Outcomes of infants with Apgar score of zero at 10 min: the West Australian experience.

BACKGROUND: Infants who have an Apgar score of zero at 10 min of age are known to have poor long-term prognosis. Expert committee guidelines suggest that it is reasonable to cease resuscitation efforts if the asphyxiated infant does not demonstrate a heart beat by 10 min of life. These guidelines are based on data from the era when therapeutic hypothermia was not the standard of care for hypoxic ischaemic encephalopathy (HIE). Hence, we aimed to review our unit data from the era of therapeutic hypothermia to evaluate the outcomes of infants who had an Apgar score of zero at 10 min and had surv

Gathering the evidence: cord gases and placental histology for births with low Apgar scores.

BACKGROUND: Acute intrapartum hypoxia is an uncommon cause of cerebral palsy. The exclusion of acute intrapartum hypoxia utilizes two vital pieces of information that can be obtained at the time of birth: (i) cord blood gas to exclude a severe metabolic acidosis, and (ii) placental histology to suggest an alternative aetiology other than acute intrapartum hypoxia. Although recommendations exist to encourage this practice in high-risk deliveries, their compliance in an Australian setting is not known. AIMS: To evaluate the frequency and utility of cord blood gases and placental histology follo

The effect of birth order in twins on fetal umbilical blood gas and apgar score.

Although recent reports have revealed no difference in perinatal mortality between a set of twins, more sensitive means other than perinatal mortality are necessary to evaluate differences in twins. This study examined differences between 34 twin pairs with respect to Apgar score, umbilical venous and arterial blood gas, and acid-base data. The parameters were measured in paired samples and compared with the paired t test and Fisher's exact probability test. The most coherent findings were a higher umbilical venous PO2 and umbilical arterial PO2 in twin A. Their differences were not due to the

Apgar Score Plus Umbilical Artery pH and Adverse Neonatal Outcomes in Very Preterm Infants.

IMPORTANCE: The Apgar score, the first clinical assessment to direct measures to stabilize newborn infants, is also used for risk assessment. Its accuracy in estimating outcomes remains poor among very preterm (VPT) infants. OBJECTIVE: To assess the utility of the combined 5-minute Apgar score and umbilical artery pH (UA-pH) for estimating risks of mortality and severe neonatal morbidity among VPT infants. DESIGN, SETTING, AND PARTICIPANTS: This cohort study (Effective Perinatal Intensive Care in Europe [EPICE]) analyzed infants born at less than 32 weeks' gestation between April 2011 and Se

The relationship between the five minute Apgar score, mode of birth and neonatal outcomes.

OBJECTIVE: To investigate the relationship between the five-minute Apgar score categories (low, intermediate, and normal), mode of birth and neonatal outcomes. METHODS: This was a retrospective cross sectional study of term singleton deliveries at Mater Mothers' Hospital in Brisbane, Australia between January 2007 and December 2015. The five minute score was subdivided in to three categories - low (0-3), intermediate (4-6), and normal (≥7). These were correlated with adverse neonatal outcomes and mode of birth. The referent cohort was the normal Apgar group. RESULTS: The study cohort consist

Five-minute Apgar score ≤ 5 and Molar Incisor Hypomineralisation (MIH) - a case control study.

BACKGROUND: The aetiology of molar incisor hypomineralisation (MIH) is unclear. The asymmetric distribution of MIH in the dentition may indicate that an insult of short duration that affects ameloblasts at a vulnerable stage could be a causative factor. Apgar ≤ 5 at 5 min may indicate asphyxia (hypoxic-ischemic insult) during birth. It was hypnotised that low Apgar score during birth may cause MIH. The present study aimed to examine a possible association between Apgar ≤ 5 at 5 min and the occurrence of MIH. METHOD: Two study groups were selected for examination. The cases comprised 67 childr

Apgar score and dental caries risk in the primary dentition of five year olds.

BACKGROUND: Conditions in utero and early life underlie risk for several childhood disorders. This study tested the hypothesis that the Apgar score predicted dental caries in the primary dentition. METHODS: A retrospective cohort study conducted in 2003 examined associations between conditions at birth and early life with dental caries experience at five years. Dental examination data for a random sample of five-year-old South Australian children were obtained from School Dental Service electronic records. A questionnaire mailed to the parents obtained information about neonatal status at del

Brainstem auditory-evoked responses in full-term newborn infants with temporary low Apgar score.

