Glenohumeral instability: evaluation with MR arthrography.

Magnetic resonance arthrography is superior to other imaging techniques in evaluation of the glenohumeral joint. Normal variants that can be diagnostic pitfalls include the anterosuperior sublabral foramen, the Buford complex, and hyaline cartilage under the labrum. Anteroinferior dislocation is the most frequent cause of anterior glenohumeral instability and produces a constellation of lesions (anteroinferior labral tear, classic and osseous Bankart lesions, Hill-Sachs lesion). Variants of anteroinferior labral tears include anterior labroligamentous periosteal sleeve avulsion and glenoid lab

Contrast arthrography in symptomatic varus knees.

The authors carried out a study of contrast arthrography in 48 cases of primary genu varum, i.e. not secondary to trauma or to pathological conditions of the hip or foot. The patients all had symptoms and were classified according to the commonly used Trillat-Dejour method into four groups or stages depending on the degree of varus and the radiographic and arthrographic findings. Associated lesions secondary to the basic pathology were observed in all cases. These ranged from hypertrophic synovitis to torn medial or lateral menisci up to total compromise of the joint typical of stage 4 arthrit

Wrist arthrography in acute carpal injuries.

A prospective study of 10 patients with acute complex carpal dislocations was undertaken. Preoperative wrist arthrograms were done in these patients. We then observed the ligamentous tear through both volar and dorsal approaches at the time of open reduction. Perilunate dislocation was always an integral part of the pathology in all patients, except in one patient with radiocarpal dislocation. Communication between the radiocarpal and midcarpal joint was found in all patients, except in one patient with radiocarpal fracture-dislocation. Volar extrusion of the contrast material occurred in all

Facet joint arthrography in lumbar spondylolysis.

Facet joint arthrography was performed in 11 patients with lumbar spondylolysis. An abnormal communication between the two facet joints bordering the separated par interarticularis was observed in nine of these patients. This communication occurred through a channel in the area of the defect. In one patient with bilateral spondylolysis of the L5 vertebra, both left adjacent apophyseal joints were observed to communicate not only with one another, but also with the contralateral facet joints, through a transverse channel joining the isthmic areas of L5. Significant pain relief following intra-a

Comparison of subtraction arthrography, radionuclide arthrography and conventional plain radiography to assess loosening of total knee arthroplasty.

The value of plain radiographs, digital subtraction arthrography and radionuclide arthrography was analysed in 23 cases of failed total knee arthroplasty. The preoperative diagnosis was compared with the intraoperative assessment. Sensitivity, specificity and the positive and negative predictive value for assessing a loose component were determined separately for the femoral and tibial components. At revision we found 13 loose femoral and 12 loose tibial implants. In eight cases both components were unstable. Plain radiography had a sensitivity of 77% for loosening of the femoral and 83% for t

Detection and staging of chondromalacia patellae: relative efficacies of conventional MR imaging, MR arthrography, and CT arthrography.

OBJECTIVE: Chondromalacia patellae is a condition characterized by softening, fraying, and ulceration of patellar articular cartilage. We compare the sensitivity, specificity, and accuracy of conventional MR imaging, MR arthrography, and CT arthrography in detecting and staging this abnormality. SUBJECTS AND METHODS: Twenty-seven patients with pain in the anterior part of the knee were prospectively examined with MR imaging, including T1-weighted (650/16), proton density-weighted (2000/20), T2-weighted (2000/80), and spoiled two-dimensional gradient-recalled acquisition in the steady state (S

Magnetic resonance arthrography in the assessment of anterior instability of the shoulder: comparison with double-contrast computed tomography arthrography.

Forty-seven shoulders with traumatic anterior instability were studied by magnetic resonance arthrography (MRA) and computed tomography arthrography (CTA) to compare the diagnostic performance of these examinations in the evaluation of Bankart lesions. All shoulders were examined by arthroscopy to verify the lesions. Labral damage evaluated by MRA and by CTA correlated significantly with arthroscopic findings (MRA, r = 0.55, p < 0.0001; CTA, r = 0.45, p = 0.0050). MRA possessed higher sensitivity in detecting torn labra (MRA, sensitivity = 87%, specificity = 75%; CTA, sensitivity = 33%, specif

Arthrographis Infections in Humans-A Narrative Review.

BACKGROUND: Arthrographis spp. are environmental fungi commonly found in soil and compost. Infections caused by Arthrographis species remain an uncommon clinical occurrence. Although these infections are infrequent in the general population, their incidence appears to be elevated among immunocompromised patients or those with significant comorbidities. OBJECTIVES: This review seeks to examine all documented human cases of Arthrographis spp. infections, with particular focus on aspects such as epidemiology, microbiological features, resistance patterns, therapeutic approaches and associated mo

Anaesthetic arthrography of the shoulder joint in dogs.

