Cortex; a journal devoted to the study of the nervous system and behavior2025eng
Distinguishing between neurological and psychiatric disorders can be challenging, particularly when apathy serves as the primary symptom, as it is a common manifestation in both neurodegenerative diseases and mood disorders. We present a cohort of 11 patients (7 females and 4 males) aged over 50 years (mean age 61.36 ± 5.870 SD). These individuals exhibited a prolonged and isolated apathy lasting over a year, without concurrent dysthymia, but with a history of a preceding major depressive episode. This article aims to comprehensively profile these patients and to explore whether this condition

Apathy is a core symptom in patients with behavioural variant frontotemporal dementia (bvFTD). It is defined by the observable reduction in goal-directed behaviour, but the underlying mechanisms are poorly understood. According to decision theory, engagement in goal-directed behaviour depends on a cost-benefit optimization trading off the estimated effort (related to the behaviour) against the expected reward (related to the goal). In this framework, apathy would thus result from either a decreased appetence for reward, or from an increased aversion to effort. Here, we phenotyped the motivatio

A reduction in goal-directed behavior, or apathy, occurs in neurological and psychiatric disorders, though its neural substrates remain unclear. Deficits in circuits connecting the prefrontal cortex to subcortical regions are considered to underlie apathy. Although apathy is empirically associated with widespread changes in these regions, studies across disorders also link apathy with the lateral parietal cortex. Such variety in regional involvement is consistent with the established role of prefrontal and subcortical regions in models of goal-directed behavior, and with the suggestion of subt

BACKGROUND AND PURPOSE: Apathy is the most commonly reported behavioural change in amyotrophic lateral sclerosis (ALS). However, the degree to which it affects prognosis and overlaps with depression in this population is unknown. The present study examined the relationship between level of apathy, mortality and survival time and whether apathy was linked to specific symptom clusters of depression.
METHODS: A cohort of 76 consecutive ALS patients attending specialized multidisciplinary clinics were classified according to level of apathy. The effects of clinical factors and apathy on survival

BACKGROUND: Subcortical vascular dementia relates to small-vessel disease and hypoperfusion, resulting in focal and diffuse ischemic white matter lesions. The main target of the disease are the frontal subcortical neural networks. There is no clinical standard definition of the pathology, on the contrary, everyday clinical practice suggests dominant behavioral alterations and dysexecutive syndrome.
METHODS: The aim of this study was to investigate gait disorders, behavioral alteration, and drug intake of a subcortical population with dementia (n = 1155). A complete neuropsychological examinat

This review explores the relationships between depression, apathy, and anosognosia in Alzheimer's disease. Depressed mood is found to be associated with less anosognosia, while greater apathy is associated with more anosognosia, and the contrasting reasons for these associations are discussed. The review also describes recent research findings indicating a dissociation between impaired awareness of condition/deficit and preserved emotional reactivity in response to illness-related material or the experience of failure in tests. We conclude by pointing to future directions for this area of rese

Apathy is one of the most prevalent neurobehavioral symptoms in Huntington's disease (HD), occurring in approximately 70% of the symptomatic HD population. Apathy scores in patients with HD are highly correlated with duration of illness, suggesting that apathy is an inevitable consequence of advanced disease. Although less distressing than symptoms like depression and less disruptive than irritability or aggression, apathy has a considerable adverse impact on those affected with HD because it leads to a decrease of the goal-directed behaviors that contribute much to the day-to-day quality of l

BACKGROUND AND PURPOSE: Stroke survivors are often described as apathetic. Because apathy may be a barrier to participation in promising therapies, more needs to be learned about apathy symptoms after stroke. The specific objective was to estimate the extent to which apathy changes with time over the first year after stroke and the impact of apathy on recovery.
METHODS: The Apathy Assessed cohort was formed from stroke survivors participating in a longitudinal study of health-related quality of life after stroke. A family caregiver completed an apathy questionnaire by telephone at 1, 3, 6, an

We examined the prevalence and correlates of apathy and irritability in a consecutive series of 101 patients with probable Alzheimer's disease (AD). Based on clinical criteria, 46 (46%) patients had apathy, and 13 (13%) patients had irritability. Apathy was significantly associated with more severe impairments in activities of daily living, significantly more severe extrapyramidal signs, and a significantly higher frequency of both major depression and dysthymia. Patients with irritability had significantly more severe impairments in activities of daily living and significantly higher depressi

Apathy, defined as a significant and sustained reduction in goal-directed behavior associated with impaired daily functioning, is a common and clinically important syndrome in mild and major neurocognitive disorders (NCDs) and is associated with poorer outcomes for patients and carers. Despite growing research, important gaps in knowledge continue to hinder accurate recognition and effective treatment. This International Psychogeriatric Association (IPA) white paper summarizes key scientific and clinical insights from a multidisciplinary Apathy Task Force, with the aim of providing a narrative

