An Update on the Diagnosis, Grading, and Staging of Appendiceal Mucinous Neoplasms.

Despite advances in our understanding of appendiceal mucinous neoplasms and their relationship to the pseudomyxoma peritonei syndrome, the classification of mucinous tumors of the appendix is still confusing. This review will provide an update on the various classification systems that have been recently proposed for appendiceal mucinous neoplasia, with a particular emphasis on how to handle and report the histologic findings for these tumors using the newly published Peritoneal Surface Oncology Group International (PSOGI) and American Joint Committee on Cancer (AJCC) eighth edition guidelines

Low-grade appendiceal mucinous neoplasm and endometriosis of the appendix.

BACKGROUND: A distended, mucous-filled appendix is known as an appendiceal mucocele. They are a rare form of an appendiceal mass and develop from both benign and malignant processes. Mucoceles can develop secondarily to an obstruction, such as from a fecalith, scarring or, rarely, endometriosis. Only 12 cases of non-neoplastic appendiceal mucoceles caused by endometriosis have been previously described. The association between neoplastic appendiceal mucoceles in the presence of endometriosis is described for the first time in this report. CASE PRESENTATION: A 57-year-old woman presented with

Clinicopathological spectrums and prognosis of primary appendiceal adenocarcinoma, goblet cell adenocarcinoma, and low-grade appendiceal mucinous neoplasms.

Primary appendiceal adenocarcinoma (APCA), goblet cell adenocarcinoma (GCA), and low/high-grade appendiceal mucinous neoplasms (LAMN/HAMN) are distinct entities with overlapping clinical presentation and histomorphology, leading to diagnostic challenges. We retrospectively reviewed our archived cases between 2010 and 2018 for diagnosis reappraisal and comparative analysis using updated terminology and modern parameters. A total of 87 cases (22 APCA, 40 GCA, and 25 LAMN pT≥3) were included. The entire cohort had 49 women and 38 men with a median age of 59.9 (range 26-88) years. There were no st

High-grade appendiceal mucinous neoplasm: case report.

High-grade appendiceal mucinous neoplasm is an extremely rare gastrointestinal tumour with nonspecific clinical manifestations that complicate preoperative diagnosis. In women, it may mimic gynecological diseases, particularly ovarian tumours, leading to diagnostic delays and inappropriate surgical approaches. We report the case of a 68-year-old woman from the San Martín region of Peru who presented with abdominal pain, progressive distension, and abnormal vaginal bleeding. Laboratory tests showed elevated carcinoembryonic antigen levels, while transvaginal ultrasound revealed a right adnexal

Clinicopathological Profile and Management of Appendiceal Neoplasm: An Observational Study.

INTRODUCTION: Appendiceal neoplasms are rare and frequently diagnosed incidentally during histopathological examination of appendectomy specimens. Recent data suggest that their incidence is increasing worldwide. This study aimed to evaluate the appendiceal neoplasm in appendectomy specimens, determine their clinicopathological presentation, histological subtypes, diagnostic modalities used, and the outcomes at the Tribhuvan University Teaching Hospital, Kathmandu, Nepal. METHODS: A retrospective evaluation of patients diagnosed with appendiceal neoplasm was conducted from January 2013 to Dec

An Uncommon Confluence: Splenic Lymphangioma and Appendiceal Mucinous Neoplasm.

Splenic lymphangiomas, per se, are exceedingly rare, benign congenital malformations of lymphatic vessels, typically diagnosed in childhood or incidentally in adulthood. Appendiceal mucinous neoplasms (AMNs) are also infrequent, often asymptomatic, and usually discovered incidentally. To date, the co-occurrence of these two distinct primary lesions in the same patient has not been previously reported. We present the case of an 83-year-old Turkish female who underwent emergent abdominal surgery for an acute abdomen secondary to a strangulated left diaphragmatic herniation. Multiple perforations

Surgical management of appendiceal mucinous neoplasm: Is appendectomy sufficient?

BACKGROUND AND OBJECTIVES: Due to the rarity of appendiceal mucinous neoplasms (AMNs), there are few established treatment guidelines. The clinical course varies from incidental detection to progressive spread with pseudomyxoma peritonei (PMP). This study investigated the extent of resection on the prognosis and outcomes of AMNs. METHODS: This multicenter retrospective study evaluated patients with AMN who underwent surgery between 4/2006 to 9/2017. Primary endpoints included overall survival (OS) and disease-specific survival (DSS). Secondary endpoints included PMP incidence and treatment wi

Appendiceal neoplasm incidence and mortality rates are on the rise in Australia.

