Incidence, case-fatality rate and clinical pattern of firearm injuries in two cities where arm owning is forbidden.

OBJECTIVE: To review firearm injuries in five hospitals of Douala and Yaounde, both towns of Cameroon, a Sub-Saharan African country where ownership of firearm is forbidden. METHODS: This was a retrospective investigation carried out within the period January 1998 to December 2002. Records of the mortuaries, the emergency departments, the intensive care units, the operating rooms and the surgical units were all analysed to identify any injury caused by firearm. Our gold standard was any individual with a clear diagnosis of firearm injury, as determined by a medical doctor. RESULTS: 286 firea

Salvage of functional elbow range of motion in complex open injuries using a sensate transposition lateral arm flap.

Complex open posterior elbow injuries pose three principal challenges to the reconstructive surgeon. First, the surgeon must provide stable soft-tissue closure over the joint/skeletal reconstruction. Second, the coverage must be thin and supple and promote the free gliding of the underlying structures. Finally, secondary and tertiary procedures must be anticipated beneath the flap, because a stiff, scarred, and adherent flap will only compromise these procedures. The results of 10 consecutive fasciocutaneous transposition lateral arm flaps for coverage of posterior elbow wounds are reported. T

The platform transfer splint: 2 case reports of a mobility aide for persons with arm injuries or conditions.

Limited or impaired mobility is a major obstacle to maximizing length of stay efficiency for inpatient rehabilitation. Trauma patients and others with multiple limb impairments present a mobility challenge to all rehabilitation centers. Of particular concern are patients with forearm fractures who are nonambulatory. With shorter inpatient stays, patients are being discharged home or to other settings with continued weight-bearing restrictions. These patients put great demands on their caregivers as a result of their limited mobility. The Platform Transfer Splint (PTS) has been developed to ove

The use of Disabilities of the Arm, Shoulder, and Hand Questionnaire in rehabilitation after acute traumatic hand injuries.

This longitudinal observational prospective study evaluates the use of a self-reported measure Disabilities of the Arm, Shoulder, and Hand (DASH) in assessing traumatic hand injury patients. A total of 146 subjects were invited to fill out a validated questionnaire (DASH) during their first consultation and at discharge from therapy. The findings revealed a significant improvement in DASH scores upon discharge from therapy. The use of Quick DASH, an 11-question shortened version of the full 30-question DASH, also generated similar results with the psychometric properties maintained. Despite th

[Shoulder and arm injuries in alpine ski racing. A survey on the frequency of these injuries with athletes of the Swiss Ski cadres and the regional cadres].

The aim of this work was to examine the frequency and pattern of injuries of the upper extremity among Swiss Ski athletes and among regional team athletes. The injury pattern regarding shoulder and arm injuries in alpine skiers in Switzerland correlates with the previous literature. The shoulder joint is most frequently injured, followed by injuries of the fingers, particularly the thumb. Regarding injury frequency no difference between ladies and men was seen. On the other hand, an increased injury rate could be seen in regional team drivers. Most injuries happened for both genders and in all

A Modified Single-Armed Suture Technique for Traumatic Cyclodialysis Cleft with Vitreoretinal Injury.

Our aim was to assess the therapeutic efficacy of a modified single-arm suture technique on traumatic cyclodialysis cleft with vitreoretinal injury. The procedure involved fixing a detached ciliary body using a single-armed 10-0 polypropylene suture under the assistance of a 29-gauge needle. Patients with a traumatic cyclodialysis cleft combined with an anterior and posterior segment injury who underwent modified internal cyclopexy together with vitreoretinal surgery were enrolled in this study. Ultrasound biomicroscopy (UBM) was used to diagnose and evaluate the cyclodialysis and anterior seg

Mechanical design of a distal arm exoskeleton for stroke and spinal cord injury rehabilitation.

Robotic rehabilitation has gained significant traction in recent years, due to the clinical demonstration of its efficacy in restoring function for upper extremity movements and locomotor skills, demonstrated primarily in stroke populations. In this paper, we present the design of MAHI Exo II, a robotic exoskeleton for the rehabilitation of upper extremity after stroke, spinal cord injury, or other brain injuries. The five degree-of-freedom robot enables elbow flexion-extension, forearm pronation-supination, wrist flexion-extension, and radial-ulnar deviation. The device offers several signifi

Missed posterior fracture-dislocation of the humeral head following an electrocution injury to the arm.

Posterior dislocation of the shoulder is a rare and commonly missed injury. While prompt diagnosis and treatment is important to prevent untoward sequelae, it is often diagnosed and treated too late. The first reported case of a posterior fracture-dislocation of the humeral head following a domestic electrocution accident in Singapore is described in a 52-year-old man. The injury was missed by several doctors before a humeral head replacement was done. The diagnostic pitfalls and management of this injury and ways to avoid a missed or delayed diagnosis are discussed.

[Tendon transfers to restore hand function following peripheral nerve injury in the arm].

Three patients, a woman aged 33 years, a man aged 42 and a woman aged 66, had a loss of hand function. This was due to denervation of a peripheral nerve, in essence the radial, ulnar and median nerves respectively. In the first two cases this was caused by trauma and in the third by compression neuropathy. The problems were loss of wrist and finger extension (a dropping hand) in the first, loss of intrinsic muscle function and thumb adduction in the second and loss of opposition in the third patient. Tendon transfers were performed by diverting a tendon insertion of a still functional muscle t

Incidence of acute injuries, active component, U.S. Armed Forces, 2008-2017.

Injuries have consistently ranked among the top morbidity burdens among U.S. military service members. This report describes the incidence, trends, types, causes, and dispositions of acute injuries among active component service members by anatomic region. From 2008 through 2017, there were more than 3.6 million acute incident injuries among more than 1.6 million individuals. The highest rates were for injuries to the foot/ankle, head/neck, and hand/wrist. Injury incidence decreased during the surveillance period for all anatomic sites except for the leg and knee. In addition, incidence varied

Illusion of arm movement evoked by tendon vibration in patients with spinal cord injury.

BACKGROUND: Studies in healthy people show that stimulation of muscle spindles through frequency-specific tendon vibration (TV) induces the illusory perception of movement. Following spinal cord injury (SCI), motor and sensory connections between the brain and parts of the body below-the-lesion level are partially or totally impaired. OBJECTIVE: The present investigation is a descriptive study aimed to investigate whether people living with SCI may experience movement illusions comparable to a control group. METHODS: Healthy and people with SCI were asked to report on three illusion-related

Reproducibility of noninvasive cardiac output during arm exercise in spinal cord injury.

The carbon dioxide (CO(2)) rebreathing method is a noninvasive technique to estimate cardiac output during exercise, but few data are available on the validity and reliability of this measure in individuals with spinal cord injury (SCI). Sixteen male subjects with SCI (mean age 45 +/- 9, seven paraplegic and nine tetraplegic) underwent three submaximal steady state arm ergometer exercise tests. We estimated cardiac output using the exponential CO(2) rebreathing technique at an individualized exercise intensity approximating 50% of peak oxygen uptake. Mean values for the cardiac output measurem

Guidance-based quantification of arm impairment following brain injury: a pilot study.

This paper reports the design and preliminary testing of a device for evaluating arm impairment after brain injury. The assisted rehabilitation and measurement (ARM) Guide is capable of mechanically guiding reaching and retrieval movements across the workspace and of measuring constraint forces and range of motion during guidance. We tested the device on four hemiplegic brain-injured individuals and four unimpaired control subjects. During guided movement, the brain-injured subjects generated distinct spatial patterns of constraint force with their impaired arms that were consistent with the s