BACKGROUND: Apathy is common in individuals with mild cognitive impairment (MCI) or dementia and mild behavioral impairment (MBI), and is associated with reduced quality of life and increased caregiver burden. Home-based robot-assisted interventions offer a novel approach for cognitive stimulation and behavioral management, but robust evidence on feasibility, acceptability, and clinical impact is limited.
OBJECTIVES: To evaluate the effects of home-based robotic interventions on the level of apathy in individuals with cognitive decline, including MCI, MBI, and dementia. Secondary objectives a

The hippocampus is increasingly recognized for its role in working memory representations and goal-directed behavior, yet whether these functions share common neural substrates remains unclear. We investigated short-term memory performance in 27 patients (21 males and 6 females) with LGI1-antibody limbic encephalitis, a condition with predominant hippocampal involvement, compared to 27 age- and gender-matched healthy controls, using an object-location continuous report task with Bayesian mixture-modeling. Patients exhibited predominantly elevated misbinding errors-incorrectly associating objec

BACKGROUND: Apathy denotes a lack of motivation, enthusiasm, feelings, or concern. Decreased daily functions and cognitive impairment are other features. In the elderly, these manifestations are overlooked and attributed to ageing or associated comorbidities. Spotted fever group rickettsial (SFG) infections are endemic in the Central Hills of Sri Lanka. We observed occult SFG infection as a treatable cause of apathy in the elderly. METHODS: We studied patients with apathy admitted to the Teaching Hospital, Peradeniya (THP), Sri Lanka, during 2018 & 2019. The diagnosis was confirmed by indirect

OBJECTIVE: Although apathy is commonly reported among patients with Huntington's disease (HD), this psychiatric symptom has not yet been clearly defined or extensively studied in HD. Most researchers have adopted descriptive approaches, showing that apathy is a multidimensional entity but leaving the processes underlying its different dimensions relatively unexplored.
METHODS: A systematic review of the literature on apathy in HD, focusing on current approaches and measurement tools, was conducted.
RESULTS: Searches in PubMed and PubMed Central yielded 368 articles, 25 of which were included

The Apathy Evaluation Scale (AES) is a tool utilized with individuals with brain injury, neurocognitive disorders, and other mixed populations to quantify and characterize apathy in adults. The scale "treats apathy as a psychological dimension defined by simultaneous deficits in the overt behavioral, cognitive, and emotional concomitants of goal-directed behavior." It has three versions: self-rated (AES-S), clinician-rated (AES-C), and informant-rated (AES-I). Using factor analysis, Marin and colleagues identified three factors for the scale, including general apathy, disinterest or amotivatio

OBJECTIVES: Apathy is the most common behavioral symptom of amyotrophic lateral sclerosis (ALS). Despite its known impact on caregiver wellbeing, apathy is typically considered a unitary construct making assessment and targeting treatment problematic. The aim of this study was to explore the relationship between caregiver burden and the behavioral, cognitive, and emotional symptoms of apathy in ALS.
METHODS: Fifty-one ALS patient-caregiver dyads from an ALS/frontotemporal dementia Clinic were assessed with the Apathy Evaluation Scale which measured the cognitive, behavioral, emotional, and no

BACKGROUND: Apathy is the most prevalent and characteristic neuropsychiatric feature of Huntington's disease. Congruent with the main early pathological changes, apathy is primarily associated with subcortical damage in frontal-striatal circuits. However, little is known about its precise subserving mechanisms and the contribution of regions other than the basal ganglia.
OBJECTIVES: We aimed to define the neural correlates of apathy in Huntington's disease based on gray matter volume and PET/CT of 18 F-fluorodeoxyglucose metabolism.
METHODS: We rated the severity of apathy in 40 mild Hunting

OBJECTIVES: Fatigue and apathy are two of the most common and most disabling non-motor symptoms of Parkinson's disease (PD). They have a high coincidence and can often be confused; moreover, their relationship is not fully understood. The aim of our study was to describe the coincidence of apathy with different fatigue domains in the presence/absence of depression and to separately describe the associations of different aspects of primary and secondary fatigue with apathy and other clinical and disease-related factors.
MATERIALS AND METHODS: A total of 151 non-demented patients with PD were e

BACKGROUND: Apathy is a major component of the behavioral and psychological symptoms of Alzheimer's Disease (AD) and other types of dementia. Most researchers have reached a consensus on a new set of diagnostic criteria for apathy (DCA) recently. However, no relevant reports on apathy exist for AD patients in Asian countries yet.
OBJECTIVES: To estimate the prevalence of apathy in Chinese AD patients.
METHODS: 83 AD patients were recruited for a cross-sectional observational study. Following the new diagnostic criteria for apathy (DCA) and DSM-IV criteria for Major Depressive Disorder (MDD),
