[Current status of aphasia therapy].

Aphasia therapy in adults has been established to a larger extent relatively lately in the history of aphasiology, i.e. after its social medical importance had been realized and one of the cardinal problems of neurology solved more satisfactorily--lesion localization by imaging techniques. In order to evaluate the efficiency of aphasia therapy--which is still not quite uncontradicted--it was necessary to acquire sufficient knowledge of the spontaneous recovery process. It takes place--e.g. after stroke--mainly during the first 3 months, coming, as a rule, to a halt during the first year. Longe

Current concepts in adult aphasia.

This paper provides a review of recent research from the areas of speech and language pathology, cognitive psychology, psycholinguistics, neurology, and rehabilitation medicine which is used to refine and extend current definitions of aphasia. Evidence is presented from these diverse disciplines, which supports a multimodality, performance-based, verbal and non-verbal, cortical and subcortical, and cognitively multidimensional view of aphasia. A summary of current practice in the assessment and treatment of adult aphasia is summarized.

Clinical Effectiveness of the Queen Square Intensive Comprehensive Aphasia Service for Patients With Poststroke Aphasia.

BACKGROUND AND PURPOSE: Poststroke aphasia has a major impact on peoples’ quality of life. Speech and language therapy interventions work, especially in high doses, but these doses are rarely achieved outside of research studies. Intensive Comprehensive Aphasia Programs (ICAPs) are an option to deliver high doses of therapy to people with aphasia over a short period of time. METHODS: Forty-six people with aphasia in the chronic stage poststroke completed the ICAP over a 3-week period, attending for 15 days and averaging 6 hours of therapy per day. Outcome measures included the Comprehensive A

[Crossed aphasia in a dextral patient--a case of sensory aphasia due to a right hemispheric lesion].

An extremely rare case of sensory aphasia due to a right hemispheric lesion in a 61-year-old, right-handed male is reported. He manifested jargon aphasia, constructional apraxia and ideational apraxia after an operation for right temporo-parietal cerebral hemorrhage. Standard language test of aphasia (SLTA) administered 6 months after the onset revealed disturbance of verbal comprehension and repetition, fluent jargon with many paraphasias and paragraphias. The brain CT, MRI and IMP-SPECT disclosed right temporo-parietal and angular lesions. There are two other reported cases of right posterio

Shifting of global aphasia to Wernicke's aphasia in a patient with intact motor function: a case report.

BACKGROUND: Global aphasia without hemiparesis (GAWH) is a rare stroke syndrome characterized by the dissociation of motor and language functions. Here, we present a case of GAWH with the patient later regaining speech fluency. CASE PRESENTATION: A 73-year-old man was admitted to our emergency department immediately after an episode of syncope. On arrival, we noted his global aphasia but without any focal neurologic signs. Computed tomography (CT) perfusion scans showed a large hypodense region over his left perisylvian area. Under the impression of acute ischaemic stroke, he received recombi

Ideomotor apraxia without aphasia and aphasia without apraxia: the anatomical support for a double dissociation.

This study aimed to verify the existence of a double aphasia/apraxia dissociation. Apraxic patients without aphasia and aphasic patients without apraxia were included in a consecutive series of patients with cortical or subcortical CT documented vascular lesions in the left hemisphere. Of 699 patients, 10 were found to be apraxic but not aphasic and 149 aphasic but not apraxic. These findings indicate an aphasia/apraxia double dissociation. This suggests that praxis and language make use of two different, partly overlapping networks.

[Aphasia: a neuronal network disorder].

Language processing requires the coordinated interaction of local and distant neural populations within distributed networks of the temporal, frontal and parietal brain regions. Poststroke aphasia is the consequence of both local as well as remote dysfunction within language-specific and domain-general networks. Language recovery, in turn, rests on reorganization processes within these networks. These comprise the resolution of an acute network failure (i. e. diaschisis), the subacute activation of right hemisphere homologous regions and the gradual reintegration of left hemisphere remote and

[Aphasia: evidence-based therapy approaches].

