Bone remineralization of lytic lesions in multiple myeloma - The Arkansas experience.

Multiple myeloma (MM) patients frequently present with extensive osteolytic bone lesions. However, the impact of myeloma treatment on focal lytic lesion remineralization has not been extensively studied. In this study, the effect of anti-myeloma treatment on the extent of bone remineralization was examined and potential mediators identified. Newly diagnosed MM patients enrolled in the Total Therapy 4 and 5 (TT4; n = 231, TT5; n = 64) protocols were longitudinally evaluated for changes in radiological parameters for a median of 6.1 years. Bone remineralization was defined as a sclerotic CT chan

Trends in prevalence and spatiotemporal distribution of gastroschisis in Arkansas, 1998-2015.

BACKGROUND: Arkansas (AR) had the highest prevalence of gastroschisis in a recent study including 15 U.S. states. Our objective was to evaluate trends in prevalence and the spatiotemporal distribution of gastroschisis in AR. METHODS: Infants with gastroschisis, born 1998-2015, were identified from the Arkansas Reproductive Health Monitoring System. Birth record data were used as denominators for calculations. Maternal residence at delivery was geocoded for spatial analyses. Annual prevalence rates (PRs) were calculated. Joinpoint regression analysis was performed to examine trends in gastrosc

Case of the month. A case report of human pulmonary dirofilariasis in Arkansas.

Our patient, a resident of Arkansas, presented with a solitary pulmonary nodule for lung resection to rule out primary lung cancer. However, pathologic examination showed the lung nodule to be a granuloma secondary to dirofilariasis. Human pulmonary dirofilariasis is a rare zoonotic disease caused by Dirofilariasis immitis (dog heartworm) that is usually transmitted from dogs to human by mosquito bite. To our knowledge, human pulmonary dirofilariasis has previously not been reported in Arkansas. In addition, awareness of this entity in the appropriate clinical and epidemiologic setting is impo

Transmission of Mycobacterium tuberculosis in a rural community, Arkansas, 1945-2000.

A cluster of tuberculosis cases in a rural community in Arkansas persisted from 1991 to 1999. The cluster had 13 members, 11 linked epidemiologically. Old records identified 24 additional patients for 40 linked cases during a 54-year period. Residents of this neighborhood represent a population at high risk who should be considered for tuberculin testing and treatment for latent tuberculosis infection.

Treatment of depression in rural Arkansas: policy implications for improving care.

Policy-makers have long suspected that greater barriers to care result in depressed rural residents being less likely to receive high-quality treatment. This study recruited 470 depressed community residents in a 1992 telephone survey, followed 95 percent of them through one year, and abstracted additional data on their health care utilization from insurance claims, medical and pharmacy records. Bivariate and multivariate models demonstrated that during the year following the baseline, there were no significant rural-urban differences in the rate (probability of any outpatient depression treat

Black/white life expectancy differences and sociodemographics: Arkansas and the U.S.

Life expectancy data by race and gender for Arkansas show whites having longer average life expectancies at birth than blacks. Racial differences in life expectancy have been found to be due to higher mortality rates for blacks due to their tendency to fall into high-risk groups. Cause-specific mortality rates by race and gender for Arkansans 25 years and older were calculated for the ten leading causes of death in the state. Blacks in Arkansas had higher mortality rates than whites for each major cause of death except pulmonary diseases and suicide.

An outbreak of histoplasmosis at an Arkansas courthouse with five cases of probable reinfection.

An outbreak of histoplasmosis occurred at a county courthouse in Arkansas in July, 1975. Fifty cases were identified by symptoms and positive complement fixation (CF) tests and 18 others by the clinical findings alone. Only three positive CF tests (greater than or equal to 1:8) were found in a control group of 55 unexposed persons of the same age. The attack rate for 84 courthouse employees was 52%. Histoplasma capsulatum was isolated from the sputum of two cases, and from bird droppings which had been dumped from the roof of the courthouse two weeks before the epidemic's peak. Window air cond

Vaccine policy and Arkansas childhood immunization exemptions: a multi-year review.

BACKGROUND: An increase in total vaccine exemptions (medical, philosophic, and religious) occurred in Arkansas after a 2003 legislation added a philosophic category and used a new process for vaccine exemptions. By legislative requirement, the Arkansas Department of Health monitored exemptions through the 2009-2010 school year. PURPOSE: The goal of the study was to determine the prevalence of vaccine exemption in 2003-2010 compared to the number of requests prior to the legislation enacted in 2003. METHODS: Exemptions were calculated by school-age category using raw numbers of exemptions, to

Medicaid Expansion And Health Services Use For Adults Experiencing Homelessness In Arkansas.

