This case report discusses the surgical management of an apical periodontal cyst in a 14-year-old male patient presenting with pain in the upper anterior jaw. The patient had a history of trauma to the upper anterior teeth, leading to the development of a radicular cyst involving teeth 11 and 12. The treatment plan included initial root canal therapy followed by surgical enucleation of the cyst, apicoectomy, and retrograde filling of the affected teeth. Titanium-activated platelet-rich fibrin (T-PRF) membranes were utilized along with an osseo-bone graft to promote healing and bone regeneratio

BACKGROUND AND OVERVIEW: In this case report, the authors aim to review the standard protocols for endodontic treatment of autotransplanted molars and report on the 5-year follow-up of a clinical transplantation case.
CASE DESCRIPTION: The authors transplanted a mandibular third molar into the position of the adjacent second molar. The authors performed extraoral apicoectomy and retrograde restoration after atraumatic extraction of the donor tooth and before transplantation. The authors did not perform conventional orthograde root canal treatment after transplantation. The patient was followe

This article describes the orthodontic relapse with mandibular incisor fenestration in a 36-year-old man who had undergone orthodontic treatment 21 years previously. The patient reported that his mandibular 3 × 3 bonded retainer had been partially debonded and broken 4 years earlier. The mandibular left lateral incisor remained bonded to the retainer and received the entire load of the incisors; consequently, there was extreme labial movement of the root, resulting in dental avulsion. As part of the treatment, the root was repositioned lingually using a titanium-molybdenum segmented archwire f

Among a variety of biomaterials that have been reported to be ideal for dental repair, calcium hydroxide has been shown to have excellent long-term biocompatibility in the pulp and periapical areas. Here we report an alternative method employing calcium hydroxide for periapical surgery in a patient who developed internal apical resorption after traumatic injury, which negatively affected the quality of the cleaning, shaping and filling of the root canal. Obturation of the root canal as far as the middle third was followed by apicoectomy and ultrasonic retropreparation, and then retrograde root

The aim of this study was to determine whether microorganisms invade periapical cementum of human teeth from the adjacent periapical lesions. We therefore attempted to isolate microorganisms from periapical cementum through the adoption of standard anaerobic procedures for obligate anaerobes. Samples of cementum were taken from 10 amputated tooth roots at the time of apicoectomy. From two of these samples, bacteria were recovered after anaerobic incubation, but no bacteria were recovered after aerobic incubation of the same samples. Of a total of eight isolates from the cementum, seven were ob

Persistent and chronic pain is more common in the head and neck region than in any other part of the body; therefore, dentists are more likely to encounter these rather complex cases in their practices. In this report, we present a brief review of neuropathic orofacial pain (NOP) that can begin after deafferentation of trigeminal nerve fibers after root canal treatment, apicoectomy, tooth extraction, or implant placement or idiopathically. A case of NOP occurring after a surgical implant procedure in a 62-year-old woman is presented. Continuous pain started 6 months after the placement of 8 de

AIM: Endodontic microsurgery (EMS) of maxillary molars may represent a complex challenge to the clinician due to the location of the roots and the proximity of the maxillary sinus floor. This report aimed to describe the simultaneous use of a computer-assisted dynamic navigation (C-ADN) system and piezoelectric bony-window osteotomy for the transantral microsurgical approach of a maxillary left first molar with adequate root canal filling and symptomatic apical periodontitis.
SUMMARY: This case report highlights the importance of C-ADN to carry out a minimally invasive buccal surgical access

OBJECTIVE: The aim of this study was to evaluate the therapeutic effect of ND:YAP laser in persistent lesions through signs and symptoms such as the presence of fistulas and pain, respectively.
BACKGROUND DATA: Periapical lesions with bacterial invasion, giving origin to bacterial infections, appear many times during the endodontic treatment, which aims at preparing and disinfecting the root canal system, in canals with mortified pulp. The endodontic treatment aim at eliminating infections and reinfection prevention; however, sometimes re-treatment is necessary, and it can be complemented wit