OBJECTIVES: The study aimed to examine the incidence and mortality rates of appendiceal neoplasms (ANs) in Australia. METHODS: A retrospective analysis was performed on national data obtained from the Australian Institute of Health and Welfare (AIHW) from 1982 to 2013. Changes to the incidence, and the cancer-specific mortality following the diagnosis of ANs were analyzed over this time period, with stratification performed for histological subtype, gender, and age groups (<50y and ≥50y). RESULTS: Incidence and mortality rates of ANs increased significantly across both genders and age groups

Mutation profile of high-grade appendiceal mucinous neoplasm.

AIMS: High-grade appendiceal mucinous neoplasm (HAMN) was recently proposed as a disease entity histologically analogous to low-grade appendiceal mucinous neoplasm (LAMN), but characterised by high-grade cytological atypia. The pathogenesis and clinical features of HAMN have not been fully elucidated. METHODS AND RESULTS: Nine cases of HAMN, eight LAMN, 10 appendiceal mucinous adenocarcinomas (MACA) and five appendiceal serrated polyps resected between 2008 and 2017 contributed by three medical centres underwent targeted next-generation sequencing of 50 cancer-related genes. [Fourteen Cases of Low-Grade Appendiceal Mucinous Neoplasm].

Low-grade appendiceal mucinous neoplasm(LAMN)is a rare disease. In consideration of consistency with the 2010WHO Classification, LAMNwas classified in the Japanese Classification of Colorectal Carcinoma 8th edition. The present study involved 14 patients with LAMNover a 5-year period. The preoperative diagnosis was made as mucocele of the appendix in 9 cases. Operative interventions included ileocecal resection in 6 cases(laparoscopic; 3 cases)and appendectomy in 8 cases (laparoscopic; 6 cases). Histopathologically, there was no LNpositive case. While adjuvant therapies were not administered i

Low-grade appendiceal neoplasm presenting as a volvulus of the cecum.

Mucocele of the appendix is an uncommon disorder that is often asymptomatic, but can present similarly to acute appendicitis. Timely diagnosis and treatment is imperative due to the many complications that can result from the mucocele, such as perforation. Appendiceal mucoceles (AM) were previously thought to be either benign or malignant; however, a different pathological classification of AM is currently favored. Also, only a few cases of volvulus of a benign AM have been reported. Here, we present the first reported case of a low-grade appendiceal mucinous neoplasm resulting in a volvulus o

Dysplastic intestinal-type metaplasia of appendiceal endometriosis: a mimic of low grade appendiceal mucinous neoplasm.

UNLABELLED: We report an example of dysplastic intestinal-type metaplasia involving endometriosis of the appendix in a 45 year old woman. One other example of this phenomenon has been reported. As it occurs within the muscular wall of the appendix, confusion with low grade appendiceal mucinous neoplasm (LAMN) may occur. Evidence supporting the metaplastic nature of the intestinal epithelium is offered. As the initial pathological diagnosis was of invasive cancer with perforation of the appendix treatment consisted of peritonectomy and hyperthermic intraperitoneal chemotherapy (HIPEC). VIRTUAL

Mucinous appendiceal neoplasms: Report of a case and brief literature review.

INTRODUCTION: Appendiceal tumors are rare neoplasms detected in about 2 % of appendicectomies. The clinical presentation is often unspecific, varying from unspecific abdominal pain or presenting as an acute appendicitis or being asymptomatic. CASE PRESENTATION: We present a case of a patient presenting as an acute appendicitis with a mucocele, and then classified as HAMN. The patient was treated with initial laparoscopic approach and then conversion in laparotomy with appendectomy. Histology demonstrated a high grade appendiceal mucinous neoplasm limited to submucosa (pT3), with concomitant a

Treatment Variation and Long-Term Outcomes of Low-Grade Appendiceal Neoplasms.

BACKGROUND: Heterogenous nomenclature describing appendiceal neoplasms has added to uncertainty around their appropriate treatment. Although a recent consensus has established the term low-grade appendiceal neoplasm (LAMN), we hypothesize that significant variation remains in the treatment of LAMNs. METHODS: We retrospectively reviewed our prospectively maintained appendiceal registry, identifying patients with LAMNs from 2009 to 2019. We assessed variability in treatment, including whether patients underwent colectomy, spread of disease at presentation, and long-term outcomes. RESULTS: Of 1

Intraoperative predictors of appendiceal abnormalities in patients with mucinous ovarian neoplasms.

OBJECTIVE: To evaluate intraoperative factors predicting appendiceal pathology during gynecologic oncology surgery for suspected mucinous ovarian neoplasms. METHODS: We conducted a retrospective study on 225 patients with mucinous ovarian neoplasms who underwent surgery for an adnexal mass with concurrent appendectomy between 2000 and 2018. Regression analyses were used to evaluate intraoperative factors, such as frozen section of the ovarian mass and surgeon's impression of the appendix in predicting appendiceal pathology. RESULTS: Most patients (77.8%) had a normal appendix on final pathol