OBJECTIVE: To evaluate the use of intra-articular anaesthesia combined with positive contrast arthrography (collectively called 'anaesthetic arthrography' [AA]) in the shoulder in order to identify and confirm the source of pain in lame dogs. METHODS: Anaesthetic arthrography was performed in 30 dogs with shoulder joint lameness with a mixture of contrast medium (iohexol: 240 mg I/ml) and a local anaesthetic (mepivacaine 2%: 20 mg/ml). The effect of intra-articular anaesthesia was evaluated by an objectified visual scoring system and the arthrograms were evaluated for their diagnostic value.

Correction of equinus in clubfoot: the contribution of arthrography.

The equinus deformity in clubfoot is twofold: talocrural and talocalcaneal. No study has been carried out on the link between these two elements and whether they are mutually dependent or independent. This has led to an unvarying operative strategy confined to opening the talocrural capsule and section of the talofibular ligament. To assess the extent to which these procedures are warranted, the authors performed perioperative talocrural arthrography, taking radiographs before and after each stage of operative treatment. In all cases, talocrural equinus seemed to disappear without the need for

Current concepts of MR arthrography of the hip.

MR arthrography is a recent addition to the imaging evaluation of the hip. The combination of joint distention and multiplanar imaging provides a detailed assessment of the intraarticular structures, most importantly the acetabular labrum. An acetabular labral tear is a potential cause of hip pain in patients with an otherwise normal joint, or patients with underlying pathology such as developmental dysplasia. A summary of the current concepts regarding identification of labral degeneration, labral tears and detachments, and potential pitfalls is presented in this article.

MR arthrography of the shoulder: variants and pitfalls.

Use of magnetic resonance arthrography to evaluate pathologic conditions of the shoulder is becoming widespread. However, normal anatomy or anatomic variations can cause interpretive errors. The most common variations occur at the origins of the glenohumeral ligaments (GHLs) and the insertion of the joint capsule. Among the GHL variants, common origin of the superior and middle ligaments is the most frequent followed by thinning, thickening, or absence of a ligament, most often the middle one. Absence or thinning of one ligament is sometimes associated with thickening of another or changes in

The role of computerized tomography in shoulder arthrography.

Double contrast computerized tomographic (CT) shoulder arthrograms of 55 shoulders were reviewed. This investigation was compared with conventional arthrograms and the findings were correlated with subsequent clinical and surgical follow-up. CT arthrograms were found to be useful in demonstrating bony, ligamentous and cartilaginous structures and have helped the surgeon in deciding the approach and type of surgical procedure. Indications for CT arthrography are recurrent dislocation, multidirectional instability, severe rheumatoid arthritis and other arthritides, complex shoulder deformities a

Arthrography in the evaluation of congenital dislocation of the hip.

To assess the value of intraoperative arthrography, 48 preoperative hip arthrograms of 35 children under 18 months of age treated for congenital dysplasia of the hip were reviewed. The frequency of the common arthrographic observations was assessed and was compared with the findings observed from the plain roentgenograms, and with the pathoanatomy seen at the time of open reduction. Arthrography proved reliable in identifying the depth of reduction (medial dye column, 100%), the configuration of the osteocartilaginous structures (96%), and the presence of potentially obstructing soft tissue (l

Computed arthrotomography as an adjunct to pediatric arthrography.

Computed arthrotomography was compared with conventional arthrography in ten cases and with both pluridirectional arthrotomography and conventional arthrography in five cases. Six elbows, five hips, three ankles, and one wrist were examined. Indications included pain, locking, congenital hip dislocation, posttraumatic joint deformity, and postinfectious joint deformity. Compared with conventional arthrography, computed arthrotomography provided additional information in ten of 15 (67%) cases and provided the same information in four cases. Arthrography and computed arthrotomography were comple

[Double contrast arthrography of the shoulder joint].

The shoulder joint consists of the soft as well as the hard components. Therefore, the shoulder arthrography is a very important diagnostic tool. Although both positive and negative contrast arthrography have been critically studied, there has been little literature on the double contrast arthrography. The purpose of this paper is to determine the standard technique of the double contrast arthrography, to describe arthrographic findings and to assess its clinical effectiveness of this method. Firstly, the author macroscopically examined the anatomical specimens and molded the interior of the j

[Arthrography in pathology of the hip in older children].

Arthrography of the hip was performed in outpatients on 25 children aged 5 to 14 years, using a supra-trochanteric latero-external puncture approach. Data obtained supplied anatomical details indispensable for the decision to operate or not, particularly in certain congenital dislocations or in Legg-Perthes-Calvé disease. The examination findings were useful not only for analyzing articular cartilages, limbus and chondro-articular surfaces in abduction and adduction but also for evaluating anteversion of bony and cartilaginous acetabula using an axial projection. The scanner provides comparabl