Speech and language therapy is essential in the rehabilitation of aphasic disorders following a stroke. Due to the predicted increase of aphasia and limited resources within the healthcare system, the development of efficient and sustainable treatment methods is of exceptional importance. The effectiveness of both traditional and innovative approaches needs to be evaluated against the standards of evidence-based medicine. Class I evidence has been established for high-intensity speech and language therapy in subacute and chronic stages of aphasia. Innovative training-based approaches have so f

Progressive aphasia with Lewy bodies.

Dementia with Lewy bodies (DLB) may include both Alzheimer and Lewy body pathology, but has never been reported to cause primary progressive aphasia. We report a 69-year-old woman who died 11 years after presenting with the syndrome of progressive aphasia. Six years after aphasia onset she developed visual hallucinations, and subsequently parkinsonism. Autopsy examination revealed Alzheimer's disease (AD), cortical Lewy bodies, and depigmentation and Lewy bodies in the substantia nigra and locus ceruleus. The aphasia most likely reflected the initial onset of AD, and the psychosis and parkinso

Hyperemic receptive aphasia on neuroSPECT.

NeuroSPECT of regional cerebral blood flow (rCBF) with Tc-99m HMPAO demonstrated left temporoparietal hyperemia in two patients with acute receptive aphasia. This finding prompted further testing with electroencephalography that added to the impression of ictal dysphasia. The differential diagnosis in one case included complicated migraine. NeuroSPECT depicts blood flow abnormalities in acute aphasic disorders, either due to ischemia, which is most commonly the cause, or due to hyperemia secondary to migraine or epilepsy. The treatment and prognosis of these latter conditions differ from strok

Phonological transformations in conduction aphasia.

Different explanations and subtypes of conduction aphasia are analyzed. Characteristics of literal paraphasias in parietal-insular conduction aphasia are discussed, emphasizing that paraphasias in conduction aphasia are articulatory-based (articulatory literal paraphasias) and due mainly to phoneme substitutions and phoneme deletions; they result basically in switches in phoneme manner and place of articulation. Similarities between errors in ideomotor apraxia and conduction aphasia language deficits are presented. It is proposed that language deviations (in oral as in written language) in con

Aphasia treatment delivered by telephone.

Telephone delivery of speech and language treatment programs may be an effective supplement to the more traditional face-to-face delivery. This report describes the response of a nonfluent aphasic patient who, at 11 years postonset, was treated over the telephone using a hierarchically structured syntax treatment program. Examination of pre- and posttreatment performance on the Northwestern Syntax Screening Test and the Boston Diagnostic Aphasia Examination Cookie Theft Description showed improvement on both dependent variables. The results support the use of telephone therapy with patients wh

Compensatory articulation in Broca's aphasia.

Thirteen patients clinically diagnosed as Broca's aphasics, were administered a vowel bite block task to ascertain their on-line compensatory articulation ability. Formant frequencies for F1 and F2 were obtained from a pitch-synchronous spectral analysis of digitized vowel waveforms. Bite block/i/productions were consistently characterized by higher F1 and lower F2 frequencies compared to normal vowel productions. Undershooting of tongue elevation and fronting explains this result. No consistent pattern of "off-target" productions were found for/a/tokens. A multiple correlation analysis among

Localization in transcortical sensory aphasia.

Transcortical sensory aphasia is a syndrome characterized by poor comprehension but excellent repetition. The lesions shown on computed tomography and isotope scans of 15 patients who satisfied the objective criteria based on test scores were studied. The overlap technique showed a unique posterior parieto-occipital location of lesions. The lesions seem to separate into two groups: one is more medial, inferior, and posterior and is clearly in the posterior cerebral artery territory, and the other is relatively more lateral, superior, and anterior and seems to be in a watershed area between mid

Closed head trauma and aphasia.

A prospective study has been done on the relationship between closed head trauma and aphasia. The most frequent type of aphasia seen after closed head injury is an anomic aphasia. This aphasia is often associated with other defects of higher cortical function. The second most common type of aphasia is a Wernicke's aphasia. Other types of aphasia were not seen in this study. The areas of the head which when injured produce aphasia are the right orbitofrontal region and the left temporoparietal region. The prognosis for recovery appeared highly variable.
