Trauma in immature teeth can lead to open apices, posing challenges for treatment. Calcium hydroxide (Ca(OH)2), the traditional option, has limitations such as lengthy treatment times and risks of fracture. Mineral trioxide aggregate (MTA) has emerged as a superior alternative, offering excellent sealing and biocompatibility. This paper describes managing an immature central incisor in a 19-year-old male with necrotic pulp using an MTA apical plug, with a 24-month follow-up. The presented case outlines the difficulties encountered in treating an immature tooth with an acute apical abscess due

BACKGROUND: Apexification in immature teeth represents a clinical challenge due to the difficulty of achieving an effective apical seal and sufficient mechanical resistance. Bioceramic materials such as MTA Repair HP, Bio-C Repair, Biodentine, and PBS Cimmo HP have been developed to improve sealing ability and bond strength. The objective of this study was to evaluate and compare, in vitro, the apical marginal adaptation by Scanning Electron Microscopy (SEM) and bond strength through the push-out test of these four calcium silicate- based cements used as an apical barrier.

BACKGROUND: Immature teeth must be managed by apexification as there is no definite apical stop. Use of intracanal medicaments prior to apexification affect the sealing ability of apical plug. Present study was done to compare the effect of various intracanal medicaments: Metapex, Chlorhexidine digluconate, Curcuma longa, and Triple antibiotic paste (TAP) on the sealing ability of MTA and Hydroxyapatite as an apical plug.
METHODOLOGY: 130 single-rooted teeth were divided into two experimental groups (n=60 each) [Group I: MTA; Group II: Hydroxyapatite], and two control groups (n=5 each) [Group

The greatest threats to developing teeth are dental caries and traumatic injuries. The primary goal of all restorative treatment is to maintain pulp vitality so that normal root development or apexogenesis can occur. If pulpal exposure occurs, then a pulpotomy procedure aims to preserve pulp vitality to allow for normal root development. Historically, calcium hydroxide has been the material of choice for pulpotomy procedures. Recently, an alternative material called mineral trioxide aggregate (MTA) has demonstrated the ability to induce hard-tissue formation in pulpal tissue. This article desc

Removal of foreign objects from the root canal can be very frustrating. The use of a variety of instruments and techniques has been suggested for the retrieval of obstacles from root canals during endodontic treatment. This article describes a method for retrieving a large mass of amalgam restoration that was wedged into the root canal. The amalgam, which had served as the provisional restorative material during apexification of an immature ante rior tooth, was inadvertently pushed into the root canal. After the mass was bypassed, the amalgam was loosened with the aid of copious irrigation, ch

An 8-year old boy was referred to the orthodontic department because of a fused tooth. The clinical and radiological examination showed that - besides the double tooth at the region of the 21 - there was a normal tooth 22 in eruption as well. Moreover a still impacted supernumerary tooth was present in the right upper front region. The treatment included the extraction of the fused tooth and an autotransplantation of the supernumerary tooth to the extraction site of the fused tooth. During the follow-up period the pulp showed a progressive obliteration starting at the crown level with a conser

The purpose of this case report is to discuss the four-year follow-up of a transalveolar transplantation of an impacted immature permanent mandibular left canine. A nine year-old-boy was referred to the dental school because of a mandibular swelling associated with the impacted canine. Under local anesthesia, the tooth was extracted and transplanted in its own space, followed by a two-week orthodontic fixation. The radiographic examination two months later revealed the presence of external inflammatory root resorption, which was treated with an apexification. The overall status of the transpla

Many studies have focused on the search for a restorative material with good sealing properties and biocompatibility for treatment of teeth with open apices and necrotic pulps, which can result from periradicular disease and root resorption. Mineral trioxide aggregate (MTA) has exhibited promising clinical results in retrograde fillings and pulpotomies as well as for treatment of root perforations, root resorptions, incomplete root formations, and pulpal necrosis. This case report describes the management of a progressive apical root resorption in a previously traumatized tooth that had been e

This work describes a case of "dens invaginatus" and analyzes the possible aspects of this malformation. An unusual type of dens invaginatus was detected in a young patient corresponding to the maxillary lateral incisor and showing extensive periradicular radiolucency and a vestibular fistula. The radiographic and tomographic examination revealed two apices: one wide open in the distal part of the root and the other normally formed in the mesial. Nonsurgical endodontic treatment was performed by using the "one-step apexification technique," filling both apexes with mineral trioxide aggregate f

Infection of the root canal system following dental trauma induces pulp and periapical disease and prevents healing of previously healthy pulp. A clinical goal in treating trauma is the maintenance of pulp vitality, and clinicians should be aware of factors that influence pulp healing. The learning objective of this article is to review the factors and techniques that influence pulp vitality and examine the influence pulp has on the healing of adjacent tissues. The potential routes for bacterial infection of the root canal system are discussed, with the clinical crown as the primary portal of

These case reports describe traumatic dental injuries affecting maxillary permanent central incisors in young children, which may lead to pulp necrosis with or without apical periodontitis. Immature teeth with necrotic pulp are very challenging to treat because of their short and thin dentinal walls, open apices, and broad root canals that make them weak and prone to fracture, which can ultimately render the tooth unrestorable and require extraction. Apexification has traditionally been the treatment of choice for such cases; however, recently, regenerative and revascularization procedures hav

Dental trauma is a very frequent occurrence in children and adolescents, which creates a great impact on the esthetics, functions, and phonetics. Traumatic dental injuries can range from simple enamel fractures to permanent tooth loss. This case report presents an eight-year-old patient with an uncomplicated crown fracture of tooth 21, and 30 days after trauma, it was diagnosed as necrotic pulp. The first treatment choice was a regenerative endodontic procedure (REP), however, the failure led to apexification with Mineral Trioxide Aggregate (MTA). The chosen rehabilitation was a composite vene

A case of extensive crown fracture associated with intrusion of the permanent maxillary central incisors in an 8-year-old boy is reported. The treatment of both injured teeth included attempts of apexification and arrest of root resorption with calcium hydroxide. After 8 months of the trauma, there was no calcified barrier formation in the apex. Mineral trioxide aggregate (MTA) was then used as a filling material. At 15-month follow up, the teeth were asymptomatic and correctly sealed, the external inflammatory root resorption had stopped, and the radiolucent image had disappeared, which meant

BACKGROUND: Regenerative endodontics has evolved in recent years with tissue engineering concepts in particular appearing promising. Endodontic tissue engineering (ETE) describes the various approaches based on the orthograde introduction of scaffolds or biomaterials (with or without cells) into the root canal to achieve pulp tissue regeneration. There are currently no systematic reviews investigating whether ETE is a suitable method for the treatment of endodontic disease in both mature and immature permanent teeth.
OBJECTIVES: The purpose of this systematic review was to determine the effec

PURPOSE: A finite element analysis (FEA) study was performed to determine whether the material of apical barrier used for root apexification and/or the use of canal reinforcement affect the stress distribution in an endodontically treated immature permanent tooth in order to infer in which clinical scenarios a fracture is more likely to occur based on the ultimate tensile strength threshold of dentin.
METHODS AND MATERIALS: An extracted human immature mandibular premolar was selected as the reference model and scanned by micro-computed tomography (micro-CT). The digital model was segmented an
