[Standard of transesophageal atrial pacing to diagnose dual atrioventricular node pathway].

In order to assess the diagnostic methods of dual atrioventricular node pathway (DAVNP), we performed transesophageal atrial pacing in 58 patients with palpitation. These patients were classified into two groups, group A comprising 40 patients without broken A-V conduction curve during pacing, compared with group B of 18 patients with broken A-V conduction curve. In our study, both atypical Wenckebach cycle and 3:2 A-V conduction during the increment atrial pacing (IAP) had no significant difference between the two groups (P greater than 0.05). The maximum increments of SR interval in Wenckeba

Association between nonreentrant supraventricular tachycardia and atrioventricular node reentrant tachycardia: a presentation of dual AV node physiology.

Persistent simultaneous conduction of P waves over a fast and a slow nodal pathway defines the nonreentrant type of supraventricular tachycardia, usually not associated with reciprocating movements. We report a unique association between this uncommon tachycardia and a usual AV nodal reentrant tachycardia, made possible by the existence of three different nodal pathways.

Atrioventricular block in the atypical form of junctional reciprocating tachycardia: evidence supporting the atrioventricular node as the site of reentry.

Serial electrophysiologic studies were performed in 19 patients with the atypical form of supraventricular tachycardia having a long RP and short PR interval. In all 19 patients, supraventricular tachycardia was found to have a 1:1 P-QRS relation during initial control electrophysiologic studies, and in all 19 patients, electrophysiologic studies suggested that junctional reentry was the mechanism of supraventricular tachycardia. Seven of the 19 patients developed atrioventricular (AV) block during initiation of supraventricular tachycardia or after induction of supraventricular tachycardia fo

Treating Incessant Tachyarrhythmia-Induced Cardiogenic Shock With Emergent Atrioventricular Node Ablation.

BACKGROUND: Tachycardia-induced cardiogenic shock (TICS) is a life-threatening condition requiring prompt intervention. CASE SUMMARY: An elderly patient with chronic atrial fibrillation and multiple comorbidities developed refractory cardiogenic shock secondary to rapid ventricular rate. Initial management with antiarrhythmics, electrical cardioversion, and vasopressors failed to restore hemodynamic stability. Emergent atrioventricular node ablation (AVNA) with temporary biventricular pacemaker implantation was performed, resulting in immediate heart rate control and subsequent hemodynamic st

The HAV pattern in pediatric patients with atrioventricular node reentrant tachycardia.

OBJECTIVES: The purpose of this study is to assess the prevalence of a His-Atrial-Ventricular (HAV) pattern, i.e. the atrial electrogram following the His bundle -HB- electrogram and preceding the ventricular one, on the catheter placed in the His position in pediatric patients during typical atrioventricular node reentry (AVNRT). MATERIALS AND METHODS: The pediatric electrophysiology databases of two separate institutions were queried for patients with a diagnosis of AVNRT. Demographic, clinical data and the electrophysiology study (EPS) information were assessed. RESULTS: Thirty-nine conse

Utility of mapping signals to improve precision of atrioventricular node ablation.

BACKGROUND: Atrioventricular node (AVN) ablation is effective for rate control in atrial fibrillation. This may require multiple radiofrequency applications to achieve complete atrioventricular block (CAB). In this retrospective study, we tested the hypothesis that mapping the AVN utilizing electrograms (EGMs) on both proximal and distal bipoles of the mapping catheter may improve the likelihood of CAB. METHODS: Lesion characteristics and EGM components on the proximal and distal bipoles of the ablation catheter in first-time AVN ablation procedures were analyzed. Outcomes of each lesion, inc

Cystic tumour of the atrioventricular node: three cases of sudden death.

Cystic tumour of the atrioventricular nodal region is a rare primary cardiac tumour that can cause sudden death. Antemortem diagnosis and successful excision of this type of tumour are extremely rare. Three cases of sudden death are reported: A 25- and 40-year-old with a history of heart block and a 60-year-old with no medical history. There have been more than 120 cases of sudden death attributed to primary cardiac tumours in the literature. Although over 100 of these lesions were histologically benign, their intracardiac locations precipitated conductive and haemodynamic abnormalities that r

Carbamazepine-induced sinus node dysfunction and atrioventricular block in elderly women.

We report on four elderly women in whom carbamazepine was suspected of inducing sinus node dysfunction (3 patients) and atrioventricular block (1 patient). Patients were treated with carbamazepine, 200 to 600 mg a day, for trigeminal neuralgia (n = 3) or epilepsy (n = 1). After 1 to 16 months of carbamazepine therapy, these patients were admitted to our emergency room because of bradyarrhythmia. Their conduction disturbances on electrocardiographic monitoring disappeared immediately after the cessation of carbamazepine intake. Provocation tests were performed on three patients. Because of rena

Regulation by acetylcholine of Ca2+ current in rabbit atrioventricular node cells.

Effects of acetylcholine (ACh) on L-type Ca2+ current (ICa) were examined in isolated atrioventricular (AV) node cells exhibiting spontaneous contractions and pacemaker current (If). ACh at a saturating concentration of 10 microM reduced basal ICa by 48 +/- 6%. The ACh effect was abolished by dialysis with 8-bromoadenosine 3',5'-cyclic monophosphate (8-BrcAMP), an adenosine 3',5'-cyclic monophosphate (cAMP)-dependent protein kinase inhibitor, or guanosine-5'-O-(2-thiodiphosphate). Dialysis with guanosine 3',5'-cyclic monophosphate (cGMP) or NG-monomethyl-L-arginine (L-NMMA) and application of

Transvenous catheter ice mapping and cryoablation of the atrioventricular node in dogs.

While radiofrequency catheter ablation is very effective, it does not allow for prediction of success prior to full delivery of the energy. We investigated the use of cryoablation using a new catheter on the AV node to determine (1) if a successful site might be identified prior to the ablation itself, and (2) the parameters of cryoablation of the AV node using a new cryocatheter. In eight dogs, the cryoablation catheter was advanced to the AV node to produce transient high degree AV block by lowering the temperature to a minimum of -40 degrees C (ice mapping). Transient high degree AV node bl

Intermittent preexcitation and mesothelioma of the atrioventricular node: a hitherto undescribed entity.

This is the first documented case of spontaneous intermittent preexcitation associated with mesothelioma of the atrioventricular (AV) node. A 66-year-old male with recurrent atrial arrhythmias, palpitations, heart failure, and marked intra-atrial conduction defect that required a pacemaker died during sleep. Electrophysiologic study revealed left free-wall bypass tract with marked intra-atrial conduction defect and prolonged conduction across the bypass tract. With atrial pacing, high degrees of block were noted in the bypass tract. Serial section of the conduction system and both AV rims reve

Atrioventricular nodal reentrant tachycardia onset, sustainability, and spontaneous termination in rabbit atrioventricular node model with autonomic nervous system control.

Atrioventricular nodal reentrant tachycardia (AVNRT) is one of the most common types of paroxysmal supraventricular tachycardia. The activity of the autonomic nervous system (ANS) is known to influence episodes of AVNRT, yet the precise mechanisms underlying this effect remain incompletely understood. In this study, we update our compact multifunctional model of the rabbit atrioventricular (AV) node with ANS control to simulate AVNRT. The refractoriness of the model cells is adjusted by a specific ANS coefficient, which impacts the effective refractory periods, conduction delays, and intrinsic