More than 500,000 people in the US experience homelessness at any given time, many of whom now qualify for Medicaid in states that expanded coverage under the Affordable Care Act (ACA). In this article we use a novel data set from Arkansas to provide the first estimates of the association between gaining coverage through the ACA's Medicaid expansion and health services use for a population experiencing homelessness. We find that Medicaid expansion was associated with large initial increases in inpatient hospitalizations and emergency department visits-which declined steadily over time-among ad

Screening for Opioid Use Disorder in the Largest Jail in Arkansas: A Brief Report.

Individuals who have experienced incarceration have an increased risk of both fatal and nonfatal overdose. Given the increases in illicit opioid use in Arkansas and across the South, many individuals with opioid use disorder (OUD) are likely to encounter the criminal justice system, particularly county jails. However, there are currently no published data on OUD among entrants into county jails in the South. This study presents data from an OUD screening project initiated by the Pulaski County Regional Detention Facility, the largest county jail in the state of Arkansas. As part of the regular

Mumps in a highly vaccinated Marshallese community in Arkansas, USA: an outbreak report.

BACKGROUND: During 2000-15, Arkansas Department of Health, Little Rock, AR, USA, investigated between one and six cases of mumps each year. From Aug 5, 2016, to Aug 5, 2017, the department received notification of more than 4000 suspected mumps cases in the second largest outbreak in the USA in the past 30 years. METHODS: Arkansas Department of Health investigated all reported cases of mumps to ascertain exposure, travel, and vaccination histories and identify close contacts. Cases were classified as confirmed if the patient had laboratory confirmation of mumps virus or probable if they had c

Epidemiologic Trends of Cutaneous T-Cell Lymphoma in Arkansas Reveals Demographic Disparities.

Accurate demographic data are critical for comprehending and treating cutaneous T-cell lymphoma (CTCL). Our research aimed to determine the demographics and incidence trends of CTCL patients in Arkansas compared to those of the national CTCL population to recognize the underlying disparities. We collected data from 143 CTCL patients at the University of Arkansas for Medical Sciences (UAMS) and national CTCL patient data from the Surveillance, Epidemiology, and End Results (SEER) database. Our analysis revealed that males are affected more than females across all ages and races. CTCL incidence

Cluster of Cryptococcus neoformans Infections in Intensive Care Unit, Arkansas, USA, 2013.

We investigated an unusual cluster of 6 patients with Cryptococcus neoformans infection at a community hospital in Arkansas during April–December 2013, to determine source of infection. Four patients had bloodstream infection and 2 had respiratory infection; 3 infections occurred within a 10-day period. Five patients had been admitted to the intensive care unit (ICU) with diagnoses other than cryptococcosis; none had HIV infection, and 1 patient had a history of organ transplantation. We then conducted a retrospective cohort study of all patients admitted to the ICU during April–December 2013

The stoneflies (Plecoptera) of Arkansas: a checklist compiled from museum specimen data.

BACKGROUND: Stoneflies are well known as indicators of water quality. Their presence in running waters, glacial meltwaters, and large oligotrophic lakes is rapidly declining the world over. In the USA, states partner with the U.S. Fish and Wildlife Service to protect habitat and wildlife through the development of State Wildlife Action Plans (SWAPs). Plants and wildlife species often enter these SWAPs as Species in Greatest Conservation Need (SGCN). Arkansas currently lists nine stonefly species as SGCNs and has funded research on them through SWAP grants. However, these nine species were init

Leveraging the Prescription Drug Monitoring Program to Curb Opioid Prescribing in Arkansas.

Effective means of accurately identifying problematic opioid prescribing are needed. Using an iterative approach with the Arkansas State Medical Board Pain Subcommittee, we modified existing opioid prescriber criteria to create seven metrics to be deployed in Arkansas. These included metrics of dose and days' supply, concomitant use of opioid and benzodiazepines, solid dosage units, and numbers of opioid patients and certain opioid prescriptions. Two of these metrics (average MME daily dose per prescription and total oxycodone 30 mg or hydromorphone prescriptions) were weighted by 2, creating