CONCLUSIONS: No abnormalities, with the exception of maturational changes, in BAER were found during the neonatal period. The results suggest that a temporary low Apgar score is not accompanied by any significant auditory impairment. OBJECTIVE: To examine brainstem auditory function in newborn infants with a temporary low Apgar score but no clinical signs of hypoxic-ischaemic encephalopathy (HIE). MATERIAL AND METHODS: The subjects were 36 full-term infants with Apgar scores of < or =7 at 1 and/or 5 min and > or =8 at 10 min but without HIE. The brainstem auditory-evoked response (BAER) was

The 5-minute Apgar Score as a Prognostic Factor for Development and Progression of Retinopathy of Prematurity.

BACKGROUND: A low Apgar score at 5 minutes has been shown to be a risk factor for development of retinopathy of prematurity (ROP). AIM: To examine the prognostic value of Apgar score at 5 minutes for development and progression of ROP. MATERIALS AND METHODS: The study included 132 preterm infants who were screened from 4th week of life onward. Of these, 118 newborns were given Apgar score at 5 minutes. The prognostic significance of this index was studied as an absolute value and as a value ≤ 6. The patients were divided into two groups: group I had no evidence of ROP (n=82) and group II had

Acidemia in neonates with a 5-minute Apgar score of 7 or greater - What are the outcomes?

BACKGROUND: The Apgar score is universally used for fetal assessment at the time of birth, whereas, the collection of fetal cord blood gases is performed commonly in high-risk situations or in the setting of Apgar scores of <7, which is a less standardized approach. It has been well-established that neonatal acidemia at the time of delivery can result in significant neonatal morbidity and death. Because of this association, knowledge of the fetal acid-base status and detection of acidemia at the time of delivery can serve as a sensitive and useful component in the assessment of a neonate's ris

Antecedents and neonatal consequences of low Apgar scores in preterm newborns: a population study.

BACKGROUND: To examine the antenatal and early neonatal correlates of low Apgar scores (<3 and <6 at 1 and 5 minutes) in preterm newborns (23-34 weeks' gestation). OBJECTIVE: The use of Apgar scoring for premature newborns remains widespread, despite controversy regarding its reliability as a measure of morbidity and mortality in the neonatal period. DESIGN: A cohort of 852 preterm newborns born during a 34-month period between 1984 and 1987 was studied. Newborns were stratified into 2 groups by gestational age (23-28 weeks and 29-34 weeks), and data were analyzed, controlling for gestationa

Low Apgar scores and birth asphyxia. Misconceptions that promote undeserved negligence suits.

The temptation to specify cause-and-effect relationships in cases of neurologic handicap has created a number of misconceptions that have stimulated irresponsible malpractice litigation focusing on childbirth. Paradoxically, the number of such lawsuits is on the increase, even though obstetric and neonatal care has improved dramatically since Virginia Apgar introduced her scoring system in 1953. Nowadays, no evidence of intrapartum hypoxia is found in the majority of newborns who require resuscitation. Consequently, the value of using the Apgar score as an index for assessing asphyxia should b

Correlation of the one-minute Apgar score and the pH value of umbilical arterial blood.

The one-minute Apgar score, proven useful for rapid assessment of the neonate, is often poorly correlated with other indicators of intrauterine well-being. Fetal asphyxia is directly associated with neonatal acidosis. A low Apgar score in the face of normal pH and base deficit does not, therefore, indicate an asphyxiated infant. We performed a study at Vanderbilt University Hospital to ascertain the feasibility of combining the pH value of umbilical arterial blood with the one-minute Apgar score for neonatal assessment. The pH values and Apgar scores were obtained on 172 singleton neonates. Wh

Apgar score and neonatal mortality in China: an observational study from a national surveillance system.

BACKGROUND: To examine the association between the Apgar score and neonatal mortality over gestational age in China and to explore whether this association changed when Apgar scores were combined at 1 and 5 min. METHODS: Data for all singleton live births collected from 438 hospitals between 2012 and 2016 were used in this study. Poisson regression with a robust variance estimator adjusted for a complete set of confounders was used to describe the strength of the association between the Apgar score and neonatal mortality. RESULTS: The relative risks of neonatal death-associated intermediate
