Blood urea nitrogen-guided protein dosage adjustment helps reduce azotemia and functional prognosis deterioration induced by high protein intake in neurocritical patients: A retrospective cohort study.

The optimal protein dosage for neurocritical patients remains uncertain. We hypothesized that higher protein dosage increases azotemia risk and worsens prognosis, and that baseline BUN can guide protein dosage adjustments. In this single-center, retrospective cohort study, we included 327 neurocritical patients aged 18 or older admitted between July 2018 and June 2023 (BMI: 18.5-30 kg/m2). Daily protein intake from all sources was calculated. The primary outcome was azotemia incidence, and secondary outcomes included 30-day functional prognosis (mRS >3). Patients were categorized by protein in

Renal and metabolic complications of undifferentiated and lymphoblastic lymphomas.

The early metabolic events in 33 patients with non-Hodgkin lymphoma were analyzed in the present study. Twenty-three patients had Burkitt lymphoma, 3 had non-Burkitt undifferentiated lymphoma and 7 had lymphoblastic lymphoma. Eight patients developed azotemia prior to starting chemotherapy while five did so during the first treatment week. All the patients but two who developed azotemia had stage C or D disease. Serum LDH prior to chemotherapy correlated well with the stage of disease and predicted the serum levels of creatinine, uric acid and phosphorus in the post-treatment period. Surgical

[Metabolic processes in children with chronic kidney failure as affected by protein-limited diets].

The time course of a number of characteristics of protein and mineral metabolism was studied in 55 children suffering from renal diseases in the stage of chronic renal insufficiency. Use was made of the diets in which protein was restricted to a varying degree relatively to azotemia severity. It was shown that the diet with protein restriction up to 1.5-1.0 g/kg bw has a pronounced antiazotemic action. Therefore, it is desirable to administer it to patients with moderate and remarkable azotemia. In stable and pronounced azotemia, it is advisable to administer the diet with a sharp restriction

University of Miami Division of Clinical Pharmacology Therapeutic Rounds: ischemic renal disease.

Ischemic renal disease (IRD) is defined as a significant reduction in glomerular filtration rate and/or loss of renal parenchyma caused by hemodynamically significant renal artery stenosis. IRD is a common and often overlooked clinical entity that presents in the setting of extrarenal arteriosclerotic vascular disease in older individuals with azotemia. IRD is an important cause of chronic renal failure and end-stage renal disease (ESRD), and many patients with a presumed diagnosis of hypertensive nephrosclerosis may actually have undiagnosed ischemic nephropathy as the cause of their ESRD. Treatment of acute kidney injury secondary to oak intoxication with hemodialysis in a miniature zebu.

OBJECTIVE: To describe the successful management of acute kidney injury (AKI) secondary to oak intoxication using hemodialysis in a miniature zebu. CASE SUMMARY: A 1-year-old male intact miniature zebu was presented for evaluation of a 2-day history of lethargy, anorexia, decreased urine production, and dry, firm stool. Initial diagnostics revealed severe azotemia and the bull was presumptively diagnosed with AKI secondary to oak intoxication due to free access to oak trees in the pasture. Despite conventional management with IV fluids and supportive therapy, the degree of azotemia progressiv

Successful hemodialysis treatment of a Quarter Horse mare with silver maple leaf toxicity and acute kidney injury.

An adult American Quarter Horse mare presented for pigmenturia and lethargy of 12 hours' duration and was diagnosed with silver maple leaf toxicity. The mare had intravascular hemolysis and azotemia. The mare was treated with a transfusion of whole blood, fluids administered IV, antibiotics, oxygen insufflation, and supportive care. The azotemia persisted despite conventional medical management and hemodialysis was elected. After 2 intermittent hemodialysis treatments over 3 days, the azotemia almost resolved, clinical signs improved, and the mare was discharged. The blood urea nitrogen, creat

Fibroblast growth factor 23 (FGF-23) concentrations in cats with early nonazotemic chronic kidney disease (CKD) and in healthy geriatric cats.

BACKGROUND: Fibroblast growth factor (FGF-23) has an important role in phosphate regulation. Its clinical relevance in cats with CKD has not been explored previously. HYPOTHESIS/OBJECTIVES: The study objectives were (1) to determine whether FGF-23 concentrations are increased in nonazotemic cats, cats which developed azotemia within 12 months of screening compared with cats that remained non-azotemic, and (2) to evaluate the relationships between FGF-23 and PTH and FGF-23 and glomerular filtration rate (GFR). ANIMALS: Sixty-two healthy client-owned geriatric cats, 14 of which developed azote

Primary renal disease in a dog.

A 7-month-old Lhasa Apso with a history of polydipsia and vomiting was depressed, thin and dehydrated. Serum chemistry assays revealed hyperphosphatemia and azotemia, and urinalysis revealed isosthenuria, suggesting azotemia of renal origin. Antemortem renal biopsy specimens contained several sclerotic glomeruli, a few embryonic renal tubules and interstitial fibrosis, indicating renal dysplasia.

[Serum uric acid level and fractional excretion of urate in fluid and electrolyte disturbances].

Changes in renal urate clearance may reflect altered glomerular dynamics more precisely than other commonly used indices. Extracellular fluid volume expansion is known to be correlated with uricosuria, while volume depletion is associated with decreased urate excretion. The diagnostic usefulness of measuring serum urate levels and fractional excretions of urate in SIADH, incipient diabetic nephropathy, and prerenal azotemia have been reviewed. In SIADH, profound hypouricemia and markedly increased fractional excretion of urate (FEUA) accompany the extracellular fluid volume expansion. In prere

Bilateral ureteral obstruction following aortic bypass surgery.

A patient developed azotemia from bilateral ureteral obstruction due to retroperitoneal fibrosis after placement of an aortofemoral bypass graft for atherosclerosis in that region. This complication of abdominal vascular prosthesis may be more common than is presently recognized; especially since the patient may remain asymptomatic till an advanced degree to azotemia supervenes. At that time irreversible damage to the kidney may occur. Furthermore, silent damage in the kidneys may be attributed to other causes in these patients who may also have severe vascular disease or prostatic disease. Wh

Modification of disease progression in rats with inherited polycystic kidney disease.

The most common inherited form of human polycystic kidney disease (PKD), autosomal dominant PKD (ADPKD), is a leading cause of chronic renal failure, but has a variable clinical presentation, with end-stage renal disease occurring in only 25% to 75%. Several findings are consistent with the idea that factors in addition to the primary mutation can affect the progression of cystic change and chronic renal failure in PKD. Epithelial cell proliferation is a central element in the pathogenesis of renal cysts. We postulated that the superimposition of a growth-promoting stimulus might promote more

HYPERCALCEMIA AND METASTATIC MINERALIZATION INVOLVING FOOTPADS IN ROCK HYRAXES (PROCAVIA CAPENSIS).

Three rock hyraxes (Procavia capensis) maintained in a zoological collection had chronic hypercalcemia and azotemia. In addition, all animals displayed signs of lameness due to footpad lesions that were histologically characterized as marked metastatic mineralization with granulomatous inflammation, reminiscent of calcinosis circumscripta. Although the animals were managed with aggressive fluid therapy, calciuresis, and dietary modification, all were eventually humanely euthanized due to the severity of their footpad lesions and/or progression of renal disease. Metastatic mineralization was al
