase

Snake venom or antivenom induced urticaria.

An amateur herpetologist developed chills, diaphoresis and generalized urticaria 5 h after receiving Antivenin (Crotalidae) Polyvalent (Wyeth) for treatment of rattlesnake (Crotalus atrox) envenomation. The patient had been bitten 8 mo earlier by a copperhead (Agkistrodon contortrix mokeson) resulting in minimal envenomation. He also had been skin testing himself for 6 mo with both diluted Crotalus atrox venom and Antivenin (Crotalidae) Polyvalent (Wyeth) to determine how sensitive he was to antivenom and how resistant he was to the effects of venom. Unusual reactions to antivenom during the t

Natural Snake Venom Inhibitors and their Pharmaceutical Uses: Challenges and Possibilities.

Nowadays, treatment with specific antivenins is considered the only cure for snakebites accidents. However, access to antivenom obstructs the successful implementation of the World Health Organization international guidelines. In the last few years, natural organic compounds, peptides, and proteins with the ability to inhibit snake toxins and obtained from different sources such as plant extracts and animal blood have been proposed as antivenoms. In this work, we will focus on the inhibitors of the main venom toxins, phospholipases A2 and metalloproteinases, and their application as novel anti…

Russell's viper bite in Sri Lanka. A study of 22 cases.

Twenty-two patients who had been bitten by a Russell's viper were studied. Neurological manifestations and generalized myalgia were observed, respectively, in 86.4% and 72.7%. Renal failure did not occur in three patients who received antivenin within 5 hours of the bite, and it is suggested that administration of antivenin within the first few hours following the bite could prevent renal failure. Of 19 patients who were in acute renal failure, seven responded to conservative management while 12 needed peritoneal dialysis. Nine patients developed pulmonary edema and four had grand mal seizures

Identification and neutralization of biological activities associated with venom from the brown recluse spider, Loxosceles reclusa.

Venom from the brown recluse spider, Loxosceles reclusa, reacted with human erythrocytes to form venom-sensitized erythrocytes. These cells were agglutinated specifically by high dilutions of adsorbed rabbit antivenin or were lysed by normal blood group compatible human sera. The specific rabbit antivenin prevented venom from attaching to erythrocytes, from interacting with serum complement, and from producing dermonecrotic lesions in rabbits. Results of experiments involving heat inactivation and adsorption to erythrocytes provide circumstantial evidence to suggest that the three biological a