INTRODUCTION: The purpose of this case report is to present endodontic and periodontal management of mucosal fenestrations with exposed root apices. The treatment protocol in present cases includes a combination of regenerative therapy using platelet-rich fibrin (PRF) with connective tissue graft (CTG) and endodontic microsurgery. Pre-existing condition of these teeth exhibits apical lesion with prominent root position and complete buccal bone dehiscence/fenestration presents a true challenge to successful outcome.
CASE PRESENTATION: Four patients having concomitant mucosal fenestrations with

One of the most important aspects in surgery is the healing process after the periapical surgery. Past studies have shown occasional encounters with vital root resection and have noted varying degrees of pulpal response after root resection in periodontal disease. The purpose of this investigation was to observe the pulpal and periapical responses to intentional apical vital root transection in one root of multirooted teeth of German-Canadian dogs over a 6-month postoperative period. This is an experimental study performed on left maxillary and mandibular quadrants of four adult German-Canadia

Actinomycosis is an insidious infection caused by the facultative anaerobic Gram-positive bacterium Actinomyces, commonly found in the oral microbiome. Among its manifestations, periapical actinomycosis stands out as a rare subtype within cervicofacial actinomycosis, speculated to play a role in the persistence of periapical radiolucencies following root canal therapy. Instances of this occurrence often arise from disruptions in epithelial continuity, stemming from surgical procedures, trauma, or prior infections, paving the way for deep microbial infiltration. A 35-year-old woman presented wi

Clinicians do not frequently see impacted dilacerated maxillary incisors in their patients. When they do, there are several diagnostic and management challenges for correcting root dilacerations. An unfavorable esthetic outcome might occur as a result of soft-tissue complications during surgical eruption procedures. We present 2 patients with an impacted and dilacerated maxillary central incisor. Computed tomography scans with 3-dimensional reformation were used to accurately assess the positions of the dilacerated teeth, the degree of dilaceration, and the stage of root formation. The therapy

Crown-root fractures in permanent teeth cause esthetic and functional problems. This paper reports the case of a complicated crown-root fracture in the maxillary right central incisor of a young patient who was treated with a multidisciplinary approach in two phases. A modified Widman flap, root canal therapy, glass fiber post cementation, and adhesive tooth fragment reattachment were performed shortly after an accident. Satisfactory esthetic and functional outcomes were obtained. However, the patient did not attend follow-up visits and returned after 7 years. During this second phase, the cli

Primary intraosseous salivary gland tumors are rare, with mucopidermoid carcinoma being the most frequent histotype. The authors present a case of adenoid cystic carcinoma, located in the mandibular incisor region, associated with pain. Endodontic treatment resulted in increased pain and progressive mandibular expansion. An apicoectomy was conducted, and an intraosseous adenoid cystic carcinoma was diagnosed at histological examination. The patient was treated by wide surgical resection, and is alive and well without recurrences or distant metastases 14 yr after the original diagnosis. The cas

INTRODUCTION: This case report presents the successful multidisciplinary treatment of a maxillary lateral incisor with a deep and narrow recession involving the root apex associated with pulpal necrosis. The tooth initially considered with a hopeless prognosis, was treated with a combined periodontal plastic, regenerative, and endodontic surgical procedure.
CASE PRESENTATION: A recently reported technique, the laterally stretched (LAST) flap with a connective tissue graft (CTG) technique for root coverage, was combined with enamel matrix derivative (EMD) for periodontal regeneration of the pe

Autogenous tooth transplantation is an alternative treatment for preserving continuity of the dental arch following early loss of teeth. The prognosis of transplants determines the indication as compared with other methods of space closure. Transplanted buds of upper premolars as well as of lower third molars continue their root growth. The clinical and roentgenological analysis of 81 autogenously transplanted upper canines, following different operative procedures, indicates, that 50% of the transplants remain in situ for five years and only 25% remain in situ longer than six years.
