Radial versus femoral approach for high-speed rotational atherectomy.

OBJECTIVE: The aim of this study was to compare in-hospital outcomes for patients undergoing high-speed rotational atherectomy (HSRA) via the radial and femoral route. BACKGROUND: Transradial access is often avoided for HSRA because of concern over limitations on guide catheter size. No studies have compared a radial and femoral approach for HSRA. METHODS: This is a retrospective comparison of in-hospital outcomes for 151 consecutive patients (75 radial, 76 femoral) undergoing HSRA. RESULTS: Significantly smaller diameter guide catheters (6.3 +/- 0.5 Fr vs. 7.1 +/- 0.8 Fr, P < 0.001) and bu

Arteriovenous fistula complicating directional atherectomy of the popliteal artery.

We describe a case of a 70-year-old man with a critical popliteal artery stenosis treated by directional atherectomy and complicated by a huge arteriovenous fistula. A 70-year-old man, previously treated by bilateral femoropopliteal bypass, was admitted to our hospital because of right leg intermittens claudication. A critical infrapopliteal stenosis was documented; this lesion was treated by directional atherectomy, which was complicated by a huge arteriovenous fistula. Due to the failure of any conservative approach, surgical repair was necessary. Following surgery, the patient was discharge

Impact of Rotational Atherectomy on Endothelial Integrity and Platelet Activation.

Rotational atherectomy (RA) is an established technique for modifying heavily calcified and fibrotic coronary artery lesions. Despite its efficacy, the use of a high-speed rotating burr can provoke platelet activation and endothelial injury, thereby increasing thrombotic risk, promoting inflammation, and impairing vascular healing. This study investigated the effects of RA and its procedural characteristics on endothelial function and platelet activation by assessing circulating biomarkers. We prospectively analyzed 34 patients undergoing elective RA at a tertiary center. Blood samples were ob

Systematic review of atherectomy of inguinal arteries for atherosclerotic lesions.

INTRODUCTION: Surgical endarterectomy represents the gold standard for the treatment of atherosclerotic lesions of the groin vessels. Endovascular treatment such as atherectomy with or without drug coated angioplasty (DCA) of the so called no stenting zones, i.e. inguinal and popliteal vessels, is gaining growing acceptance as alternative option to open surgery. This systematic review aims to scrutiny the current clinical evidence regarding atherectomy and DCA for the common artery (CFA). EVIDENCE ACQUISITION: We conducted an exhaustive research in multiple platforms (Medline, PubMed, Cochran

A case report of fractured guidewire removal by rotational atherectomy.

BACKGROUND: Fractures occur in association with manipulation and because of the complexity of the coronary artery, and they can cause a series of serious complications, such as myocardial infarction and secondary thrombosis. Common treatments for fractured guidewires include conservative, interventional and surgical methods. CASE PRESENTATIONS: A 67-year-old male was admitted to our institute. He had recurrent chest tightness and chest pain for half a month, which worsened in one day. He was diagnosed with acute non-ST-segment elevation myocardial infarction. Guidewire fracture was caused by

Directional coronary atherectomy for the treatment of acute myocardial infarction.

Directional coronary atherectomy (DCA) was performed after intracoronary thrombolysis in 32 patients with a first acute myocardial infarction. DCA was successful in 31 (97%) of 32 patients. Abrupt closure of the treated segment occurred in one patient but was managed successfully by conventional balloon angioplasty. Repeat angiography was performed in 32 patients before discharge (2.7 +/- 0.7 weeks later) and in 29 patients during the follow-up (4.5 +/- 1.5 months later). No restenosis (stenosis > 50%) occurred before discharge; however restenosis occurred in 12 (41%) of 29 patients during fol

Directional atherectomy with the Omnicath: a unique new catheter system.

Omnicath is a directional atherectomy catheter that employs deflecting nontraumatizing wires to anchor the cutting window at the atherectomy site. This anchoring system regulates the depth of cut and provides directional control and distal perfusion. The system continuously removes debris through a suction port from the operative site. To demonstrate the performance of the device, the Omnicath was tested in the external iliac arteries of ten atherosclerotic Hanford miniature swine in which concentric and eccentric lesions were induced. Five animals were sacrificed 3 days after atherectomy; the

Atherectomy With Multimodal Embolic Protection for Severely Calcified Femoropopliteal Lesions.

Severe calcification in femoropopliteal lesions remains a clinical challenge to achieve both the technical and long-term success in endovascular therapy. Although atherectomy is an effective treatment for calcified lesions, it is associated with the risk of distal embolization, even with the use of embolic protection devices (EPDs), and currently, no definitive strategy has been established to prevent this complication. We present a case successfully treated with atherectomy under EPDs and Parodi Anti-Embolism System (PAES). We report the case of a 77-year-old man with intermittent claudicatio

Rotational atherectomy to left circumflex ostial lesions: tips and tricks.

Serious complications including vessel perforation may occur during rotational atherectomy (RA) to left circumflex (LCX) ostial lesions. In fact, if perforation occurs around LCX ostium, bailout procedures including deployment of covered stents may cause fatal ischemia in the territory of left anterior descending artery, which results in broad anterior acute myocardial infarction and subsequent death. In this review article, we described tips and tricks for RA to LCX ostial lesions. First, we should cautiously decide the indication for RA to LCX ostial lesions, because there are several reason

Sex differences in procedural and clinical outcomes following rotational atherectomy.

AIM: Evaluate sex differences in procedural net adverse clinical events and long-term outcomes following rotational atherectomy (RA). METHODS AND RESULTS: From August 2010 to 2016, 765 consecutive patients undergoing RA PCI were followed up for a median of 4.7 years. 285 (37%) of subjects were female. Women were older (mean 76 years vs. 72 years; p < .001) and had more urgent procedures (64.6 vs. 47.3%; p < .001). Females received fewer radial procedures (75.1 vs. 85.1%; p < .001) and less intravascular imaging guidance (16.8 vs. 25.0%; p = .008). After propensity score adjustment, the primar

Angioscopy-Guided Excimer Laser Atherectomy in Femoropopliteal In-Stent Occlusions.

BACKGROUND: In-stent occlusions in femoropopliteal lesions (FP-ISOs) remain a significant clinical issue. Excimer laser atherectomy (ELA) can ablate tissue, including thrombi, and may therefore be effective in FP-ISOs, which frequently contain both thrombi and neointimal hyperplasia. Angioscopy can directly visualize the stent lumen, making it useful for distinguishing thrombi from neointimal hyperplasia. Here, we investigated whether angioscopy-guided ELA was useful for FP-ISOs. METHODS AND RESULTS: We studied 10 consecutive patients with FP-ISO who underwent endovascular therapy (EVT) with
